If your character is closest to Astarion, and you don't kill Isobel, Sceleritas delivers the news that you'll be forced to kill Astarion. And what does he say about Astarion? "He is so afraid - so, so afraid of everyone...besides you, who he ought to fear most. You could do so much better."

Maybe that wasn't the first time Sceleritas used those words.
