There’s so much I love about this but first I was selfishly admit that I love that you didn’t assume I was assigned Emergency Medicine as my specialty. I wasn’t. This was my top choice, along with Pediatrics and R&O. So thank you for stroking my well-maintained ego :)

Alright. So there’s a few reasons I chose Emergency Medicine. In sharing these I’ll try to show what made a difference for me and maybe that’ll help you to sort out some thoughts. Mainly though, this is a hugely personal decision/journey…and like you said, only some of it is in your control so I think you should a) have a plan and b) make sure that plan is flexible.

1. I wanted to become a doctor because of pikuach nefesh, or the Jewish belief that saving someone’s life will always take priority. G*d will not be offended if you break Shabbat to save someone’s life - it’s a requirement. Why do *you* want to become a doctor?

2. Being a Jew (I promise I don’t plan for everything to come back to this but I’m a big Jew so for me it often does - sue me) I thought that was the coolest shit. For me, technically, pikuach nefesh also excuses a doctor sometimes needing to work on Shabbat. Listen. I’m the daughter of immigrants and that pressure to be constantly working is a very real thing. At the same time, again - big Jew, so I wanted to make sure I was being as efficient with my life-saving time as possible. It just made sense that my specialty check all of those boxes: aka works with my lifestyle. Is there anything in your life that you think might make one specialty more desirable over another? Sure, we all want to cure cancer but at the end of the day this is a profession and you’ll do your best when the specialty is the right fit.

3. I was a combat medic. In a war zone. I did everything from desk-based vaccination rollouts to pulling soldiers from burning cars and administering aid in dust storms. I thrived in that world. I am very, very cool under pressure. So for me, Emergency Medicine was a no-brainer. One of our other doctors was a nurse in the army and we have that in common. The ED can feel like war - during the pandemic it was war…some days it still feels like it is. There’s also a running joke in my life that I have unreal amounts of energy. I was blessed/work hard to cultivate a clear mind and healthy body…all on very little sleep. There’s also a joke amongst doctors that the staff in the ED are a bunch of fitness freaks with too much energy…we are. We have to be. What’s the thing about you that you can bring to your specialty? What background/trait do you have that makes you unique and desirable in that field?

4. Sometimes picking fields you don’t want is a great place to start. Like OBGYN? I’d rather die. Also wasn’t much for PhysMed or ophthalmology. So that helps too. What specialties can you easily push to the bottom of the list and pray you don’t get matched on?

5. This kind of relates to #3 but I’m mentioning it in a more cynical/realistic way. Like I said, this is a profession. It’s sadly not meant to be our lives. This means that some people choose specialty based on the life they want to have - aka I’m 31 and am not yet married and do not yet have children because (pandemic aside) those things are very difficult for me to do with my current work schedule. That might have been easier if I did choose Pediatrics..or really so many other specialties aside from EM. Is there anything that might hold you back in a specialty? Ie. *what specialty can you die with without resenting it first?* Or, the nicer way of putting it: What specialty can you remain married to? You may dislike it some days, sometimes you’ll want to call it quits, but at the end of the day it’s the best way to spend your life and it helps you be the best doctor you can be - which specialty is that? My aunt (also a doctor) posed this to me once. I thought it was a great way to look at things. And this is 100% how I feel about Emergency Medicine.

Basically:

1. Do what you can but remember that you’ve worked your butt off and now, yes, your hard work is being put into a database with everyone else’s hard work and it may not play out how you planned. And that’s okay! Trust the process! (There’s not much you can do anyway - sorry)

2. You will kill it in any specialty. I’m serious. You’ve done your rotations. You made it through med school! You are so capable to do anything!!

My final rotation of the year! It seems like yesterday that I was following a hand drawn map of the hospital to locate the ophthalmology room for my very first rotation. How far I’ve come!

On this rotation we began with a small group of four students (rather than the usual eight) which was then divided in half, with two students beginning on soft tissue and the other two on orthopaedics. I was assigned to soft tissue surgery for the first week and it was hectic! My buddy and I were run off our feet trying to complete the work of four students. We arrived at 7:30 each morning and didn’t leave until after 7:00 at night when our patient records were completed. Once home, our evenings were spent frantically researching surgical procedures to avoid looking like complete idiots when the specialists inevitably quizzed us the following day.

Students were assigned to cases and responsible for collecting a history during the initial consultation with the owner, performing a physical examination, scrubbing into the surgery, writing a detailed surgical report, looking after the patient in hospital, administering medications, overseeing wound care, recording vitals and the progress of recovery, and eventually discharging the patient. Our ultimate goal was to get our patients through all of those stages and discharged as quickly as possible, to minimise the number of animals in our care and allow us to leave the hospital at a semi-reasonable hour each day.

Although being a group of just two students meant that we had an insane workload to keep on top of, we did get special treatment in the form of being allowed to scrub into almost every surgery! Students in previous groups were lucky to scrub into five surgeries during the whole rotation, whereas I scrubbed into ten just in the first week! Even so, being specialist surgery, our job primarily consisted of passing surgical instruments and cutting suture material (which was never quite the right length). One day towards the end of the first week, the surgeon surprised me by letting me place two sutures: a simple interrupted and a cruciate. That was two more than my fellow student, so I counted myself lucky!

During the first week, I was involved with a huge variety of soft tissue cases (prepare for big words) including an abdominal hernia repair, ventral bulla osteotomy, two dermoid sinus removals, multiple wound repairs, adrenalectomy, thyroidectomy, melanoma removal and skin flap, tongue biopsy, emergency plication to correct an intussusception, gastropexy, and ovariohysterectomy.

The most memorable case from this rotation was a soft tissue sarcoma removal from the hind leg of an elderly Golden Retriever. The surgery was performed on Monday and I arrived early the following day to find her leg very swollen. Over the week, the leg continued to swell and her condition steadily declined. By Thursday her breathing was laboured and I could hardly hear a heart beat. Our patient was transferred to the ICU to spend the night in an oxygen tank. The following morning she was much the same, still struggling for each breath. The vets had tentatively diagnosed her with von Willebrand Disease, an inherited clotting disorder caused by a defective or deficient protein. This meant the swelling in her leg was likely pooled blood as a result of uncontrolled bleeding from the surgical site. The disorder had never been detected previously and so it was an incredibly unfortunate and unforeseen complication. On Friday evening I went to check on her before heading home and reached the ICU just as someone yelled, “SHE’S ARRESTED!”. The emergency team sprung into action and began CPR. Her owners were contacted and the decision was made to let her go. It was a devastating end to what should have been a simple mass removal. Everyone involved was deeply affected by her death.

At the end of the first week, the resident came to see us in the student tutorial room. He told us we had done a fantastic job and he really appreciated our help. The hard work of final year students is often taken for granted, so the few times people have acknowledged and appreciated my efforts have really stuck with me!

Just as we were beginning to feel comfortable with soft tissue surgery, Monday came around and it was time to switch to orthopaedics. New surgeries, new patients, new team. At least I still had my student buddy for support and entertainment. There was an interesting mix of cases on orthopaedics, including bilateral hip dysplasia, intervertebral disc disease, two medial patellar luxations, shoulder arthroscopy, stifle arthroscopy and joint tap, and many tibial plateau levelling osteotomies.

Over the two week rotation, we had several tutorials on wound management, brachycephalic airway syndrome, neurology and fracture management. On the last Friday we had a short exam, followed by an orthopaedic cadaver lab, where we practiced our surgical approaches to the hip and stifle joints, and performed a femoral head ostectomy (a procedure in which the head of the femur is cut off to remove the hip joint).

The last surgery finished late on Friday and the hospital was eerily quiet. It was the strangest feeling saving my final reports, packing up my belongings and preparing to leave the hospital for the last time. The four of us didn’t really know how to react. We congratulated one another on finishing and headed home in stunned disbelief, unsure whether to laugh or cry. We didn’t have much time to process these feelings before our minds became consumed with panicked thoughts of the impending exams. It was time to put our heads down and bums up for one final push to the finish line!

According to Beth Kimmitt, DVM, resident of ophthalmology at Purdue University’s College of Veterinary Medicine in West Lafayette, Indiana, a dog’s eye is always producing tears, and these tears typically drain at the corners of the eye. “Technically, a normal eye should not have any ocular discharge, but a small amount of clear discharge may be OK,” she tells us. Clear dog eye discharge might look brown and slightly crusty when it dries.

That said, some dog eye discharge is not normal. Read on to find out what’s normal and what needs a vet exam when it comes to dog eye discharge:

If your dog has colored green eye discharge, yellow eye discharge or another colored eye discharge, schedule a vet appointment immediately. Other signs of problematic dog eye discharge include squinting, a red-looking eye, or if your dog is rubbing or pawing at his eye. If you think something is wrong with your dog’s eye, don’t wait too long to make that vet appointment — his eyesight could be at risk.

Abnormal eye leakage might signal a dog eye infection or other issues.

“The presence of ocular discharge is a non-specific sign,” Dr. Kimmitt says. “This means that it can be caused by a variety of ocular disorders. Common causes of dog eye discharge include ulcers, entropion, keratoconjunctivitis sicca and conjunctivitis (allergic or bacterial).” A corneal ulcer is damage to the cornea, which is the clear membrane that covers the iris and pupil.

Entropion is a condition in which the eyelid rolls inward, causing eyelashes to rub against the cornea. Keratoconjunctivitis sicca (called dry eye) is dryness of the conjunctiva, which is membrane that covers the white part of the eye. Conjunctivitis (sometimes called pink eye) is inflammation of the eye. A vet exam and tests can pinpoint the cause of your dog’s abnormal eye discharge.

Certain breeds are prone to dog eye discharge.

You can help minimize dog tear stains by wiping the under-eye area frequently and keeping it as dry as possible. You can also try one of the whitening products sold specifically to help with tear stains.

It’s important to keep your dog’s eye area clean.

“A soft, wet cloth can be used to gently wipe away the discharge,” Dr. Kimmitt advises. You can also use a veterinary eye cleaning product to combat dog eye discharge — just make sure it doesn’t contain any alcohol.

An assignment diary...

I’m a freelance Photojournalist. I do assignment for few national dailies based in Bangladesh. I also do work for some news photo agencies. I start practicing Journalism in 2008 when I was students of higher secondary school. It was UNICEF where I started my journey of journalism. After then I worked for some agencies. My dream as a photojournalist is covering war and conflict. I do cover many conflicts happened in Bangladesh. As I’m a lazy(!) person I upload a little amount of work on my flicker account. I am going to share one of my memorable experiences as a photojournalist. It was 4 August 2008 just 2 days after my 27th birthday. Student of High school in Bangladesh are stand in the road and protest for a better road as well as road safety. In this time I was working for a news agency call Focus Bangla. I was appointed for covering the area of Mohammadpur, Shamoli and Asad Gate from 1st August to 3rd August.

In 4th August situation of this protest is gone critical. I was moved from Asad Gate to City College and Zigatola area. On one side of it were thousands of school and college students. On the other end, a small group of people belonging from front organizations of the ruling government party. Both groups were armed with stones and sticks, chasing each other intermittently. It’s about 2:30Pm. There I was capturing photographs with my Colleagues from different newspapers. Suddenly they started attacking on journalists. Then we journalists are moved on a safe position.

At the time of approximately 3.30; peoples from anti-protest are started gun fire on the Student activists and also journalist. They targeted Journalist who is capturing photographs and taking videos. I was in the middle of the road divider with one of my friend and colleagues Reyad.

Suddenly a bullet from short gun hit in my face. That my eyes what is affected. In that moment everything in front of my eyes are just white. I can’t see anything. That moment when its hits on me I was just thinking that I am going to die and I was impassible.  After few second I realize that I’m alive and I’m trying to escape from this place. I sought out for help. Then a young boy rescues me from that place and took me to emergency department of Popular Hospital.

They give me Fast aid and suggested me that to go a specialized eye hospital. The Injury of my eyes is very critical. I go out from there to find a good consultant for my eyes.

I’m praying and thinking that can I see again properly. My car is reaching at National Institute of Ophthalmology & Hospital and my tension is increasing...

After That

After That I was refused from another three hospital because of they are not agree to took the risk. They told me that it was looks like very deep injury and need to go for an eye hospital. After wasting two hours one of my cousins and two of my friend took me to National Institute of Ophthalmology & Hospital.

It’s almost 5.30 afternoons. They bring me to emergency and do a little surgery on upper side of my right eye. They told me that condition of my eye vision is looks like good. They stitch and do bandage, told me to come next day and consults with senior doctor.

The next day I went hospital and the doctor make some test and assure that the danger is gone and suggest me to take rest and come after 15 days. I was regularly taking medicine but it’s going wrong. I was consults with doctor for 4-5 times but the doctor says that it will be fine. After passing 18th day I decided to change doctor. I go for a private hospital and doctor told me to do a X-Ray and A MRI scan. She assumed that there is something is still inside my eye lid.

I immediately do an X-Ray and find that really something is still exist under my eye lid. Then I go for a MRI scan. I do every formality for MRI. Just couple of second before entering MRI machine I inform the technician that it could be a metal bullet.  As I know it might be very dangerous to having a metal something inside the body while doing MRI. Then they tested that it is a metal something then they suggest me to do a city scan. They I do it.

One of my eye bones was broken. Then doctor told me that who did the 1st surgery was not successfully done. It’s been 22-23 day delayed and my injured eye was having an infection.

Then after one month and four days the bullet was removed from my body doing second surgery and I was perfectly well after few days. I was very lucky that I am alive.

Doctor says that ��Boy you are lucky; you might be blind for the rest of your life. It might take your life. This is a miracle that nothing is happened with you. You have to say a thank you to god.’’
