RPS Science Photographer of the Year competition 2020
Surgery can be challenging at the best of times, but this operation has an extra layer of tension. Colleagues watch closely as surgeon Professor Robert MacLaren carries out a pioneering procedure to inject gene therapy into the eye of a patient with an incurable form of blindness caused by a genetic fault. Genes correcting the problem are packaged into harmless viruses, which have to be delivered directly to the cells at the back of the eyeball. Gene therapy for sight loss is an area where incredible progress has been made in recent years, with the first commercial gene therapy, Luxturna, now available for patients in Europe and the US with a condition called Leber congenital amaurosis. Researchers are working on gene therapies for other sight loss conditions caused by faulty genes such as retinitis pigmentosa, which causes the light sensitive cells at the back of the eye to break down.

These pricey telescopic prescription glasses have been a huge help for me dealing with low vision due to my RP(retinitis pigmentosa). Doesn’t help with lighting issues but at least I can see normal for the most part. Besides it does make me look as if I’m ready to perform a complex surgery of sorts. “Yeah, if something comes at me from the side, I often can’t see it until it’s, like, right there, in my center vision. My eyes don’t adapt well from light to dark, and extremely bright light is just as bad as extremely dark areas. There are...tiny black spots in my vision, kind of like floaters, and they’re easy to ignore at this point, but I have cataracts already. Up to one in 4,000 people in Europe are affected by retinitis pigmentosa (RP) – a rare genetic disorder that affects the layer of light-sensing cells at the back of the eye (retina). Many people with the most common form of RP have a change in the gene encoding the light-sensitive pigment rhodopsin. These changes cause rhodopsin to fold into the wrong shape, triggering problems inside retinal cells and ultimately causing them to die. In search of solutions, researchers have been studying fruit flies to investigate the genes that help to get rid of misshapen rhodopsin. These images show how various combinations of changes in certain genes lead to different effects in the insects’ eyes. One of these genes, known as SORDD1, is particularly good at getting rid of faulty rhodopsin, so finding a way to encourage its activity could prove to be a way to treat RP in the future.
