“Could you hand that to me?” 

You followed Victor’s guiding gaze towards the purple ribbon laying on the bathroom vanity. Silently, you picked it up and handed it to him, as he had asked. His fingertips brushed yours in the exchange; his gentle touch sending tingles through your body.

Quickly retreating, you returned to your spot against the wall where you so often watched Victor perform his morning routine. You had come to visit him so often in the morning, that it had become your routine to watch him in the morning.

You looked on with lidded eyes as Victor took the length of ribbon between his teeth, his hands working in tandem to tame his long tresses. Pressing your body against the cold wall, it did little to cool your heated core.

Loose locks fell to one side, curtaining his face, his jeweled gaze intense as he stared into the mirror as he swept his long strands into a ponytail. Your lips parted as you watched him tame his hair, hair that you so wanted to run your fingers through and feel his long, silky strands against your skin. Hair that you wanted to wrap around your fingers and give a gentle tug. 

Do you want to touch it?  You could hear his voice in your head, a soft, sensual purr teasing you with his wicked smirk.

Knees weakening, your body slid down against the hard tile of the bathroom walls. Your hand, resting on your belly, resisted the urge to inch down and touch yourself. Instead, you imagined the face that Victor might make if your body was atop his, your knees straddling his hips and you were to tug on his hair. A soft sigh escaped your lips. A sigh so soft, you didn’t even realize you had made the sound. 

That was, until Victor called your name. 

Snapping out of your reverie, your body straightened like an arrow as you attempted to regain some form of composure. You offered him a small smile, hoping this would keep your thoughts hidden. 

“After you,” he said, opening the door, his arm extended in a flourish, waiting for you to exit first. Embers burned in your belly, their fire not yet extinguished. 

And that was okay. Because as Victor had his morning routine, you had your evening routine where Victor came to visit you.

And lived out your morning fantasies.

Everything? Any and all things? He’s such an anxious baby. But more seriously, people questioning him or his behaviour, no matter how little they may mean by it really sets off his anxiety.

Not physically. Not yet.

35. If they could change one thing about themselves, what would it be?

Maybe give himself a little confidence, but it’s hard to say. He’s pretty self-critical.

48. Is there anyone that they would willingly kill?

No one specific in his story at the moment, but if it came to it, I think so. It would have to be for the sake of someone else, and it would either have to be excruciatingly planned out of extremely spur of the moment.

You know, I read a lot of fanfiction and theories based on the 'Who is the strongest side' hypothesis, and I wondered how you think who has the greatest power / influence? Anyone in particular, or do they all have the same 'power'?

Short answer: it depends. Being a Side already means having a huge power/influence over the person. If we think about it, only the most important aspects of Thomas’ personality are represented by a Side. All smaller parts of him aren’t strong nor important enough to become Sides.

Therefore, a Side is already a very powerful being and when we talk about “who is the strongest”, we’re talking about power levels that are already very huge.

Having said that, let’s try to see each Side in detail.

Logan

* Logic can improve or destroy every creative work. You can write the most beautiful story of all time, but if the plot and the action don’t make any sense, the entire story fails.

Also, logic knows no compromise. No matter if you decide that 2+2=7 in your mind: logic will always tell you that 2+2=4. You can protest and scream and do whatever you want, but between your mind and pure logic, logic always wins.

* Logic is completely untouched by feelings. Feelings are susceptible to change, if you’re overwhelmed by feelings your perception of reality is altered. But logic doesn’t do that. Logic offers you clear, straight facts, with no emotions attached. Therefore, Logan shouldn’t be affected by emotion-based Sides like Patton and Virgil. Or, at least, their influence shouldn’t be as strong on him as it can be on Creativity.

* We humans are not creatures made of pure logic: on the contrary, we’re heavily influenced by feelings. And that means often pushing our logic to the side, to let emotions take over.

In other words, Logan might be powerful over other Sides, but his influence on Thomas is probably the weakest.

Roman and Remus

* Creativity is a very fickle force: some days it blasts 200 ideas per minute, other days it’s anywhere to be seen. Sometimes it pretends to be satisfied, other days leaves you to anguish.

* Creativity should always deal with logic. Creativity is heavily influenced by feelings, emotions and the person’s background. It can be nullified by anxiety and pushed back down.

* And yet, Creativity is one of the primordial forces of the individual. Creativity shaped us, since we lived in caves. Creativity has always been with us and, more than once, it asked us to dedicate our lives to it. And no matter how high the price is, we pay it.

So Roman and Remus might be not as powerful as other Sides, but their influence on Thomas is probably the strongest.

* Who’s more powerful between Roman and Remus? I like to think they’re on the exact same level. The only difference is that Remus is free to unleash all of his power into pure creation, while Roman uses it to create and shape his works in different ways.

Virgil

* In the worst case scenario, Anxiety has an incredibly destructive potential. A severely worsened anxiety can be so destructive, to the point of nullifying all other Sides’ influence.

Two words: paranoid delirium. A pathogeny so severe to completely change the person’s life, who will see everything as a threat to their survival.

In other words, Virgil has the terrible potential to wipe out Thomas’ morality, logic and creativity, all while closely tying with self-preservation, in order to justify his behaviour as a way to protect Thomas.

* In the best case scenario, Anxiety can become a careful guardian, who can be pacified by logic and doesn’t hold too much influence over emotions. So he can work with them on the same level.

* Anxiety and Creativity have a strong influence over each other. Creativity can soothe anxiety (as we saw during Are There Healthy Distractions) and anxiety can improve creativity (as Roman admitted in AA - Part 2):

[Roman]: Anxiety, you're... ...what pushes Thomas to rehearse and rehearse before performances (...) I think that's as good a sign as any that... you're willing to work as a team... and that you make us... better.

In conclusion: Virgil has the potential to destroy the mind and Thomas’ life, but if properly tamed, he can work with the other Sides on their same level and have a very tamed influence over Thomas.

Patton

* Patton has a colossal influence on Thomas for obvious reasons. He isn’t just his emotions and feelings, but “my sense of right and wrong that I’ve learned ever since I was a kid” (My True Identity). In other words, Patton is one of the foundations that make Thomas who he is. Of course he has such huge influence over him.

* Since he’s the embodiment of Thomas’ feelings, he’s able to shut up logic.

* Patton is basically the reason Thomas has two Creativities: Thomas’ education, “his sense of right and wrong” is the main reason why he classified products of his creativity as “welcomed” or “forbidden”. So maybe Patton had no real influence over the creative split, but his morals sure had.

And if there is a Side who’s strong enough to be the reason why there are two versions of the same force, then said Side should be a pretty strong one.

Janus

* It has been canonically said Janus has power over all Sides. During CLBG, Roman explained to Thomas that:

[Roman]: If you really don't want to know something, he can keep our mouths shut.

and

[Thomas]: Are there any other Sides that Deceit has hidden away from me?

[Patton]: ...Yes.

So not only Janus has power over every other Side, since he’s able to physically control them and force them to shut up, but he’s strong enough to overcome and hide other Sides. And, as I said, Sides aren’t weak forces.

* Janus is self-preservation. And if there’s a force that is so goddamn unstoppable, is self-preservation. Self-preservation overcomes EVERYTHING. If there’s some threat to your life, everything else disappears, except for your friggin’ desire to LIVE. And, as we saw with Virgil, even if paranoia takes over, self-preservation can’t be put down, but on the contrary, it’s enhanced by it.

Also, self-preservation is the reason why committing suicide is so difficult. All your body wants is to live. Every single creature on this planet wants to live. And when you want to end your life, you have to overcome something SO powerful, that is literally able to push you away.

* Under normal circumstances, self-preservation doesn’t have too much influence over the individual. Despite being so powerful, it could be more like a placid, background guardian - a bit like a tamed Virgil.

“Morning…”

Liam was perched on the edge of the bed, his eyes bright and wide, twinkling like gemstones, awaiting your return like a loyal pet. His soft smile a reminder that last night was real.

You had spent the evening together – it started with a date in the city and ended with a late night in your bedroom. A very, very late night. When you woke before Liam, who was sound asleep by your side, you quietly went to the powder room to get ready for the day.

“Hello…” you replied as you approached him, his hand on his lap a subtle invitation to sit. He extended his other hand to you; his hand was soft and smooth as you slipped your smaller one in his.

Once seated, he pulled you closer to him, one arm gently wrapping around your waist, the other cradling the back of your head. Resting his chin on your shoulder, he let out a long sigh, his breath a warm whisper against your bare skin.

"When I woke up alone, I thought maybe last night was a dream." His lips ghosted the mark he left last night, his tongue traced its outline. "But then I saw you," he continued, "and I realized it wasn't a dream."

He pulled away, his eyes twinkling like gemstones as he stared straight at you. A shiver crawled down your spine, a delicious anticipation left in its wake. 

You didn't say it, but you needed Liam. Right now. And judging by the look in his eyes, he felt the same way.

Bodies shifted until you found your back pressed against the bed, the skirts of your dress conveniently pushed around your waist. His mouth met yours in a searing kiss as his body pressed against yours. 

His body rocked against yours, filling you with an immeasurable amount of pleasure, heat flooding your core with a familiar warmth. 

"Liam…Liam…" His name fell softly from your lips, like petals drifting from a flower. With each stroke, you whispered his name again and again, begging for more and more.

And more he gave, until you were consumed with rapture and the waves crashed around you both, leaving you both in a state of bliss.

It was still morning as you nestled your head against Liam's chest, his fingers twining in your hair. You smiled to yourself, knowing this reality was better than any dream either of you could have had. 

"I love you, Liam," you whispered, unsure he could hear you. "I really, truly love you."

Role of Amylin in Thyroid Cancer- Juniper Publishers

Abstract

The prevalence of thyroid nodules and cancers were shown to be associated with type II diabetes mellitus (T2DM), obesity especially central obesity, and metabolic diseases. Recent studies reported that amylin, an amyloidogenic pancreatic peptide deposited in the pancreas in T2DM, theses aggregates enter the circulation and recently reported to be deposited in the brain and involved in the development of Alzheimer’s disease. These circulatory amyloid aggregates may also deposit in the thyroid gland and causes activation of some pathogenic pathways that may result in the development of thyroid nodules and cancers. This review presents some of the available data about the role of amylin in type 2 diabetes mellitus (T2DM), the association between T2DM with thyroid carcinoma, and the possible role of amylin in the pathogenies of thyroid nodules and cancers.

Introduction

Amylin and its Physiological Functions

Amylin is a 37-amino acid peptide that is co-secreted with insulin from β cells of the pancreas in response to nutrients intake, such as glucose, amino acids and lipids [1]. It is also called as islet amyloid polypeptide (IAPP) as it is found in the form of myeloid aggregates in the pancreatic islets [2]. Amylin plays a role in glucose homeostasis, reducing blood glucose level, inhibiting food intake, an action that can be mediated by the effect of amylin on leptin expression and signalling in the ventromedial nucleus of the hypothalamus [3]. The effect of amylin on food intake can be also due to the central action of amylin on the area postrema as ablation of this area abolished the inhibitory effect of amylin on food intake [4]. Amylin decreases gastric motility and delay gastric emptying, an effect that helps in delaying the entry of nutrients into the small intestine, delaying intestinal absorption and subsequently decreasing the serum levels of different nutrients especially glucose. All those effects of amylin can explain athe effectiveness of using amylin agonists in the treatment of diabetes and the potential use of amylin obesity [5]. In addition to the centrally-mediated effect of amylin in delaying gastric empting and food intake, it has been also reported that amylin mRNA and its receptors are expressed in the stomach, suggesting a direct effect of amylin on gastric emptying [6]. Amylin is a member of the calcitonin family, which includes, αcalcitonin-gene related peptides (αCGRP), βCGRP, adrenomedullin (AM) and intermedin (AM2). The effects of the calcitonin family members are mediated by calcitonin receptors (CTR) and CTR-like receptors (CLR). The specificity of those receptors for the members of calcitonin family depends on certain receptor-associated proteins called receptor-activity-modifying proteins (RAMPs) [7]. The receptors for amylin include, AMY1, AMY2, and AMY3, each consists of a combination of CTR with RAMP1, RAMP2, and RAMP3 respectively [8]. Amylin is metabolised via renal excretion and by proteolysis into less active and inactive metabolites [5]. Amylin (1-37) is the major molecular form of human amylin revealed by radioimmunoassay of the pancreatic tissue. There are two forms of circulating human amylin, the non-glycosylated form which is the biologically active form and the glycosylated form [9].

Dysregulation of Amylin with Diabetes Mellitus

Although amylin works to decrease blood glucose level, it is reported that amylin can be cytotoxic due to the formation of amylin aggregates; amyloids that associated conditions of T2DM [10]. Amylin aggregation in the pancreatic β cells induces endoplasmic reticulum stress and damage of mitochondrial membrane resulting in death of the β cells of the pancreas [8]. Normally, these aggregates once, formed, an autophagy process with be stimulated and degrade the aggregates protecting the cells from their cytotoxic effects as evidenced by the appearance of amyloids in the pancreatic β cells of mice lacking ATG-7, ATG the autophagy-related proteins [11]

Diabetes Mellitus and Thyroid Cancer

Studies over decades confirmed the association of different types of cancer with diabetes especially T2DM. Worldwide, thyroid cancer is the most common endocrine malignancy among women [12]. The increasing prevalence of thyroid carcinoma as well as diabetes suggesting a link between them [13]. In 2017, Li et al. [14] published a met-analysis of 16 cohort studies that included 10,725,884 individuals, with a total of 8032 cases of thyroid cancer reported a positive association between the diabetes mellitus and increased risk of thyroid cancer mainly among women. In addition, a recent study reported a decrease in survival among patients with thyroid carcinoma who suffer from T2DM as compared with thyroid cancer patients who are not diabetics [13]. A more recent study reported an association between the increased risk of thyroid nodules with obesity, central obesity, insulin resistance and disturbed lipid profile [15]. Based on these studies, amylin may play a role in the pathogenesis of thyroid carcinoma. Understanding the regulation of amylin can help in enhancing its uses as a treatment or even as prophylactic.

Conclusion

Amylin is physiologically important in glucose homeostasis. However, if it forms aggregates, it causes cellular stress resulting in diseases such as diabetes mellitus. Increased association between T2DM with thyroid nodules and cancers suggests a possible role of amylin in the pathogenesis of thyroid carcinoma mediated by deposition of amyloids.
