Since I colored Ness with his snowman, I chose this panel of Kaiser to go with it. Originally the sky was blue, but I had the sudden idea to do a sunset. I think this is better than what I could have done with the blue sky.

I don't think Kaiser and Ness would be good together
Which is why i think about their relationship constantly despite that, i love these freaky little soccer dudes and their obsessive codependency
If there's one ship in bllk that I absolutely adore the content for and think about constantly besides Kunigiri, it's Kainess. And it's not because I think "Oh they'd be such a wonderful couple" or "They look good together", this is one of those ships where if they ever got together I feel like they'd actively make each other worse because they have so much internal stuff to work on alone first, you know? And just thinking about that potential train wreck of a relationship is enough to keep me entertained for weeks on end.

I've said before that Kaiser and Ness are a Bachira and Isagi parallel in that they show the toxic bad sides of a codependent relationship, but I think there's more to it than that. Like Kaiser and Ness are reliant on each other for both ego and attention, but they're also both deeply lonely people at the end of the day. Even in the scenes where it's just the two of them, they never let up the act of trying to surpass everyone and be at the top.
They have no silly banter or back and forth, even in this panel Ness looks more like Kaiser's personal servant than his friend or partner.

Where as everyone else in the Blue Lock compound has some healthy way of destressing after a match, these two weirdos (affectionate) sit in dark rooms and watch the same matches over and over again while plotting like saturday morning cartoon villains on what to do next game. I mean, even Itoshi Rin has a destress activity he does that's NOT soccer related that helps him not morph into some soccer obsessed monolouging freak, so to see these guys just constantly always planning and thinking and practicing it really puts into perspective how much of the time that they spend together is them not having a fun time casually. And if you compare what they do on their down time with what the other "partners" in Blue Lock do, this difference becomes even more apparent.
And I don't know, I find that to be a really interesting aspect of their relationship just because even though they are so clearly missing all of the key elements meant to make a healthy partnership, it still works on the field anyways because they're both equally obsessed with the same thing. Kaiser loves football and wants to be the best. Ness want's to see Kaiser become the best because he loves football. Their devotion to the game drives them closer to one another, but it's clear only one side holds any real "affection" for the other directly outside of the game.
Now, I do NOT think Kaiser hates Ness which I think people assume if you say "Kaiser doesn't hold a lot of empathy for Ness, or cares for him" that it translates to "Kaiser hates him".
I think Kaiser keeps Ness around because deep down he knows that at the end of the day the only person who would be willing to follow him even if he couldn't become the worlds best is Ness. And I think that's because Ness loves soccer in a different way than Kaiser does. To Ness, soccer is a really magical sport and one where amazing things can happen. And Kaiser is the only character who has done the most insane feats in the manga so far. Kaiser does things on the field that seem impossible until he pulls it off, which is the whole crux of his ego anyways: Making the impossible, Possible.

So as long as Kaiser plays that way and does all of the amazing things he's been doing, Ness would follow him literally anywhere because that's what drives him.
But Kaiser is different, it's clear that to him soccer/football is not something that is grounded in the fantastical, but something that's tangible and real to him. He takes it seriously even if he goes about it in the most dramatic and campy way possible. Looking at old chapters of Blue Lock when Kaiser was first introduced is so interesting to me because I forgot about Kaiser's whole "king" attitude where he pretty much talks and acts like some nobleman with a crown and scepter.
He even makes Ness "bow" to others or makes Ness physically lower than him like a king does with some peasant. You could chalk it up to him making Ness "apologize" in the Japanese way by also bowing, but I don't think that's why he does it just because his entire character is just so "king" coded.

It's worth noting that he also makes Ness "bow" whenever Ness seems to lose any kind of faith in them winning. Which is why I don't think the above two times was him making Ness apologize, but that its something he does to ground himself or make Ness fall more in line with how he's thinking at the moment. Notice how he seems to only do it when Ness isn't sticking to the right "script" or seems to show the wrong reaction openly etc.

But anyways, Kaiser and Ness love soccer but they don't love it for the same reasons and that is what ultimately conflicts with their relationship in the end. That, and they've both got their own issues to work out.
I feel like if they did ever get together, it just wouldn't work because in order for it to work, Kaiser has to first realize that he can still be an amazing player and have people regard him as the best without obsessing over where he sits in the rankings. That he can perform things nobody else can and never will and that is the thing that will separate him from the rest of the crop, not a trophy saying "Number 1" on it.
Maybe before when they first met things could've worked out well, but even still I think Kaiser was dead set on his goals of becoming number 1 long before he ever met Ness. We will definitely get to see the extent of that next week for sure I hope, when we learn more about Kaiser's past.

Ness is actually a very rational player, despite his obsession with magic (and Kaiser)
It’s past 3am and I was just about to go to sleep, but I suddenly had a eureka moment and now I’m wide awake! Forget what I said about going on a semi-hiatus. Ness occupies my mind, and I need to get this out.
Before I saw the c263 leaks, I jokingly posted about how Ness could be Bastard Munchen’s new ace should Kaiser ever leave for Re Al. But now this idea is no longer just a joke. It’s actually really possible...
Let’s look at this panel from c262:

Don’t attempt any more irrational plays.
The official translation used the word “impossible,” but a look into the raws would show that it’s not the same “impossible” Kaiser is obsessed with. More info here.
And not only did Ness call out Kaiser on being irrational, he also told him the most rational thing to do, which was to wait.
Kaiser, of course, just brushed him off—but who cares about that ungrateful bastard! Let’s focus on Ness.

Ness has a very scientific approach to his plays. He looks at existing data and decides his next move based on it. When he’s stumped, he needs a strategy to follow or a role to adhere to. In other words, he needs clear directions:

Ness may not realize it, but growing up in a family of scientists has greatly influenced how he thinks. His rationality makes him a highly skilled player, but at the same time it limits him.
Because of his extremely rational way of playing, he’s unable to look beyond what is possible and achieve the magic he seeks. But Kaiser is different. Unlike Ness, Kaiser is always out to attempt the impossible.
So when Ness is with Kaiser, his rationality gets shut down and he is led into the realm of magic—or at least, the magic he think it is.

What should we do, Kaiser?
Just keep doing like we practiced. I’m not asking you to be creative... Just have faith. Nothing’s impossible for me.
It also doesn’t help that whenever Ness wants to discuss proper countermeasures with Kaiser, the latter shuts him off and tells him to just have faith—an extremely irrational thing.
But is it really magic when thinking stops? Is it really magic to just leave things to faith?

Obviously, the thorns from the blue roses represent Kaiser’s control on him.
But what about his chains? Why are they separate from the thorns and roses? Kaiser has a glass chain, so why can’t Ness have an all-thorns one?
I think it’s because there are actually two things chaining Ness down.
If the thorns and roses represent Kaiser and magic, then the hard and cold metal one represent what science and logic have been to Ness.
Aside from breaking free from Kaiser, for Ness to truly awaken… he needs to reconcile the realms of science and magic.
Because science is magical.
Ness thinks football is magic. And Noel Noa, the best striker in the world, is a very logical player.
I used to think Ness being in a team that prioritizes rationality was a mistake… but now I think it’s fate.
Ness may just be Bastard Munchen’s new ace.

I’m not sure of the next page either; the big text is read as “Compatible with Isagi”, but there might actually be a negation before that, changing the meaning? I’m really not too sure.
In any case, Isagi is going through the motions of stealing the ball from Kaiser and explaining his thought process; I look forward to seeing this bit translated. Also, Kaiser’s face and that “Again” from him make me way too happy; if the Blue Lock boys have their say in it, he won’t be touching a ball for THE 👏🏻 REST 👏🏻 OF 👏🏻 THE 👏🏻 MATCH 👏🏻!!!
Also happy to see Isagi slipping into flow easier and faster, once more. Even so, things won’t go according to his plan, not at this point, cuz HERE COMES REO!!!

Summary:  As one would have expected, coming of age isn’t quite easy when you’re a Yato. Kagura goes to an Alien Hunter school and Okita longs at the sky.

chapter ii - penpals in space
There aren't many things Sougo find unsettling. After all, his boss and foster father are known as both a gorilla and an insufferable stalker while also leading the police force. That being said, Hijikata is more of a leader than Kondo—at least when it comes to the technical aspect of leading and being the pain-in-the-ass man of Gintama he excels at his job. For now. There's no telling when one of his assassination attempts will succeed and when the rise of Kaiser Okita Sougo de Sade II will finally happen.
Okita Sougo is a disturbed individual who is rarely surprised or startled, even when his boss comes in naked and marries a gorilla princess from outer-space, or when Hijikata-san slides away to a dark corridor sniffing mayonnaise like a cat on drugs. As a matter of fact, he's barely moved when he finds after a rather rowdy and drunken night, Kondo, the boss and a cardboard hobo together in a bed that's probably stained with suspicious night activity.
Yet it seems like his mind cannot help but notice how a certain red-haired brat hasn't shown up at their little destructive rendezvous. He doesn't really care at first. There are times where the Yorozuya somehow find a job and she's gone for a couple of days—often less since the Shinsengumi and the Yorozuya tend to meet in a twist of fate, making the trio as much as allies as they are foes to them. For the same reason, he and China forged a bond that is neither friendship nor hateful but something in-between. They hold a healthy rivalry wherein both can unleash their potential, get loose without consequences—besides the traumatised civilians and destroyed buildings—and waking up all the muscles and nerves of the body. Consequently, his body's been more than dull these weeks because she doesn't show up anywhere.
Whether it’s the park, Gran's candy shop or Kabuki-Cho that girl is nowhere to be found, even though he catches sight of the boss and Shinpachi-kun wandering around. He wonders if she's avoiding him, despite finding the idea absolutely aberrant since that person wouldn't ever run away from him: if he ever goes in the wrong path she'd dash at him in a furious battle cry.
He waits patiently for the whole week. That girl does have a complex family so perhaps a redeemed-terrorist brother or an overprotective father swooped in.
They share a pact of privacy, not that they talked or decided it. It happens that she doesn't ask him anything and he doesn't, either. Okita does not understand what she thinks about it but he's bad at chatting, much less with a female for he comes from a poor, rural region like Bushu. But he's a great swordsman, and much more agile with his sword than his words. It's not that they hate each other but there's simply no need for useless blabbering.
Or so he thinks. Until now. He’s truly puzzled by how far a brat like her could go to. In the middle of his though Okita is brought back by the familiar, deadpan-toned ringing of the phone nested in his pocket.
I have a Hijikata~ I have a tabasco~ Die Hijikata, Hijikata die~
With a sudden slowness, Okita catches the call and lie through his teeth, "Hijikata-san you shouldn't bother an upright policeman like me, I'm patrolling and this is an important duty for the citizens. But the trash's duty is to die, so die Hijikata-san."
Hijikata's voice comes through tired by his antics. "What upright policeman?! I'm sure you're somewhere skipping patrols again."
Children's laugh in the background and sat on his favourite bench under the comfortable shade of a tree, Sougo deadpans yet again. "I don't know what you mean."
Hijikata coughs, "anyway the princess and the Yorozuya are at the terminal. They want to go to the emergency transmission room and you have the keys, right? I don't care about the perm idiot but don't make the princess wait too much."
"Alright." He cuts the call and yawns. So much for slinking away of the patrols and going through suspect alleyways, nobody knows better than him. At the very least the Terminal isn't too far, Okita comforts himself with the idea that he'll reach the massive, monolith building in less than fifteen minutes if he doesn't get accidentally murdered like that one time.
And as planned, Okita arrives in less than fifteen minutes at the Terminal where he's welcomed by the princess and the boss though the latter looked like he just survived a near-death experiment.
"Thank you for being so fast Okita-san," the princess nods at him and smile. "I want to communicate with a ship in space so I need our best antenna, it's important to me."
Wordlessly, Okita guides them through the intricate hallways of the Terminal until they reach the inner core where the Altana flow the strongest. As they reach further in the giant building the doors become more and more secure and ending with shuttle doors that lock soundly behind them, though they're usually found on space ships to airlock.
"This is the emergency transmission room," Okita says as he passes the key—a card and enters the clean and neat room. As the name suggests, the transmission room is small and have one computer as well as some chairs laying around in a nearly all-white room that reminds him too much of a hospital. He switches on the computer in a flick of the finger and the panel brightens up. "Do you have the ID of the ship, princess?"
"Use this Sofa-kun," Gintoki scurries through her kimono and proffers him with a messy piece of paper that Okita can barely read, as the notes are obviously scrambled hurriedly.
Taking a minute or two deciphering Okita finally starts typing on the computer.

"What are you doing here, boss?" Okita slouches down onto the chair and yawns yet again, though the perm should probably be doing the yawning instead of him. His eyes are almost red from lack of sleep, not to talk about the big dark circles beneath them and the yellow, sickly tone of his skin. Okita has never seen Gintoki in this state.
"Using this too," Gintoki points the machine, "I asked Tama but apparently Gengai didn't put any antennas that connect to outer-space. That old man had one job."
"Why need that though?" He looks up the computer, the request is confirmed, "if you're smuggling some illegal SpaceBay goods I'll need to arrest you, boss."
"Doesn't Okita-san know?" Soyo interrupts with a surprised face, hand over the mouth and all. "Kagura-chan, she has gone to space."
Gintoki cocks an eyebrow as well, "that brat went off quickly and didn't really prepare anyone for this, not even her dear Gin-san so I guess Souichiro-kun you weren't spared either."
"...Space?"
"Yeah. Space." Gintoki rolls his eye, "she said she wanted to go to a school and become an alien hunter like the baldy. It was his idea, anyway."
"But fortunately Kagura-chan gave us her ship's ID and maybe her school's station too. She said it'd be like sending letters. Maybe you should give it a try as well Okita-san, I'm sure Kagura-chan wants to hear from you too!"
Sougo is tempted at first but his throat dries up, his stomach knots and he refuses. "I don't really care if that brat is in space," he lies through his teeth but it soothes his pride. It's amazing how he even bothered to tell her his farewell and she decides to skip right through it in return, not even bothering to get someone to warn him while he's scurrying the town like an idiot. Intense need for killing Hijikata suddenly surges through his mind, wanting desperately to release his anger. The irritation almost makes it to his face and he barely manages to hide his feelings before Gintoki starts grinning at him.
"Soda-kun—"
"It's Sougo."
Gintoki brushes it off in a dismissal wave, "all the same, all the same. Kagura's next break is going to be in two months, you know? It's fine if you want to chat with her."
Two months.
"I'm fine boss, she and I can always have a match after that. I can't say it's that easy for you though, you look like you haven't slept since she left."
And it's probably what happened.
Before Gintoki can retort the computer beeps in loudly, followed by white lines of codes scrolling through.

Two messages pops up on the panel: one for the princess and the other for the boss, they seem to be long and including few other friends or family—saving up on the number of messages she sent. That's almost too smart from her, considering her usually brash and reckless behaviour. And then, a third message pops up.
From: Kagura
To: Idiot
Subject: I'm in space, loser
[Message Encrypted]
It's for him. He's almost sure of it and it should make him angry that he identifies himself to an idiot but then again that's exactly the kind of things she'd pull off. Of course, she would, that's just how she is and it's infuriating that even for a fraction of seconds he felt hurt. Indeed, he has been fooled, because there's no way he's ever gonna be so mushy mushy with her, they simply aren't like that.
Okita let a grin distorts his face, he wants to reply and curse her right now and then.
"I thought you weren't interested, Solar-kun." Gintoki sounds too victorious for his taste.
"I'm not going to answer," his grin fades back to his usual poker face. "I have too much duties to play the pen pal of a brat."
"Hah," Gintoki scoffs and raise his arms for more dramatic effect. "That's new from you Soda-kun, if Shinpachi's a virgin you are virgin from any work."
"Right back at you, boss." Okita rolls his chair away from the screen and stretches, "anyway princess you can use the computer, you should be able to read and answer her now."
While Soyo excitedly takes a seat in front of the computer and starts to furiously type on the keyboard Okita rummaged his pocket again for his phone. The model is black and printed with the golden pattern of their uniform and looks below-average, whereas the average citizen possesses a slim smartphone the Shinsengumi's is rather bulky. It has to survive their activity, to the weather and should they throw it their phone have probably more chance knocking someone cold and very much dead but that's not all. With the human's genius and Amanto technology they could create a slim and solid phone but what they have is much more.

Okita selects the latter and the name of the antenna— a series of number— shows up.
Unlike any ordinary citizen, the captains of the Shinsengumi have access to all the antennas of the Terminal, the antennas were of course powerful enough to be reachable anywhere from Japan as long as they have a computer to connect with.
Simply put, that computer was built small enough to fit in their pocket— in the form of a phone.
He opens the message, decrypting in the background and waiting patiently for each letter to start forming on his screen. On the other hand, the princess is giggling and without a doubt pleased by the surprisingly detailed message she received and after a series of furious assault on the keyboard, she leaves her seat for Gintoki.
Silence washes over the room.
Ten miserable lines that is barely sufficient for the silver perm.
"Is that really all she has to say?" Gintoki's voice drop down at near the subzero temperature and messes his hair in sheer frustration. "'Gin-chan I have a real room now'? I scrambled off all my pachinko money to pay for her food!"
"But I think Kagura-chan is doing her best." Soyo pipped in, her finger gently traced the screens, kanji by kanji, "your message doesn't have a lot of errors. The one she sent me is full of them, but I'm her best friend so it's ok. She probably wants you to be proud of her, Sakata-san."
Gintoki suddenly falls in a silence that Okita interpret as one desperate attempt at not tearing up in front of a princess and a rather sadistic policeman. All is well for the two of them, and they seem to be satisfied of the messages China sent but he, unfortunately, cannot say the same.
Okita stares blankly at the meagre,  a mere three characters worth one-liner that is plenty enough to fill his body with confusion and anger, his irritation howling the deep of his heart in the form of soon-to-explode veins.
LOL
That brat.
He nearly crushes his precious, millions-of-yens-worth phone into thousand pieces as a wave of disappointment settles in his stomach. He doesn't know what he expected yet he's still angered by all the carelessness transpiring from the message yet a part of him knows it's just the way she is.

The next presidential primary contests are more than a year away. But presumed candidates are already trying to stake a claim to one of health care’s hot-button concerns: surging prescription drug prices.
“This is a 2020 thing,” said Dr. Peter Bach, who directs the Center for Health Policy and Outcomes at Memorial Sloan Kettering Cancer Center in New York and tracks drug-pricing policy.
Spurred on by midterm election results that showed health care to be a deciding issue, lawmakers — some of whom have already launched presidential run exploratory committees — are pushing a bevy of new proposals and approaches.
Few if any of those ideas will likely make it to the president’s desk. Nevertheless, Senate Democrats eyeing higher office and seeking street cred in the debate are devising more innovative and aggressive strategies to take on Big Pharma.
“Democrats feel as if they’re really able to experiment,” said Rachel Sachs, an associate law professor at Washington University in St. Louis who tracks drug-pricing laws.
Some Republicans are also proposing drug-pricing reform, although experts say their approaches are generally less dramatic.
Here are some of the ideas either introduced in legislation or that senators’ offices confirmed they are considering.
Make a public option for generic drugs. The government could manufacture generics (directly or through a private contractor) if there is a shortage or aren’t enough competitors to keep prices down. This comes from a bill put forth by Sen. Elizabeth Warren (D-Mass.) and Rep. Jan Schakowsky (D-Ill.).
Let Medicare negotiate drug prices. This idea has many backers — what differs is the method of enforcement. Sen. Sherrod Brown (D-Ohio) has suggested that if the company and the government can’t reach an agreement, the government could take away the company’s patent rights. A proposal from Sen. Bernie Sanders (I-Vt.) and Rep. Elijah Cummings (D-Md.) would address stalled negotiations by letting Medicare pay the lowest amount among: Medicaid’s best price, the highest price a single federal purchaser pays or the median price paid for a specific drug in France, the United Kingdom, Germany, Japan and Canada.
Pay what they do abroad. Legislation from Sanders and Rep. Ro Khanna (D-Calif.) would require companies to price their drugs no higher than the median of what’s charged in Germany, Japan, France, the United Kingdom and Canada. If manufacturers fail to comply, other companies could get the rights to make those drugs, too. Penalize price-gouging. This would target manufacturers who raise drug prices more than 30 percent in five years. Punishments could include requiring the company to reimburse those who paid the elevated price, forcing the drugmaker to lower its price, or charging a penalty up to three times what a company received from boosting the price. Backers include Sens. Richard Blumenthal (D-Conn.), Kamala Harris (D-Calif.), Jeff Merkley (D-Ore.) and Amy Klobuchar (D-Minn.).
Import drugs. A Sanders-Cummings bill would let patients, wholesalers and pharmacies import drugs from abroad — starting with Canada, and leaving the door open for some other countries. Sen. Chuck Grassley (R-Iowa) and Klobuchar have a separate bill that is specific to patients getting medicine from Canada alone. Abolish “pay-for-delay.” From Grassley and Klobuchar, this legislation would tackle deals in which a branded drugmaker pays off a generic one to keep a competing product from coming to market.
This flurry of proposed lawmaking could add momentum to one of the few policy areas in which conventional Washington wisdom suggests House Democrats, Senate Republicans and the White House may be able to find common ground.
“Everything is up in the air and anything is possible,” said Dr. Walid Gellad, co-director of the Center for Pharmaceutical Policy and Prescribing at the University of Pittsburgh. “There are things that can happen that maybe weren’t going to happen before.”
And there’s political pressure. Polls consistently suggest voters have a strong appetite for action. As a candidate, President Donald Trump vowed to make drug prices a top priority. In recent months, the administration has taken steps in this direction, like testing changes to Medicare that might reduce out-of-pocket drug costs. But Congress has been relatively quiet, especially when it comes to challenging the pharmaceutical industry, which remains one of Capitol Hill’s most potent lobbying forces.
One aspect of prescription drug pricing that could see bipartisan action is insulin prices, which have skyrocketed, stoking widespread outcry and could be a target for bipartisan work. Warren’s legislation singles out the drug as one the government could produce, and Cummings has already called in major insulin manufacturers for a drug-pricing hearing later this month. In addition, Rep. Diana DeGette (D-Colo.), the new chair of the House Energy and Commerce Oversight and Investigations Subcommittee, has listed prescription drug pricing as a high priority for her panel. As co-chair of the Congressional Diabetes Caucus, DeGette worked with Tom Reed (R-N.Y.) to produce a report on the high cost of insulin.
To be sure, some of the concepts, such as drug importation and bolstering development of generic drugs, have been around a long time. But some of the legislation at hand suggests a new kind of thinking.
House Speaker Nancy Pelosi (D-Calif.) has labeled drug pricing a top priority, and the pharmaceutical industry has been bracing for a fight with the new Democratic majority.
Meanwhile, in the GOP-controlled Senate, two powerful lawmakers — Sen. Lamar Alexander (R-Tenn.) and Grassley — have indicated they want to use their influence to tackle the issue. Alexander, who chairs the Health, Education, Labor and Pensions Committee, has said cutting health care costs, including drug prices, will be high on his panel’s to-do list this Congress. Grassley runs the Finance Committee, which oversees pricing issues for Medicare and Medicaid.
“The solution to high drug prices is not just having the government spending more money. … You need to look at prices,” Gellad said. “These proposals deal with price. They all directly affect price.”
Given the drug industry’s full-throated opposition to virtually any pricing legislation, Sachs said, “it is not at all surprising to me to see the Democrats start exploring some of these more radical proposals.”
Still, though, Senate staffers almost uniformly argued that the drug-pricing issue requires more than one single piece of legislation.
For instance, the price-gouging penalty spearheaded by Blumenthal doesn’t stop drugs from having high initial list prices. Letting Medicare negotiate doesn’t mean people covered by other plans will necessarily see the same savings. Empowering the government to produce competing drugs doesn’t promise to keep prices down long term and doesn’t guarantee that patients will see those savings.
“We need to use every tool available to bring down drug prices and improve competition,” said an aide in Warren’s office.

It's also interesting to note that Kaiser sees this game as a battle between him and Isagi, and truthfully in the beginning it definitely seemed like one. But now, Isagi is so fixated on just trying to beat Barou and stay ahead of the game, that he doesn't even seem to care about Kaiser or Ness or any of the silly drama they had in the previous chapters.
I wrote earlier in my post about how "Kaiser doesn't know what Isagi's thinking" that Kaiser misunderstands every little action Isagi takes in games. He presumes that Isagi is anxious when he gets quiet, when it's the opposite. He seemingly has no idea how him and Isagi keep going for the ball at the same time. He's annoyed because they're always on the same page, and while Isagi sees that as the two of them working together, Kaiser only sees competition. He sees someone who is directly competing against him rather than someone he could potentially work together with.

And it's also cool how in the above image, Isagi was thinking this about someone else entirely, but its framed like the two of them are facing off against one another. Essentially Kaiser is losing to Isagi in this game, there's no doubt about that. And the reason for it is because Isagi is so fixated on other players, that he won't even pay Kaiser any attention during the game.
Isagi isn't using Kaiser to make plays, or stealing balls from Kaiser, and I don't think they've even spoke to one another that often during the whole match. With that in mind, it does make sense no why Kaiser looks like he's ready to murder someone in that panel because he's stuck in a one sided rivalry with someone who he believed was easy to beat.

And here kaiser is again making predictions for what he thinks Isagi will do. So far in the game he has been right about what plays Isagi will make, but that's just because the two of them are thinking the same things (so they incidentally end up playing the same way). Kaiser might only be thinking this is what Isagi's plan is because that's what he would do.
But we know already from Isagi's conversation with Hiori that our star player...really has no fucking clue what they're gonna do next lmao:

Maybe if they finally work together in this game they have a chance of winning it. Bastard Munchen is SO disorganized though (by design) with different players all trying to just beat one another alongside the other team. They need to be able to at least see the field in the same way or else they won't be able to keep up.

California To Widen Pipeline Of Psychiatric Nurse Practitioners
Jane Gunter, a nurse practitioner in Tuolumne County, California, has long wanted to specialize in mental health so she can treat patients who have anxiety, depression and more complicated mental illnesses.
Her county, a rural outpost in the Sierra Nevada foothills with a population of about 54,000, has only five psychiatrists — “a huge shortage,” she said.
But Gunter, 56, wasn’t about to quit her job at the Me-Wuk Indian Health Center in Tuolumne and relocate to some distant campus for two years to get certified as a psychiatric nurse practitioner.

Then, in February, she learned that the University of California was launching a new program to provide that certification online in just one year. She fired off her application, and last month she received an acceptance letter.
“Sometimes I think, ‘What are you doing?’” Gunter said, referring to the online classes that will take over her nights and weekends once the program starts. “But I care about the community.”
The online certification program, conducted jointly by the nursing schools at the University of California-San Francisco, UCLA and UC-Davis, was scheduled to start in the fall, but it has been postponed until January because the on-site clinical hours required as part of the training are not possible during the COVID-19 shutdown.
Despite the delay, the potential expansion of psychiatric care is opportune given the expected increase in mental health problems due to the social isolation and financial stress stemming from the pandemic.
The need for more mental health nurses is about to be bigger than ever, said E. Alison Holman, a health psychologist at UC-Irvine who studies emotional responses to collective trauma.
“We now have 30 million Americans who have lost their jobs, who have no income — and how are they going to pay rent? How are they going to buy food?” Holman said. “And then you have to stay home. This event is rolling out like a long, chronic stressor.”
Even before the current crisis, California faced a serious shortage of mental health professionals, especially in rural areas. California’s psychiatrists and psychologists are approaching retirement age in large numbers, and fewer medical students are choosing psychiatry.
A UCSF study projected that the state would have 41% fewer psychiatrists than needed by 2028. More than half of Californians with mental illness receive no treatment, according to a February 2019 report by the California Future Health Workforce Commission.
UC’s online mental health nurse practitioner program is one of the solutions recommended by the commission, a statewide, multisector panel that created a master plan to address the Golden State’s shortage of health care workers.
The program is expected to put 300 more mental health NPs into California communities, particularly rural ones, over the next five years. Applicants such as Gunter, who live in underserved rural areas, will be given priority in the hope that they will stay in their communities upon completion of the training, said Deborah Johnson, a UCSF nursing professor who is co-director of the program.
Forty spots are available for the class that begins in January, and not all have been filled yet, Johnson said. Applications are being accepted until June 1, and 65 additional spots will open in fall 2021 and each fall thereafter for three more years, she said.
The UC system received a $1.5 million grant from the California Health Care Foundation to develop, design and launch it. But tuition is expected to make it self-sustaining. (Kaiser Health News, which produces California Healthline, is an editorially independent publication of the foundation.)
Applicants for the new program must already be advanced practice nurses, which means they hold either a master’s degree or doctorate in nursing. More than 27,000 NPs now practice in California, but only 1,200 are certified to treat psychiatric patients.
Three hundred more psychiatric NPs won’t completely fill the growing mental health care need, but they are expected to treat nearly 400,000 patients over a five-year period.
Though the online program means working nurses won’t have to leave their jobs and their lives to relocate, they will still face challenges.
For one thing, their certification will require 500 hours of supervised clinical training with patients in hospitals, jails or schools. And some applicants live in communities where such opportunities may not be available, which could require them to commute long distances to meet the requirement.
Another challenge is that, even after nurse practitioners are certified, state law requires they find a medical doctor to supervise them. Havilyn Kern, a school nurse in Nevada City, California, quit her job two years ago so she could spend three days a week at UCSF — 155 miles away — to train as a psychiatric nurse practitioner.
She graduates in June, so the new online program is too late for her. Kern, who plans to work in her own community, hopes she will find a psychiatrist in the Bay Area willing to tele-supervise her.
“It shouldn’t have to be this way,” said program co-director Johnson. “California is so archaic. It’s the most restrictive state in the western portion of the country.”
Twenty-eight states plus Washington, D.C., allow nurse practitioners to work autonomously. Santa Rosa Assembly member Jim Wood, a Democrat, has introduced a bill, AB-890, that would allow California NPs to practice without doctor supervision. It passed the Assembly in January and is pending in the Senate.
“If AB 890 passes, it will certainly help fill the loss of specialty physicians such as psychiatrists everywhere, including in underserved areas,” Wood said.
But that’s a big “if.”
California’s powerful doctors’ lobby, which has repeatedly scuttled similar legislation, is aggressively fighting it again. They argue that letting NPs order tests and prescribe medications independently would “dilute care.”
Doctors also have a financial incentive to keep things the way they are. It restricts competition, and they bill NPs between $5,000 and $15,000 a year to review their charts and prescriptions every few months, according to a report by the California Health Care Foundation and UCSF.
Johnson suggested it is time for a change.
“We are the workhorses,” she said. “Oh, my God, there is so much need. This new program could not come at a more important time.”

“Telehealth is a digital distribution channel for health care” – catching up with Roy Schoenberg, President and CEO of American Well
Ten years ago, two brothers, physicians both, started up a telemedicine company called American Well. They launched their service first in Hawaii, where long distances and remote island living challenged the supply and demand sides of health care providers and patients alike.
It’s ten years later, and I sat down for a “what’s new?” chat with Roy Schoenberg, American Well President and CEO. In full transparency,  I enjoy and appreciate the opportunity to meet with Roy (or very occasionally Ido, the co-founding brother-other-half) every year at HIMSS and sometimes at CES. In our face-to-face brainstorm this week, we covered a wide range of topics, starting with some historical context to get to the “new and now.”
That launch of AmWell in Hawaii was with a payer, a unit of Blue Cross. The company’s early bet and first strategic choice was to work with payers. That segment is a significant business now, Roy explained. The company works with virtually every major payer-player, and 45 Blue Cross Blue Shield plans as well. This is a core business for AmWell.
The company’s second strategic choice was to “put the technology in others’ hands,” Roy described, with AmWell’s services, “baked into other businesses.” Doing so in this way could help health/care ecosystem stakeholders to innovate their own business and pricing models. AmWell invested in making their own technology easier to tie into other businesses’ infrastructures and architectures. Consider the plethora of EHR systems, scheduling idiosyncrasies, physician adoption and workflow considerations, and patient matching challenges, for example.
On the health system (provider) side, there is substantial pressure to reduce costs and at the same time, provide innovative services. These services are increasingly ambulatory and mobile, based on both cost and consumer preferences. AmWell’s customers cover about 150 million lives. Don’t mistake that for “reach,” Roy warned me when I quickly calculated, “So, you reach 50% of the U.S. population.” It’s a potential consumer/patient market that AmWell could serve, but not every person with access to telehealth services takes advantage of that supply side.
Remember the Field of Dreams effect? “If you build it, they will come?” In telehealth, even with employers covering the service as a workplace benefit, most employees still don’t “benefit from the benefit.” We discussed this disconnect on our panel today at HIMSS19, addressing digital traction in consumerization of health care.
Dr. Peter Rasmussen explained that there are many reasons consumers don’t yet engage in virtual health services. In some systems, like Kaiser-Permanente, at least 50% of encounters are done via non-traditional channels: via secure email with clinicians, via video chat, and asynchronous chat, for example. Intermountain and Mercy Virtual, too, conduct lots of visits via telehealth means. However, some recent market research estimates have identified very low, single digit consumer market adoption.
A major limiting obstacle for providers has been licensure, where a doctor peer of Peter’s in Cleveland cannot provide a second opinion for a patient in rural Kentucky for a rare cancer due to the Ohio physician’s lack of license in the Bluegrass State. I noted that in the EU, this isn’t a problem for physicians operating in different countries, from the far north to the Mediterranean south.
On the consumer side, a Jetsons-style telehealth visit with a clinician on a screen can seem too Star Trek tricordery, whether small on a phone or on a large-screen PC monitor. But once a patient-consumer experiences that virtual visit, most people want to repeat the telehealth encounter. Here, Jane Jetson is very happy to be able to access a telehealth visit for her son, Elroy — and true enough, back here in the digital health Stone Age, Moms with young children are particularly keen on virtual visits especially on nights and weekends.
To smooth organizations’ way to offer telehealth, American Well offers an SDK, a software development kit. This package drops into the client’s technology. Samsung, for example, build an American Well ap into their phone. New York Presbyterian Hospital has an app for telehealth from American Well. AmWell SDKs have also been used by Medtronic, Cerner, and Philips, among others. The Apple Heart Study used the SDK.
“American Well is like the Intel inside,” Roy agreed with my analogy.
In a new HIMSS19 announcement, Netsmart uses the AmWell SDK for a behavioral health opioid program, announced today.
On our panel today, Greg Orr from Walgreens, Jessica Gelzer from Heal, and Dr. Rasmussen of Cleveland Clinic discussed the blurring between the traditional physician and the newer digital side. In retail , this is happening, with the poster child of Amazon invoked at every health care meet-up over the past year and ongoing.
I believe this is “omnichannel” in retail, and it’s happening in health care deliery, too.
Roy’s Amazon-analogy pointed me to the concept of “inventory.” Amazon shows us that stuff doesn’t have to live on Amazon’s warehouse shelves. “You don’t need to own it, but to route it,” Roy explained.
So for that industry disruptor, the key tactic is to develop and implement skills around inventory distribution.
Thus, the quote in the title of this post: as Roy asserted, “telehealth is the distribution channel for health care services.”
And here’s the next money quote and lightbulb moment: “We have to let go; we can’t be the product.”
In sense of the services available in the “inventory,” those, too, have expanded since the start of offering a physician visit. Today, specialists are part of the AmWell physician network, as well as womeen’s health experts, nutrition counselors, weight loss coaches, lactation support, and the fast-growing area and need on the demand side for behavioral health.
 Health Populi’s Hot Points:  Live telemedicine visits are more prevalent among younger urban-dwelling consumers, with lower adoption among rural, older residents, Rock Health learned in its fourth and lattt digital health consumer survey.
This means that some of the very people who most need access to health care services — rural, older, frailer health citizens — aren’t tapping into virtual visits. As Dr. Rasmussen pointed out, there can be may reasons a person doesn’t engage in telehealth: cultural, financial, or trust/privacy. I would also add in the role of of connectivity and broadband as a social determinant of health, clearly a barrier to adoption in rural areas that can lack connection to the last mile, and to the last person/patient.
In the growing health ecosystem landscape of retail health for patients-as-payors, attending to social determinants of health is at least as important as paying attention to health care services and specialty care access. I expect that AmWell will expand its footprint and services from the nutrition and weight loss areas to other behavioral and social factors that play so integrally into peoples’ overall health and wellness.
At HIMSS19 we see some exhibitors that will support this kind of consumer engagement and ecosystem build-out: AWS (Amazon Web Services), Google, Microsoft (with announcements for the Azure cloud collaborating with Walgreens, for example), and Salesforce bringing CRM capabilities to health and social services.
Ten years from now — even three years from now — I cannot forecast  what new-new collaborations and services Roy and I will talk about. I’m certainly doing scenario planning on this for my advisory work.
But of one thing I am sure: in that decade-away scenario, the home will certainly be the hub of health.

But of one thing I am sure: in that decade-away scenario, the home will certainly be the hub of health.

“Telehealth is a digital distribution channel for health care” – catching up with Roy Schoenberg, President and CEO of American Well
Ten years ago, two brothers, physicians both, started up a telemedicine company called American Well. They launched their service first in Hawaii, where long distances and remote island living challenged the supply and demand sides of health care providers and patients alike.
It’s ten years later, and I sat down for a “what’s new?” chat with Roy Schoenberg, American Well President and CEO. In full transparency,  I enjoy and appreciate the opportunity to meet with Roy (or very occasionally Ido, the co-founding brother-other-half) every year at HIMSS and sometimes at CES. In our face-to-face brainstorm this week, we covered a wide range of topics, starting with some historical context to get to the “new and now.”
That launch of AmWell in Hawaii was with a payer, a unit of Blue Cross. The company’s early bet and first strategic choice was to work with payers. That segment is a significant business now, Roy explained. The company works with virtually every major payer-player, and 45 Blue Cross Blue Shield plans as well. This is a core business for AmWell.
The company’s second strategic choice was to “put the technology in others’ hands,” Roy described, with AmWell’s services, “baked into other businesses.” Doing so in this way could help health/care ecosystem stakeholders to innovate their own business and pricing models. AmWell invested in making their own technology easier to tie into other businesses’ infrastructures and architectures. Consider the plethora of EHR systems, scheduling idiosyncrasies, physician adoption and workflow considerations, and patient matching challenges, for example.
On the health system (provider) side, there is substantial pressure to reduce costs and at the same time, provide innovative services. These services are increasingly ambulatory and mobile, based on both cost and consumer preferences. AmWell’s customers cover about 150 million lives. Don’t mistake that for “reach,” Roy warned me when I quickly calculated, “So, you reach 50% of the U.S. population.” It’s a potential consumer/patient market that AmWell could serve, but not every person with access to telehealth services takes advantage of that supply side.
Remember the Field of Dreams effect? “If you build it, they will come?” In telehealth, even with employers covering the service as a workplace benefit, most employees still don’t “benefit from the benefit.” We discussed this disconnect on our panel today at HIMSS19, addressing digital traction in consumerization of health care.
Dr. Peter Rasmussen explained that there are many reasons consumers don’t yet engage in virtual health services. In some systems, like Kaiser-Permanente, at least 50% of encounters are done via non-traditional channels: via secure email with clinicians, via video chat, and asynchronous chat, for example. Intermountain and Mercy Virtual, too, conduct lots of visits via telehealth means. However, some recent market research estimates have identified very low, single digit consumer market adoption.
A major limiting obstacle for providers has been licensure, where a doctor peer of Peter’s in Cleveland cannot provide a second opinion for a patient in rural Kentucky for a rare cancer due to the Ohio physician’s lack of license in the Bluegrass State. I noted that in the EU, this isn’t a problem for physicians operating in different countries, from the far north to the Mediterranean south.
On the consumer side, a Jetsons-style telehealth visit with a clinician on a screen can seem too Star Trek tricordery, whether small on a phone or on a large-screen PC monitor. But once a patient-consumer experiences that virtual visit, most people want to repeat the telehealth encounter. Here, Jane Jetson is very happy to be able to access a telehealth visit for her son, Elroy — and true enough, back here in the digital health Stone Age, Moms with young children are particularly keen on virtual visits especially on nights and weekends.
To smooth organizations’ way to offer telehealth, American Well offers an SDK, a software development kit. This package drops into the client’s technology. Samsung, for example, build an American Well ap into their phone. New York Presbyterian Hospital has an app for telehealth from American Well. AmWell SDKs have also been used by Medtronic, Cerner, and Philips, among others. The Apple Heart Study used the SDK.
“American Well is like the Intel inside,” Roy agreed with my analogy.
In a new HIMSS19 announcement, Netsmart uses the AmWell SDK for a behavioral health opioid program, announced today.
On our panel today, Greg Orr from Walgreens, Jessica Gelzer from Heal, and Dr. Rasmussen of Cleveland Clinic discussed the blurring between the traditional physician and the newer digital side. In retail , this is happening, with the poster child of Amazon invoked at every health care meet-up over the past year and ongoing.
I believe this is “omnichannel” in retail, and it’s happening in health care deliery, too.
Roy’s Amazon-analogy pointed me to the concept of “inventory.” Amazon shows us that stuff doesn’t have to live on Amazon’s warehouse shelves. “You don’t need to own it, but to route it,” Roy explained.
So for that industry disruptor, the key tactic is to develop and implement skills around inventory distribution.
Thus, the quote in the title of this post: as Roy asserted, “telehealth is the distribution channel for health care services.”
And here’s the next money quote and lightbulb moment: “We have to let go; we can’t be the product.”
In sense of the services available in the “inventory,” those, too, have expanded since the start of offering a physician visit. Today, specialists are part of the AmWell physician network, as well as womeen’s health experts, nutrition counselors, weight loss coaches, lactation support, and the fast-growing area and need on the demand side for behavioral health.
 Health Populi’s Hot Points:  Live telemedicine visits are more prevalent among younger urban-dwelling consumers, with lower adoption among rural, older residents, Rock Health learned in its fourth and lattt digital health consumer survey.
This means that some of the very people who most need access to health care services — rural, older, frailer health citizens — aren’t tapping into virtual visits. As Dr. Rasmussen pointed out, there can be may reasons a person doesn’t engage in telehealth: cultural, financial, or trust/privacy. I would also add in the role of of connectivity and broadband as a social determinant of health, clearly a barrier to adoption in rural areas that can lack connection to the last mile, and to the last person/patient.
In the growing health ecosystem landscape of retail health for patients-as-payors, attending to social determinants of health is at least as important as paying attention to health care services and specialty care access. I expect that AmWell will expand its footprint and services from the nutrition and weight loss areas to other behavioral and social factors that play so integrally into peoples’ overall health and wellness.
At HIMSS19 we see some exhibitors that will support this kind of consumer engagement and ecosystem build-out: AWS (Amazon Web Services), Google, Microsoft (with announcements for the Azure cloud collaborating with Walgreens, for example), and Salesforce bringing CRM capabilities to health and social services.

Making Sense of the Health Care Merger Scene

In the past 12 months, there has been a raft of multi-billion-dollar mergers in health care. What do these deals tell us about the emerging health care landscape, and what will they mean for patients/consumers and the incumbent actors in the health system?
Health Systems
There have been a few large health system mergers in the past year, notably the $11 billion multi-market combinations of Aurora Health Care and Advocate Health Care Network in Milwaukee and suburban Chicago, as well as the proposed (but not yet consummated) $28 billion merger of Catholic Health Initiatives and Dignity Health. However, the bigger news may be the several mega-mergers that failed to happen, notably Atrium (Carolinas) and UNC Health Care and Providence St. Joseph Health and Ascension. In the latter case, which would have created a $45 billion colossus the size of HCA, both parties (and Ascension publicly) seemed to disavow their intention to grow further in hospital operations. Ascension has been quietly pruning back their operations in markets where their hospital is isolated, or the market is too small. Providence St. Joseph has been gradually working its way back from a $500 million drop in its net operating income from 2015 to 2016.
Another notable instance of caution flags flying was the combination of University of Pittsburgh Medical Center (UPMC) and PinnacleHealth, in central PA, which was completed in 2017.   Moody’s downgraded UPMC’s debt on the grounds of UPMC’s deteriorating core market performance and integration risks with PinnacleHealth. As Moody’s action indicates, investor skepticism about hospital mega-mergers is escalating. Federal regulators remain vigilant about anti-competitive effects, having scotched an earlier Advocate combination with NorthShore University HealthSystem in suburban Chicago. The seemingly inevitable post-Obamacare march to hospital consolidation seems to have slowed markedly.
However, the most noteworthy hospital deal of the last five years was a much smaller one: this spring’s acquisition of $1.7 billion non-profit Mission Health of Asheville, NC, by HCA. This was remarkable in several respects. First, it was the first significant non-profit acquisition by HCA in 15 years (since Kansas City’s Health Midwest in 2003) and HCA’s first holdings in North Carolina.  While Mission’s search for partnerships may have been catalyzed by a fear of being isolated in North Carolina by the Atrium/UNC combination, Mission Health certainly controlled its own destiny in its core market, with a 50% share of western North Carolina. Mission was not only well managed, clinically strong and solidly profitable, but its profits rose from 2016 to 2017, both from operations and in total.
Precisely because it was not a distress sale, and because Mission was in an unassailable market position, this deal should have sent shockwaves through the non-profit hospital industry. Yet, there was remarkably little public discussion of its significance. There was no burning platform here. Rather, the ability of HCA to lower Mission’s operating expenses with its austere management culture and break even on Medicare may have been viewed as a key to long-term sustainability by Mission’s board, as well as access to HCA’s more-or–less bottomless capital pool.
HCA’s willingness to be patient and wait for the right deals, and crucially, its ability to break even at Medicare rates, are the real sources of its long-term strength. It may well be that HCA’s ability NOT to follow the herd, and to decide which assets, markets, and relationships make sense long term is more valuable than mass and scale. The Rick Scott Columbia HCA had 360 hospitals at “peak roll-up.” The present, better focused HCA is a much stronger company at half the number of hospitals.
Implications
So many large non-profit and investor-owned health systems formed as roll-ups of smaller enterprises are struggling to generate operating earnings just now, including many prominent market leading systems. For this reason, many other potential roll-ups in the vein of Ascension-Providence and Atrium-UNC might not survive the courtship stage. Those roll-ups might actually weaken the combined enterprise by burdening them with hospitals that could not have survived on their own and which probably should close.  Bigger may no longer equal stronger in hospital management.
It has never been clear how actual patients would benefit from vastly greater scale of hospital operations. The burden of proof is on the industry that patients will notice a difference in service quality or lower prices from further consolidation of hospital systems. It is not clear that benefits to patients or their physicians has played any meaningful role in the flurry of post-Obamacare deals.
Physicians – Is Vertical Integration Inevitable?
In December 2017, United HealthGroup’s $100 billion subsidiary Optum purchased the troubled DaVita Medical Group for $4.9 billion. This deal set off a frenzy of speculation that United was positioning itself to become the next Kaiser. Industry pundits opined that Optum and United will transform itself into a closed panel vertically integrated care system that would enable United to sell a comprehensive exclusive care system product. I believe this is not a strong likelihood.
Optum’s first entry into the physician group business was opportunistic, obtaining a captive physician delivery system in Nevada as part of United’s 2008 acquisition of Sierra Health Plan.  The physician group asset did not belong in the health plan part of United and was therefore lodged in Optum as a one-off. Subsequent Optum acquisitions in California, Texas and Florida consisted of successful risk contracting Independent Practice Associations with significant and diverse (e.g. non-United) contracts. Some of those IPAs had a core multi-specialty employed medical group at its core. Optum’s early strategy was not a “physician employment” strategy, but rather not dissimilar to that of MedPartners or Phycor in the 1990’s: buying risk-bearing contracts through the acquisition of physician enterprises that had negotiated them.
Obamacare was expected to catalyze a wave of capitation. Owning risk-bearing physician groups was an asset-light way of playing this presumed shift to capitation. However, the expected post-ACA surge in delegated risk contracting did not materialize. Optum ceased buying care system assets in 2012 because the bidding for physician groups, particularly from health systems, had gotten out of hand. They resumed buying in 2016, adding urgent care centers and ambulatory surgical centers to the portfolio, in addition to the DaVita deal.
While some have claimed that Optum now employs 47,000 physicians, this number seems more likely to be the sum of its IPA networks.  The actual employed physician cadre is probably more like 15 thousand, a number smaller than the combined Permanente Medical Groups inside Kaiser.  There are roughly a million licensed physicians in the United States.
Presently, Optum has care system assets in markets which contain 70% of the US population, but there is limited “integration” among the care system assets, or between Optum and United’s Health Insurance operations. Obviously, United’s health insurance subscribers can use Optum’s group physicians. But Optum patients are not required to or even encouraged to use United’s health insurance products. It would damage the Optum care system asset value to exclude other insurers from paying Optum for a physician or ambulatory care.
Despite its large footprint, I believe that Optum’s strategy in the physician space is disciplined but opportunistic “conglomerate” style diversification. In only two markets, greater Los Angeles and San Antonio, does Optum have a significant local market share in the risk-bearing care system market? Optum has not shown any interest in canceling the substantial number of non-United network contracts and going “closed panel.” Nor is there yet evidence of a backlash from non-United insurers in anticipation of a closed panel strategy that would cause United’s health insurance competitors to shun contracting with Optum care system assets. United/Optum has more to lose than to gain in contracting advantage by closing their panels.
Optum is also unlikely to diversify into the slow growing hospital business. Despite a “buyers’ market” for hospital-based physician enterprises like Envision, Team Health, and MedNax, Optum has thus far studiously avoided acquiring hospital-linked assets. Rather, it is capable of surrounding hospitals with low-cost alternatives and stepping in front of them where possible as risk bearing physician-based care systems, leaving hospitals in those markets, as one analyst put it, as “stranded assets.” We will be watching the “integration” of these diverse Optum assets closely but are skeptical that “integration” will yield significant earnings or growth potential.
Implications
Regardless of who owns their physicians, a significant fraction of Americans will need to use the hospital as they age, and an increasing percentage will be publicly funded. Though successfully organized physicians can rigorously minimize the use of the hospital by substituting lower cost non-hospital alternatives (e.g. in surgery and imaging), the residual demand for hospital care related to complex conditions and for the fragile elderly seems likely to grow, not shrink, in years ahead.
The challenge hospitals face is making money at publicly funded rates and driving out the unnecessary or inappropriate use of its services. Hospitals can learn from Optum’s long time horizons, its market-by-market pragmatism about organizational models and insistence on deals being “accretive” rather than “mission driven.” Strategic discipline is the best response to the threat posed by Optum and other organizers of physician care.
Consumers may or may not be willing to “bond” with a corporate giant like Optum. They are likely to make their decisions about where they get their physician care based on responsiveness to their needs and the strength of the physician relationships that develop.
Optum seems unlikely to noticeably lower the cost of physician care to patients, as there are yet no demonstrable economies of scale in physician services.
Pharma Distribution: The “Amazon is Coming” Freak-out
In December 2017, CVS, the nation’s largest drugstore chain, and Aetna, the nation’s fifth largest health insurer, announced a $69 billion merger. Aetna had been blocked from its planned acquisition of rival Humana over anti-trust concerns. But CVS, the acquirer, had a much larger and more urgent concern – the mooted entry of Amazon into the pharmaceutical supply chain, either through wholesale distribution, direct-to-consumer strategy or both.
As its retail sales have slowed, CVS has become increasingly dependent on their CVS-Caremark
pharmaceutical benefits management (PBM) business both for revenue and earnings growth. The entire complex and costly US pharmaceutical supply chain is buckling under the financial pressure created by rising drug spending. The PBM business model has come under increasing regulatory scrutiny over concerns over lack of transparency and that PBM rebates negotiated with pharmaceutical firms do not seem to be reaching consumers. In the Aetna transaction, CVS looked to diversify out of its two main businesses to re-establish growth and establish closer and more comprehensive relationships with corporate customers.
Of course, retail in all its forms is being disrupted by Amazon. That Amazon might disrupt the pharmaceutical market by selling directly to consumers became a good deal less speculative with Amazon’s recent $1 billion acquisition of PillPack. BOTH of CVS’s current businesses may be in Jeff Bezos’ crosshairs.
Having said this, the CVS Aetna combination is an “out of the frying pan-into the fire” merger. CVS will discover that the health plan business is actually an extremely fragile web of short-term contracts between the insurer and employers, as well as between the insurer and its care networks. Many of these latter contracts may not be renewed under their present terms, which have been highly favorable to and profitable for insurers. This is because many care systems have been bitterly disappointed by the lack of return to them from the deep front-end discounts made in those contracts in anticipation of rapid growth in “narrow network” lives which have not materialized.
Health insurance is nearing the end of an exceptional profit cycle begun during the roll-out of Obamacare. The creation of health exchanges and the new narrow network contracts designed for them catalyzed a 2010-2014 hospital pricing panic similar to that which ensued on the rollout of PPOs in the mid-1990s. This pricing panic has damaged hospital system earnings and prevented them from recouping escalating losses from Obamacare Medicare rate concessions and the 2012 federal budget “sequester,” which cut Medicare rates by 2% annually going forward.
As with the Optum-DaVita combination, much has been made of the “vertical integration” aspect of Aetna having access to CVS’ network of instore clinics. CVS’s clinical assets – its 1,100 Minute Clinics – are more “nurse in a broom closet” than “doc in the box.” Fully loaded with CVS’s hefty corporate overhead, the Minute Clinics probably lose $20 a visit, with the fond hope of making some of it up on shampoo sales. Despite outgoing Aetna CEO Mark Bertolini’s vision of the CVS clinics as a health care equivalent of Apple’s Genius Bar, CVS/Aetna won’t be a credible player in disease management or anything else complicated by relying on a spindly network of nurse-driven instore clinics. As they did before the deal, consumers will find in CVS’s clinics a great place to get flu shots and back to school physicals, though.
It is also not clear how either business will grow as a result of the combination. CVS Aetna’s consolidation won’t lower the cost of health care for Aetna’s members or corporate clients, nor bring Aetna new health benefits customers.  Though Aetna has a number of large national accounts, it remains a marginal player in most large geographical markets, the venue where bargaining clout really matters. Having a bunch of drugstores and a PBM will not increase Aetna’s leverage with its care networks, hospital or physician. It will also not materially lower Aetna’s drug spend.
On the CVS side, merging with Aetna won’t drive more customers into CVS’s stores, or bring them any additional PBM business, because CVS/Caremark already managed Aetna’s pharmacy claims. And it might lose CVS the recently announced pharmacy benefits management deal with Aetna’s competitor, Anthem, that looked for a new PBM after dumping Express Scripts. There are no good reasons why Anthem would want to contract with a PBM owned by a competitor to their core business.
Implications
Hospitals ought not to be threatened by the CVS-Aetna combination, nor the copycat CIGNA-Express Scripts deal that followed it. Neither is likely to affect the prices paid for the specialty IV drugs that have pressured hospitals in the past several years.
Amazon’s core strengths – merchandising clout, logistics and cloud computing – are minimally relevant to health care provision. Amazon has no significant presence in any service business at the present time, other than cloud computing. But as suggested earlier, the pharmaceutical supply chain is ripe for disruption. Anything that lowers the cost of drugs to patients or care givers will help both cope with tightening cash flows and be welcomed by all.
While Amazon’s future incursion into health care remains “notional” at this point, the spate of deals that have been spawned by the mere potential of its entry into the pharmaceutical business resembles nothing so much as one of those chain reaction freeway collisions, where the first driver was distracted by the sight of a large moose walking out of the woods and up to the roadway. It is worth noting that other tech company invasions of the so-called “health care vertical”- Apple, IBM, Microsoft, Google – have not gone very well.
The Future of Mega-Medicine
In his 2012 book Anti-Fragile: Things that Gain from Disorder, finance guru Nassim Taleb makes a convincing argument that scale and the search for security in the corporate and financial world actually increased those institutions’ fragility and exposure to franchise risk. The reciprocal drive of health systems and health insurers, in particular, to become larger and more “unavoidable” may, ironically, have made them more, rather than less, vulnerable to economic shocks. This includes the effects of the inevitable economic downturn that awaits the American economy in the next year or two.  Larger health care organizations are inevitably more bureaucratic and take far longer to make decisions.
On the narrower issue of “integration,” the economic literature on the effectiveness or economic benefits of vertical integration in health care is remarkably devoid of evidence of consumer or societal benefits, or even benefits to the organizations themselves  https://www.nasi.org/research/2015/integrated-delivery-networks-search-benefits-market-effects.
Health care remains the most intimate personal service in the US economy. Health care organizations that wish to consolidate are increasingly constrained by the legal and political consequences of their actions. They are also increasingly tempting targets for the hostile populist sentiments accumulating on both the left and right sides of the political spectrum.
The lack of evidence of measurable consumer benefits and the rising risks haven’t yet stopped the wave of consolidation in health care. Despite the pro-merger puffery of prominent strategy consulting firms and bankers, it remains to be seen if $50 billion-plus mega-corporations can connect with real people on a consistent basis and deliver measurable benefits that meaningfully affect their health.

I'm 27 years old. I live in the Bronx, New York. I have good credit. TransUnion: 765 Experian: 764 Equifax: 734 I want to financial or lease (I'm not exactly sure what the difference is. Do I still have to get full coverage regardless of which I choose?) a 2009 Benz C300. It's valued at about $30,000. I live in an apartment with my father who has a 2001 Pontiac Montana. He pays about $150-$200 per month in insurance. I tried to find out how much it would cost on Geico and they called me and told me it was gonna be $150 for the first month and $140 for the next seven months. I thought there must have been some mistake because how could it cost the same for a new car and raggedy car? The guy was like, Well, looks like we can get you a great price . Is this a ploy to get me to switch insurance companies and then they'll charge me higher rate after the first year or something? I think they must have thought I already own the car outright and didn't know I was planning on financing it. Or is it because we will have two cars from the same household an we'll get a multi-car discount. I know this was true when my brother had a car on the same policy with my dad and they got a discount, but they lived at separate places. Me and dad live together, so I think they won't even allow me to get insurance. Someone please tell if this is true or not. My friend, who thinks she knows it all, said its a scam and they will charge me alot more as soon as I sign up. But believe me she's just jealous cuz she's high class and I live in the Bronx and she makes way more than me and doesn't have a Benz. So, she is definitely just assuming. If you know for sure let me know you know or that you think that's what will happen. I assumed the insurance would be $400 - $450 per month. Hey, so I have my driving test on June 3rd but i am looking to buy a car before then in these next few weeks. Hi. Recently i changed my address so i had to let the car insurance company know about that. But they charged(actually are willing to chage) me 423 pounds !! Yes, there's not a misspeling. It's 423 . As far as i ve seen, most people get charged no maximum than 50 pounds for this change. Also, there are no changes on my policy. I still have a garage, i still do the same mileage/year and so on. They even confirmed that there's no change in policy details except the new address. The new address it's in Manchester (central area) and before i lived in Bradford(UK). Now, in the mean time, i had a claim. If i would have found about this fee before, i would have cancel my insurance policy but now that i have a claim, i can't cancel it anymore and being refunded (as i paid the insurance for 1 year in one go, Not monthly). I found about this crazy 423 fee only now when i had this claim. You can't find anything about any fees related to changing the address neither on my policy papers nor on their website and online policy books. I suppose they considered this fee just as an administration fee. And i suppose they could have charged me even 10 000 pounds as an administration fee just because they can. I don't feel protected by the company anymore !! that's why it's an insurance company, to protect and take some risks out. The insurance company it's Admiral. I tryed to talk and explain them that this fee it's just nuts but they didn't agreed. The fee it's about 1/4 of the total insurance cost for 1 year. Maybe they try to take advantage of the fact that i can't cancel it (coz i have a claim in process) and i can't get refunded ? Any ideea how should i proceed ? Ok so i currently DRIVE a 2005 Mazda 3. When I originally moved to Florida 3 years ago, My good friend, who I moved to florida to live with, so graciously loaned out the car for me. Three months ago, My friend moved to california with his Fiancee and I moved back to where i grew up in pennsylvania. I am a safe driver. I've never had a car accident or ticket. As a consequence of my choices, I pay very low rates for car insurance. Other people I know have had accidents and many tickets, they pay a lot for car insurance. In this system, each person pays a different rate based on the consequences of their choices and actions. So what is wrong with this system? Why can't we use the exact same system for health insurance? If I eat right, exercise, and wash my hands then I will rarely get ill and should pay very low insurance rates. My neighbor eats junk food, smokes, and never exercises, has lots of medical issues and will for as long as he lives, shouldn't he pay a much higher rate for insurance? I just recently passed my driving test and I'm completely taken away by the prices I've saw for car insurance on my parents Astra, I'm getting prices up at 7K. I've decided to go out and buy a category 1 insurance car (or any that's a low category) so it will be cheaper to run but the only problem is, I don't have a job at the moment so only have savings. It wouldn't bother me not taking out my own policy because I understand that can be more expensive but is there an alternative for me until I get a job and earn money to get the policy so I can collect NCD's? One of my friends use to have his own policy on his family car, a big car like ours, until he had an accident and the price shot up. Now because he can't afford the monthly costs he just takes out 1 weeks insurance at a top when needed and this costs him around 40. What I don't understand though is that he's 18, the same age as me, and I thought you werent able to do short term insurance as a new driver? Although I think he somehow takes it under his parents policy? I'v no idea how this works if someone could shine a light on it? Another friend also had a Nissan Micra and was paying around 80-90 per month which I thought was very cheap (also the same age as me) for an insurance which makes me wonder if he is on his own policy? I know there are a lot of things to take into consideration when getting insurance quotes and all but I was on craigslist and I found a nice little 2005 Audi A4 1.8t and I wanted to get an idea of insurance. I know that it is considered a luxury sedan and its foreign from Germany so those are going to make it more expensive. It puts out around 170hp and 160 trq. The car has great crash test ratings too. Seller is asking a little under $6000 and I don't know how many miles are on it yet. Other things to consider are that im only 19 so it would go under my fathers name. Basically my hubby took out insurance about 10 months ago with a new insurance company. He had 4 years no claims with his old company so informed the new company of this. The new insurance company have been taking out the payments every month from our bank however we have never had his insurance documents off them. When he contacted them the other day to query this they informed him that because the old insurance company will not supply them with no claims info they can not cover him. However all this time and even today they have taken their monthly payment. Surely they should not be taking payments for something they are saying they can not cover?? Lots of info, sorry! My car was parked on the street and hit during the night. I drove the car to the repair place the following morning and the airbag deployed on the way there. (Stupid, I know, I wasn't thinking). My insurance company, GEICO is insisting that the damage is not consistent with a hit and run accident. They have taken recorded statements from me and my boyfriend. They have contracted an independent accident reconstructionist to read the airbag deployment. The reconstructionist originally told me that he was unable to retrieve the data, but the insurance company is now telling me that he was able to get a partial reading. And that it shows that the airbag deployed as a result of impact. The airbag did not actually fully deploy. There is no powder or injury to me to justify a complete deployment. Does anybody know if they have the right to deny my claim based on suspicion? They keep trying to make me say that my boyfriend was driving the car, which he wasn't. We were in the house together all night. Also: car was in previous fire; severe damage to front of car, all repaired. accident happened in CA, full coverage policy written in GA I know its a lot of info. I'll do my best to summarize my situation, and am using estimates for days and amounts... I received a quote for auto insurance for $590 for a 6 month period from an independent auto agent. When I received my insurance policy and premium price it way nearly double (over $1,100). I cancelled the policy within a week after receiving the huge increase in premiums. Because of the time it took for me to receive my new premium and the 5-7 days for me to cancel, I received a bill for an amount due of roughly $40.00. I previously had auto insurance through the company/agent but it was for a different vehicle and paid a month in advance. My question is, why am I responsible, and how am I responsible for the premium that is so much higher than what I was quoted? I didn't sign any paperwork, I called over the phone and had the agent switch the policy based on the new quote. Auto insurance played a huge factor in budgeting for the vehicle and I feel that I was ripped off. hi all i was hoping you could get some facts/and or legal advice. Ok so i have been renting this car for about 6 month now because i cant afford a down payment for my own. I had my own car insurance which covered the rental. However(bad luck struck) i got into a car accident 2 days after my car insurance lapsed i have since renewed my policy right away with another insurance company that gave me better rates. The fault of the accident has not yet been determined.
