Hello from your local intersex person. Don’t say I have a DSD. I am intersex. My sex and the way my body works is not disordered. I have certain sex traits that would be considered male, and others female. I am intersex. If we need to get specific I have CAIS. But not a DSD because my CAIS just is the way it is. Not good, not bad. And not a disorder.
No, I’m not ‘disabled male’ as some terfs like to call us (I am disabled, but not due to my being intersex). And no, I’m not a disordered female either. I am a bit of both male and female. And to try to erase that not only is wrong, but it is actually dangerous, especially medically. Dancing around the fact that I am intersex ignores my specific health needs.

The genetics behind intersex variations, mine (CAIS) in particular is a special interest of mine because I’m autistic, and I’ve been thinking. In theory it would be possible for someone with XX chromosomes to have AIS. It’s exceedingly unlikely of course, and it could only happen if there was a mutation in the androgen receptor on both X chromosomes. If someone inherited one mutation from their mom and had a de novo mutation on the x inherited by their dad, or had two separate de novo mutations on their X chromosomes, they would have a form of AIS. It probably wouldn’t be recognized unless say they were a trans man, and they’d develop perisex, but certain things like little body hair, dry skin and little body odour would be in common with regular CAIS. A similar situation could probably also happen with Turners syndrome as well, if someone with Turner’s had an AR mutation on their X chromosome they’d have AIS. The probability of these would be really low and the probability they’d be diagnosed probably even lower, but I think it’s cool to think about. Interestingly, there has been a case of someone with Klinefelter’s syndrome with X chromosome duplication who also had CAIS.

idk if this sounds weird or whatever but I’m relatively confident I could accurately diagnose CAIS in someone else if I needed to. Like, I know what symptoms to look for, things like not having a period by later teenage years, less body hair, dry skin with not much acne etc, then would probably get bloodwork ordered for testosterone, if it came back as really high but with not really any androgen based physical effects, then I’d start to really suspect it, then would get an ultrasound done probably since most of us don’t have a uterus (though a case or two have been published of people with CAIS and persistent Müllerian duct syndrome, so while the lack of uterus is a pretty defining symptom it’s not necessarily a factor to rely on on its own for making a diagnosis- like I said persistent Müllerian duct syndrome exists and MRKH (basically similar to CAIS in that there’s either no or little Müllerian duct but individuals with MRKH usually have XX chromosomes and ovaries) exists so other factors and tests need to be taken into account, then you’d need a karyotype to confirm XY chromosomes, at which point with the high androgens without masculinizing symptoms and a female phenotype the diagnosis is pretty clear as CAIS, though AR gene sequencing would be a good idea for both the individual and their mother especially if their mother has sisters with kids since if it was inherited from the mother there’s a possibility her sisters are carriers and so should watch for CAIS in their kids.
I just think it’s funny that since it’s rare enough, I have it and have a special interest in it since I’m autistic a lot of even doctors don’t even get what it is let alone how to diagnose it and besides the interpretation of bloodwork results id probably be better equipped to diagnose CAIS than most doctors.

Hi, genderfluid/transmasc person with CAIS here. I know T isn’t an option for me and transitioning, still struggling with that a bit but I do have a bit of a question. Minoxidil I’ve heard can do stuff with facial hair growth, and I still have to do lots of research and everything, but to my knowledge it’s not an androgen. Would minoxidil work in people with CAIS? And would it be able to achieve any facial hair?

So there's this one case study talking about someone with CAIS who used minoxidil successfully for hair regrowth, but they were using it for head, not facial hair. It seems plausible that it might also be effective for facial hair growth, but I wasn't able to easily find any more research on facial hair specifically or anyone online talking about it.
