In 2017 I interviewed Bernadette Wren, then head of psychology at the Tavistock Gids clinic, and asked what effect puberty blocking drugs have on the adolescent brain. Looking highly uncomfortable, she replied that the evidence so far was only anecdotal but that the clinic would study its patients “well into their adult lives so that we can see”.
Even back then, before whistleblowers had exposed the rush to medically transition children, it was alarming to hear that heavy-duty GnRH agonists such as triptorelin — used to treat advanced prostate cancer and “chemically castrate” sex offenders — were being prescribed to arrest puberty in hundreds of children as young as 11.
Moreover, they were being used “off-label” before any clinical trials. And the long-term study Wren promised never materialised: Gids (the Gender Identity Development Service) routinely lost touch with patients, and the 44 it did follow reported little long-term mental health improvement.
This shocking chapter in medical history, where the ideological objectives of trans rights campaigners trumped the welfare of disturbed children, is coming to an end worldwide. The decision by NHS England effectively to ban the prescription of puberty blockers comes after the Cass review noted these drugs could “permanently disrupt” brain development, reduce bone density and lock children into a regime of cross-sex hormones requiring life-long patienthood.
NHS England unites with other national health services including those in Finland, France, Sweden and, most notably, the Netherlands — where the “Dutch protocol”, a regime of early blockers then hormones, was devised in 1998 — in pulling back from prescribing them.
Even in the United States, where a toxic combination of extreme activism and medical capitalism has pushed child gender medicine to grotesque extremes, with double mastectomies performed on 14-year-old girls, there is some retrenchment.
Leaks from the World Professional Association for Transgender Health, the body which formulates guidance on “trans healthcare”, reveal doctors perplexed at how they should explain to an 11-year-old child that drugs will render them infertile. Crucially, liberal media such as The New York Times are now reporting grave medical misgivings about child transition, once dismissed as a culture-war issue for the Republican right.
Yet the question remains: how was this ever allowed to happen? For years, puberty blockers were cheerily billed as a mere “pause button”. In 2014, Dr Polly Carmichael, the last head of Gids before the Cass review ordered its closure, went on CBBC in a show called I Am Leo, saying of blockers: “The good thing is, if you stop the injections, it’s like pressing ‘start’ and the body carries on developing as it would if you hadn’t started.”
The BBC permitted her to make this unevidenced claim to an impressionable audience of six to 12-year-olds. Imagine hearing this as a developing girl, freaked out by your new breasts and periods. No wonder Gids referrals subsequently rocketed.
Carmichael failed to mention that she did not know if pressing “restart” on puberty is always medically possible — it is not — and in fact, almost every child Gids put on blockers went on to irreversible cross-sex hormones.
After years in a Peter Pan state while their peers developed, they understandably felt there was no way back and forged on with treatment. Yet if allowed to experience natural puberty, almost 85 per cent of gender dysphoria cases resolve themselves.
Nor did Carmichael tell CBBC kids that the blockers-hormones combination, if taken early enough, not only results in sterility but kills the libido so that a young person will never experience an orgasm.
At the 2020 judicial review brought by a former Tavistock clinician and Keira Bell, the brave young detransitioner rushed onto hormones by Gids, judges expressed astonishment at Gids’s lack of an evidence base.
Reporting on this issue for seven years, I too have been struck by a complete clinical incuriosity. Not only was data not collected, but those who queried treatments or pressed for evidence faced angry condemnation. Perhaps activists knew what research might find because one long-term Finnish study, recently reported in the BMJ, destroyed the myth used to justify blockers: that a child will commit suicide if denied them.
The Finns found that “gender-affirming care” does not make a dysphoric child less suicidal. Rather, such children had the same suicide risk as others with severe psychiatric issues. In other words, changing bodies does not fix troubled minds.
Yet even after NHS England’s announcement, activists refuse to heed the now-overwhelming evidence. In its response, Stonewall persists with the myth that puberty blockers “give a young person extra time to evaluate their next steps”.
Many questions remain unanswered: will private clinics still be permitted to prescribe puberty blockers; and is Scotland’s Sandyford child gender clinic still determined to close its ears to all evidence? Plus, we have few details on how the NHS’s new “holistic” treatment for gender-questioning children will operate when it opens next month.
This repellent experiment — in which girls who like trucks or little boys who dress as princesses, and who invariably grow up to be gay, are corralled inexorably down a road towards life-changing treatments — belongs in the book of medical disgraces. As do the cheerleaders who raised money for Mermaids and those who persecuted whistleblowers or damned journalists asking questions as transphobic.
In 50 years, chemically freezing the puberty of healthy children with troubled minds will be regarded with the same horrified fascination as lobotomies — which, never forget, won the Portuguese neurologist Antonio Egas Moniz the 1949 Nobel prize.

Monoclonal Antibodies
So I listened to a talk from a drug rep the other day who came into the clinic to promote Cytopoint, the anti-allergy injection for dogs.
It’s a great drug, overall. Super useful for many, literally life changing for a few, and now a mainstay of the allergy treatments we offer. But I had to laugh at something the rep said:
“It’s not really a drug. It’s a bio-mimicry molecule! It’s replicating normal molecules that would be found in the body and just providing more of them!”
And I’m sitting here thinking: that is still totally a drug. A drug is any substance which when given to a living organism produces a physiological effect. 10mg of lokivetmab is most definitely a drug.
But if you are going to argue that Cytopoint is not a drug because it mimics a natural molecule found in the body, then by that logic neither are insulin, thyroxine, cortisone or just about every single hormone.
For goodness sake. I have to wonder what planet marketers live on some days.
We also had a bit of a debate because we’re widely told that Cytopoint shouldn’t have any side effects. Partly this is because monoclonal antibodies aren’t well known in veterinary medicine and they haven’t been around that long.
They’re certainly much safer than the equivalent drugs in other classes, but they’re not ‘side effect free’. They’re a protein, so it’s theoretically possible to be allergic to it. Like the Kesimpta I take it seems likely that some dogs will be ‘off colour’ for a day or so. The most common side effect is being quiet the next say, so it is pretty mild for a typical dog and still worth it. But it’s not zero.
Having a headache for a day is much milder than having acute kidney failure, but I wonder if we’ll get more reports in the future of extremely sporadic weird side effects as we gain more experience with this class of medication.
Medical advancements are wonderful and exciting. But don’t let marketers do all the talking!
And remember to report side effects to the manufacturer.

! Finally a doctor that won't invalidate your experiences or just send you home! kuras would take their symptoms seriously and prescribe medication, etc as needed (i don't know how advanced they are surgery wise but if it was safe and possible...!)
he offers to let them stay at the clinic with him, but they both know he'd prefer they did assuming it isn't too difficult to set up
...methodical? when it comes to checking on mc, especially when they're suffering the worst of their symptoms! even when it seems like he's super busy and doesn't have time, he makes time.
he's good staying calm at signs of pain, strong emotions, etc., but it's a little different because mc isn't Just a patient. sometimes he can't help his worried expression when mc is in obvious pain, and he wouldn't buy tea for just anybody now would he! or let them pull him to the bedside and chat to distract them, comply when asked to hold his hand over their stomach, etc.
food wise.... he learns mc's favourites quickly, at least what they can stomach. best soup flavours, best drinks, soft foods... plus the nearest places he can run to. mc can be like Kuras It's literally 10 minutes away but he's like.. What kind of doctor would i be if i let my patient leave in this state. Can you lie back down now?
when they're home, kuras is freer to spend time with mc if they'd like! if mc says he has to cuddle because they're in pain, he can slip into bed with them. (he might be a little awkward the first couple of times because, well, cuddling isn't a usual treatment he offers to patients is it? and i don't think he's cuddled a lot in general, but he gets the hang of it!)
LEANDER
the wet wick isn't really where you want to be when you're dealing with cramps, nausea and anything else. so when mc mentions that they'll probably be dealing with their period soon and they tell him how bad it can get, leander makes sure to find a more comfortable place, even if it's just for a while. (or you know. maybe they live together somewhere else already and that'd just be very lovely and convenient !)
if there's any Magic Treatments he can do himself to soothe their pain, do some magic equivalent of a heating pad, etc. leander will absolutely use them! though he might make a joke like, Is the spell as good as me though? and when mc is like you know what? yeah. he's like :( / oh i see how it is...after everything we've been through /joking
i don't care if he doesn't like naps he will nap with mc if they ask. He Will. even if he doesn't stay the whole time, he'll at least rub their back and wait for them to fall asleep.
at their beck and call while he's around!! they need help getting to the washroom? he's there to help. they want a little snack? he's bought some! they ask him to run a hot bath? do they want him to help them wash because he will! their heated towels aren't hot enough anymore? no need to get up, leander's already on his way. they want to cuddle with him? What kind of guy would he be to say no?
if mc is more sensitive and their emotions are kind of all over the place, it probably surprises leander the most LOL. like everyone's caught off guard and worried walking in on them crying, but leander is most visibly like wait shit what happened? what's wrong? are you okay? did someone do something? because he's done everything he can to make sure they're comfortable like woah!!😭

do you guys know if vere's job is a 9 to 5 deal... well. while he's on the job, trust me when i tell you he's thinking of mc more often than he'd like to admit. he wishes he could check on them himself, but if he can't, he'll ask one of the others to. you'd think ais considering ais is the only one vere likes, but depending on what you need, he'll have to suck it up and ask. leander maybe?
yeah okay he'll rest his tail over your stomach and let you play with it now stop giving him puppy eyes and asking /lighthearted /silly. he'd appreciate it if you didn't squeeze very tight, like please hold onto something else besides his tail. but staying near you and getting that something isn't at all an issue for him!
if they want to cuddle Vere and not just his tail, he pretends he's Relenting and Allowing it, but he does enjoy it! (you can tell by the smile on his face and the way he shifts the pillows and blanket to make sure they're both comfortable for the foreseeable future.) he lets them rest their head on his chest and listen to his heartbeat, and he wraps his tail around just for extra warmth.

makes sure they're drinking enough water and eating what they can despite his teasing. says shit like "okay, if you drink enough water i'll buy you some sweets" "think of how disappointed princess will be to find out you're pushing yourself" etc etc. of course when he's especially worried about them not eating, he'll take a more serious tone, but it's what he can do to keep their mood up.
even though exercising while you're cramping and bleeding often sounds. Miserable and Impossible. exercising regularly can help when the time comes! ais offers to join them on a couple walks a week :-) if mc thinks that they can do some light exercise, he'll be there to help them and suggest some easier workouts they can do. i don't think he's a huge yoga person, but he's got some stretches for sure!
guy who goes to buy some things and comes with more than mc expected or listed. he shrugs and says it's better to be prepared, and he was there so why not? he even asked some of the clerks for their opinion and was told some nice advice from a stranger who overheard him and who's he to say no.
might be below leander in terms of cuddling enthusiasm, but is still more comfortable than the others! he'll wrap himself around them, keeping a light pressure on their stomach and rubbing circles against their skin. he never complains about quietly staying and watching them, or talking if mc asks him to distract them. (they wonder how many of his stories take place in a bar. Have you never had a fun experience perhaps at a festival or park? he feigns offense before telling a 'nicer' story anyway.)

the first time it happens? they spot mc obviously in pain and watch them for a while because, well, that isn't their problem is it? they're just watching because they're curious, not because they really care. inevitably, they go up to check on them and bring them somewhere more comfortable because it was "hard to watch"
 it's like... mhin knows what to do, but there's hesitancy because they feel awkward About Offering. do they just hand mc the warm towel? do they ask them to roll over? if they ask what mc wants to drink, will they have the ingredients for it? they leave a bowl by the bed and say "in case you vomit" then hesitate before telling them where the washroom is, but the bowl is just in case. the others are more comfortable asking and taking initiative when it comes to mc y'know. but once they've established a relationship, mhin is quick to know what they like and need without mc even asking! ^^
"mhin" "hm?" "i think i'm dying" "you aren't dying" "i'm going to die" "don't die on my favourite blanket" "what the hell" (/lighthearted) (they do in fact care very much and feel bad. if mc wasn't busy suffering maybe they'd have noticed mhin preparing tea and a hot pack across the room.)
doesn't even try to entertain the thought of mc getting up and doing something strenuous. mhin shoots them a look like ? are you kidding. stay here and i'll take care of it. they won't force mc to stay in bed and assumes they know their limits, but that also means if mc pushes themself too much, mhin is all the more 'annoyed' (concerned!! they're concerned more than Angry, but can you please listen to them for once!)
omfg mhin bringing mc to a place where stray cats like to visit just to cheer them up a little. mhin handing them some food to hold out in their hand. there's a specific cat who really likes to purr and cuddle that they get the attention of just for mc. yeah.

It's not the treatments themselves but the constancy. This isn't just one day, this is the rest of your life. Several days a week walking through those doors, spending hours with a needle in your arm, only to walk away feeling nauseous and dizzy.
The effects only last a few hours, just until your blood pressure evens out, but it's enough to put you out for the rest of the day. With a bandage on your arm, you fold up your laptop and slide it into your bag. What better way to multitask but work while you have the life drained from you. Well, the alternative is hardly preferable, a grizzly, toxic death of drowning in your own waste. You've always been an optimist.
You take a moment before you leave, trying to steady the hazy lines in your vision. You take a breath and leave the office, bidding a quick farewell to Louise, your attending nurse. Outside of the clinic and work, you don't get out much, and much of the latter you can do from home.
You go down the clanging metal stairs to the lobby. As you cross the floor, a man stands by the index of offices, scratching his flopping hair as he glances over. You give a sheepish arch of your brows and tuck your chin down.
"Hey," he stops you before you reach the door, "er, I'm looking for the Wellness Studio? Er..." He turns to you as you stop with your hand on the front door, "Colson's?"
You furrow your brow. That sounds familiar but you really don't know about anything else in the building. You just come here to get your dialysis.
"Erm, I..." you peek out the window and see the sign across the street, red font on a white background, "is that it?"
You point to the moniker that reds 'Colson's' and the man nears to look over your shoulder. He blows out a huff and tuts, "oh god, I must seem like a moron."
"Uh, no..." you push through the door and he catches it behind you, extending his arm over your head.
"Thanks, sweetheart," he nudges your arm as he follows you outside.
"Mhmm," you turn down the sidewalk, set on your path to the station.
"Hey, wait, can I get a name?" He sprints up next to you, "you've been so helpful--"
You look at him, unused to that level of familiarity. You're not exactly that discernable from the brickwall beside you. Your expression must betray your confusion and surprise.
"Just a name," he says as he puts out his hand, "Pete. So, trade?"
You hide your discomfort and reach to shake his hand, eking out your name. You clear your throat and glance around him, not wanting to be rude. You're not quite sure how to gently mention that your train is due.
"So, you come here often?" He stretches his arm out to lean on the brick facade, hand pushing his jacket back as he grips his hip.
You nod and peer around. He's a stranger even if you know his name. You're not very fond of those.
"Am I keeping you?" He asks coolly.
"I just... gotta catch the train," you utter, "sorry, I--"
You go to step around him and he pushes away from the wall, blocking your way, "alright, alright, can I get a number?"
"Er, oh, no," you blurt out in shock, "no, I mean... I don't know you."
He rolls his eyes and smirks, "yeah you do, I'm Pete."
You shake your head and step sideway again. He moves with you. Your chest boils with frustration and a tint of fear. This is why you shouldn't talk to strange men. Especially men twice your size.
"Woah, woah, don't look so scared, honey, I'm not gonna hurt you," he puts his hands down, "I just think you're a cutie. Forgive me for being so forward." He backs away, "don't let me keep you, you go on and get your train."
You frown, uneasy at his sudden appeasment. You swallow and step past him cautiously. You keep your head straight and march down the sidewalk between the passing pedestrians. Their indifference makes you feel even more uneasy.
As you go to turn the corner towards the station, you look back. The man stands amidst the city rush, unbothered by those around him as he watches you, his hands casually tucked into the pockets of his dark red blazer. You shudder and scurry behind the shield of the buildings. You might just ask to go out the back door next time.

AITA as a therapist, to limit when patients can reach out to me?

Before you scroll down and vote YTA please read. To start I don't have paid clients. I work at a public clinic where all the "clients" are people that don't pay per session, they pay taxes, taxes pay me. I work on business hours and I am paid a fixed amount per month independent of if I had 30 or 130 sessions/month. I usually have around 144 sessions/month, though.
The thing is, I am paid a laughable amount per patient by the psychology council (think about ⅕ of what I should be being paid by patient), and I have about 120 patients/month, i am fully booked for at least 1 month and half in advance and the work is rewarding but very demanding. So when I get home all i want to do is to open my Minecraft and have a good night of sleep before starting my routine of having 9 patients per day.
This out of the way, I have some patients that are mostly okay, some that are feeling unwell some of the time, some that are feeling bad most of the time, some that are very very bad all the time.
The issue i have with messaging as a therapist is,
1) tone doesn't carries well over messages
2) I've been receiving messages from patients at 10/11pm when I am about to sleep and I usually answer with "I am unavailable right now but I'll talk to you by the morning. If you're in an emergency, you can go to the municipal hospital to get a fast and effective treatment". Most of messages are patients trying to tell me they'll skip the next session for some reason, though.
3) if someone is really trying to kill themselves, me being over message will do jackshit.
A patient called me out on that saying that I am a therapist, therefore I should be available to patients at all times because what if someone is trying to kill themselves and I am the only thing that saves them? And I have been thinking that maybe limiting when my patients can reach out to me might be an asshole move. (This patient was just trying to reschedule, though)
Tldr: am i the asshole for limiting when patients can talk to me over text message?

The Health Service in the U.K. has decided that it will no longer prescribe puberty blockers to children and minors due to insufficient evidence of their safety. In a public statement a spokesman for the NHS said:
"We have concluded that there is not enough evidence to support the safety or clinical effectiveness of puberty suppressing hormones to make the treatment routinely available at this time.”
Of course there isn't.
As I've often remarked, we are still researching the long term effects of consuming red meat, and yet we are thoroughly certain of the long term safety of administering a treatment of this nature to young children? But certain parties in the United States have decided to place ideology above a common sense concern for the well being of children. It is very important to certain people for the male/female man/women distinction to be undermined within the culture, and any thing that advances that goal is reflexively and zealously promoted by them. The full reason for this goes beyond concern for sufferers of Gender Dysphoria, it is far deeper than that. There are a host of implications that potentially follow from the realization that men and women are naturally different, and the ideological Left despises all of them. The term Feminism itself was invented in the 19th century by a man named Charles Fourier (one of the first Socialist thinkers) who believed that the distinction between men and women had to be entirely erased so that individual humans become interchangeable.
But this would not be the first time that ideological zeal on the Left got ahead of a basic concern for the well being of children.

"Scientists have created mice with two biological fathers by generating eggs from male cells, a development that opens up radical new possibilities for reproduction.
The advance could ultimately pave the way for treatments for severe forms of infertility, as well as raising the tantalising prospect of same-sex couples being able to have a biological child together in the future.
“This is the first case of making robust mammal oocytes [a.k.a. egg cells] from male cells,” said Katsuhiko Hayashi, who led the work at Kyushu University in Japan and is internationally renowned as a pioneer in the field of lab-grown eggs and sperm.
Hayashi, who presented the development at the Third International Summit on Human Genome Editing at the Francis Crick Institute in London on Wednesday, predicts that it will be technically possible to create a viable human egg from a male skin cell within a decade. Others suggested this timeline was optimistic given that scientists are yet to create viable lab-grown human eggs from female cells.
Previously scientists have created mice that technically had two biological fathers through a chain of elaborate steps, including genetic engineering. However, this is the first time viable eggs have been cultivated from male cells and marks a significant advance. Hayashi’s team is now attempting to replicate this achievement with human cells, although there would be significant hurdles for the use of lab-grown eggs for clinical purposes, including establishing their safety.
“Purely in terms of technology, it will be possible [in humans] even in 10 years,” he said, adding that he personally would be in favour of the technology being used clinically to allow two men to have a baby if it were shown to be safe.
“I don’t know whether they’ll be available for reproduction,” he said. “That is not a question just for the scientific programme, but also for [society].”
The technique could also be applied to treat severe forms of infertility, including women with Turner’s syndrome, in whom one copy of the X chromosome is missing or partly missing, and Hayashi said this application was the primary motivation for the research.
Others suggested that it could prove challenging to translate the technique to human cells. Human cells require much longer periods of cultivation to produce a mature egg, which can increase the risk of cells acquiring unwanted genetic changes.
Prof George Daley, the dean of Harvard Medical School, described the work as “fascinating”, but added that other research had indicated that creating lab-grown gametes from human cells was more challenging than for mouse cells. “We still don’t understand enough of the unique biology of human gametogenesis to reproduce Hayashi’s provocative work in mice,” he said.
Study Methods
The study, which has been submitted for publication in a leading journal, relied on a sequence of intricate steps to transform a skin cell, carrying the male XY chromosome combination, into an egg, with the female XX version.
Male skin cells were reprogrammed into a stem cell-like state to create so-called induced pluripotent stem (iPS) cells. The Y-chromosome of these cells was then deleted and replaced by an X chromosome “borrowed” from another cell to produce iPS cells with two identical X chromosomes.
“The trick of this, the biggest trick, is the duplication of the X chromosome,” said Hayashi. “We really tried to establish a system to duplicate the X chromosome.”
Finally, the cells were cultivated in an ovary organoid, a culture system designed to replicate the conditions inside a mouse ovary. When the eggs were fertilised with normal sperm, the scientists obtained about 600 embryos, which were implanted into surrogate mice, resulting in the birth of seven mouse pups. The efficiency of about 1% was lower [although not THAT much lower] than the efficiency achieved with normal female-derived eggs, where about 5% of embryos went on to produce a live birth.
The baby mice appeared healthy, had a normal lifespan, and went on to have offspring as adults. “They look OK, they look to be growing normally, they become fathers,” said Hayashi.
Going Further
He and colleagues are now attempting to replicate the creation of lab-grown eggs using human cells.
Prof Amander Clark, who works on lab-grown gametes at the University of California Los Angeles, said that translating the work into human cells would be a “huge leap”, because scientists are yet to create lab-grown human eggs from female cells.
Scientists have created the precursors of human eggs, but until now the cells have stopped developing before the point of meiosis, a critical step of cell division that is required in the development of mature eggs and sperm. “We’re poised at this bottleneck at the moment,” she said. “The next steps are an engineering challenge. But getting through that could be 10 years or 20 years.”

hi! I have a question about sexual health clinics - long story short I have a pretty strong fishy smell coming from my vagina so I'm planning on visiting the sexual health clinic at my college. I'm nervous though, and while you obviously couldn't know exactly what procedure is at my local clinic, I was wondering what general things should happen at an appointment? What sort of questions will I be asked? Do I have to show someone my vagina? Will they believe that I'm not sexually active? (true fact but doctors are ALWAYS sceptical when I say this)

you're right that this will vary from one location to another. if you're having a health issue with your vagina then your healthcare provider will likely be asking questions about your sexual history; whether or not they believe your answers isn't something that I can answer, although a good healthcare provider should, of course, listen to their patients. there's a good chance they'll want to perform an exam and possibly take samples, which is something to be aware of and prepare for but also something that you can call ahead and ask about.
you can also call ahead to the clinic and ask if there's information about what you can expect if you come in with a likely case of bacterial vaginosis (the most typical cause of the fishy odor you're describing) and request further information about what kind of treatment you can expect from your visit. knowing what to expect in advance can be a huge help!
if it does come to a vaginal exam and you're feeling nervous, that's totally normal! very few people enjoy vaginal exams, but it's totally possible to get through them with minimal pain. my best advice is to come in prepared for a little pain if you're not accustomed to vaginal penetration and have a really honest chat with the person performing your examination about your expectations. when I get my pap smears done, I always open by telling the gyno that vaginal penetration isn't super easy for me and that I may need to swear through the exam and take breaks if it gets overwhelming. every examiner I've ever had has been totally understanding and down to work with me on that; communicating can help so so much.

Other Chapters
In all of your years of knowing each other, Azriel never forgot your anniversary. Each year (except for the fifty that Amarantha ruled) he made it a point to bring you flowers. This one was no different. Some years he gave you your favorite flowers from the market, other years he gave you extravagant bouquets from an artisanal shop, but you always felt extra special when he would pick them himself. It couldn’t happen every year, but whenever he saw a field of wildflowers he would make a mental note. 
He landed on a patch of green sprinkled with vibrant colors. There were daisies and cosmos and snapdragons and so many others he could not name. Azriel picked whichever caught his eye, only the most pristine made the final arrangement. When he was satisfied, he took out a piece of twine and tied them all together. 
When he’s in front of you, that familiar wave of longing spreads through his chest. He takes a breath and kneels on the newly sprouting grass. He grips the bouquet so tight his fingers indented the stems. A breath, then another and he can reach out and touch the headstone. 
It happened slowly at first, then all at once. The Mother granted you twenty years of mated bliss with Azriel. But one day you came home early from the clinic. It was your head, you said, it hurt. That became a regular occurence, until Madja didn’t let you leave and instead sat you down on the exam bed. 
You contracted something from one of your patients, remembering when it happened. “She was just a baby… I didn’t think…” It was a horrifying realization. That patient did not respond to any treatment and in short days the illness had run its course. You knew it was only a matter of time. 
Azriel refused to believe it. Refused to accept it. But nothing Madja did helped. Not even Feyre could help. Not even Thesan. It was a wicked thing that ran through your veins and shut your organs down one by one. During your final months Azriel did not sleep. Always looking for an alternative, a cure, more time, because it wasn’t fair. You had spent your life healing. You had put him back together more times he could count and he couldn’t figure out how to stop the inevitable? Eventually, when you begged him to let it go, to just spend whatever time was left together, he relented. 
By then walking was difficult. A few weeks later, eating was impossible. The last days you mostly slept, the moments you woke were filled with your family saying goodbye. You made Cassian and Rhysand promise to care for him “he’ll suffer, he’ll mourn. But you cannot let him waste away. Please, promise me. Promise me he won’t drown, he’ll be safe, he’ll have a life.” Of course, they both promised. 
Nesta spent hours by your side, brushing your hair, massaging your aching joints. “You can just tell Madja to give me more mirthroot, it helps with that.” 
“Let me care for you, y/n.”
“Can you tell me some gossip? I don’t want all of my last conversations to be so melodramatic.” She scoffed but gave in “Tarquin is betrothed.” You made your best attempt at a gasp. “To who?” 
“I believe she’s an emissary from Day.” 
“He sent me a letter.” Now it was Nesta who gasped. “Tarquin?” 
“Yeah, he found out about me dying somehow and wrote saying that he was sorry things ended on bad terms.” 
“I was always sad we never swam in those pools.” You laughed at that. 
You made sure to send the love that flooded your chest after each visit to Azriel. Wanting him to know that you were ok. That you were loved and not afraid of what came next.
“I think it’s time,” Nesta said with tears welled in her eyes, her lip quivering. A wave of numbness crashed into Azriel. He knew what those words meant. He could feel the bond dwindling each second that passed. Your breath was so shallow, barely there. Until you exhaled one last time. 
Despair snuffed out his numbness. He held your hand and knelt by your bedside, kissed your hands and arms and face. He gripped your hands like they would keep you there, as if holding on tighter would keep you alive. 
He summoned everything he could: love, admiration, devotion. He summoned the nights spent tangled in bed, the days walking by the beach, every touch, every kiss, every “Az”, every scoff, every fight, everything. It all accumulated in his chest and he pushed it down the bond. You had to know the extent of it. You had to know how much he loved you, how that word wasn’t enough. 
But you were already on the other side, waiting for him.

Now obviously you could never dream of affording the money to see him but, it just so happens he hears of your case and takes an interest in you... Happy and healthy reader... and having an instant connection with Zandik!! Oh my gosh yes. Zandik would be the most confused and annoyed because he had never felt a connection with anyone his whole life, he's always been the one by himself. He tries to ignore it, ignore the building feeling in his chest, but as fate would have it, he finds himself bumping into you far more frequently than he'd like. You've had enough of his pretty man constantly invading your mind, so you approach him one day and it all begins from there. (I imagine if he ever dreamed of his past life with you, it'd teach him not to take you for granted. Especially if it was after a fight with you or something.)
I imagine Omega didn't regret his actions, but he still deeply lamented his relationship with you - he missed whenever you'd barge in and lavish him with your affection, how you'd freely rant to him, how you loved him. All were replaced with stone coldness. But perhaps, this is the price of selfishness, one that he had to pay.

hi! love your blog, i love getting to see all these cool cat colors i didnt even know were a thing LOL like i didn’t know there was such a thing as lilac or cinnamon but theyre so pretty.
ive been looking into getting a ragdoll in the future, the ones i have met have been the sweetest cats and they’re so pretty.
ive been doing research into possible health concerns they can have, and all im really seeing is the same things your typical random little guy could have when improperly cared for and things like that. kidney disease while on a bad diet, issues with hairballs when not brushed and groomed properly due to being long haired, etc. ive seen a few places say that they’re more prone to respiratory issues and heart disease, but the latter also seems to be something that breeders work to make sure their cats are safe from? at least in my research.
i guess im basically just wondering if you know anything else that can affect ragdolls specifically, or if there’s anything horribly unethical that’s totally swept under the rug or hidden about them! additional things to research and places to look would be appreciated, i never know where to research stuff like this reliably and you seem to know where to find some good info and stuff on cat breeds :)

Ragdolls are definitely cool cats and they’re a pretty health breed, one of the major things to be mindful of is that this assessment only applies to well-bred individuals from good breeders.
Common breeds mean a lot of breeders which, unfortunately, also means a lot of scammers and bad breeders.
And the Ragdoll is a very common breed so there are a lot of not so good breeders out there… and the temperament and health of these backyard bred cats is a gamble, one that can end in heartbreak.
Now with that warning out of the way the big thing to worry about with the breed is Hypertrophic Cardiomyopathy. Fortunately the mutation responsible for this condition in the breed has been identified and can be tested for. Staying on top of heart health is an absolute must for a good breeder, this means regular echocardiogram’s as well as DNA testing.
The other big ones are Polycystic Kidney Disease and Progressive Retinal Atrophy, both of which can be tested for. These are common inherited disorders in the purebred population, you’ll have a hard time finding a breed which neither of these have been documented in.
Mucopolysaccharidosis VI is a storage disease that has been documented in the Ragdoll breed but it can be tested for and I’m not sure how common it is in current breeding populations.
A breeder shouldn’t cut corners when it comes to health testing, there are several commercially available tests which look for a wide variety of inherited disorders. Optimal Selection is becoming increasingly widely used, although personally I would feel most comfortable with testing submitted through somewhere like UC Davis.
But HCM is the big big one because heart disease is a silent killer. With the other conditions clinical symptoms will be apparent but a cat with HCM could appear normal and you wouldn’t know without testing. And remember - not all cardiac cases will have a heart murmur, either!
The other thing to keep in mind with Ragdolls is they seem to be more prone to developing Feline Infectious Peritonitis. This one is a little more complicated.
FIP occurs due to mutations in the feline coronavirus, which is a common viral infection in cats. Until recently FIP was considered nearly 100% fatal but thanks to Dr. Niels Pederson we now have a promising cure. One of our patients was actually one of the original study cats and has been doing well all this time, she’s amazing to work with - like a piece of living history!
One of the big problems with the treatment is the legality of it… for a long time it was only available through the black market and could easily run you thousands of dollars. Treatment is gradually becoming available through legal venues but it depends on where you live.
Anyways, the point is that we don’t know exactly why these mutations occur in some cats and not others but there’s strong evidence that there’s a genetic component - it’s common for related cats to develop FIP, we’ve seen this multiple times with littermates. We also know that it occurs at a higher incidence in some breeds than others…
And presumably your kitten wouldn’t have been exposed to feline coronavirus at the cattery but it’s not unlikely that they would be exposed at some point in their life given they don’t reside in a bubble… and if or when this happens what are the chances it’ll become the dreaded FIP?
So mitigating this risk when purchasing from a breeder can be a little more complicated… it’s not something I think should turn you off of the breed but it is something to keep in mind - and if you look into a breeder that seems good but has a lot of reviews or reports of their graduate kittens and cats developing FIP? Probably best to keep looking.
I think that’s a pretty good summary on the breed’s health but people are always welcome to chime in!
