Part 2 of a Computer Science student's analysis of the FNAF: SB intro
Full with tech lingo, abundant personal interpretations, and translations so that my tech illiterate fellas may undertand whatever the fuck im yapping about!
This post is written under the context that you've read my last analysis. I highly recommend you first check out these two posts before continuing with this one if you haven't already: > First post + Continuation ( IMPORTANT!! ) > An addition to the first post
Once you've read through those two (three?) posts, come back here! You're back? You've read them? Awesome! Let's begin then. =)
Reminder! This analysis has been done based off of my own understanding of the subject of both computing and programming - which I am currently studying. I would also like to yet again shout out this reddit post, that also gives a great perspective. Definitely check it out if you're interested later!
Also I have not re-read this, you may find typos - don't hold it against me, they will be fixed, someday =(
Now then, fellas, this is where shit gets wild.
Last post, I talked about the command box we can see at the top right corner of the intro - what each command did and how it basically corresponded to what happens to freddy in the events of the intro.
Tumblr media
However, you might recall I mentioned a second command box, the one found at the left side of the screen. This command box is by far the most important piece of information we have throughout the entire duration of the intro. Mostly, because it changes 3 times.
It changes a total of 3 times in the time it takes for the right command box to finish.
Each time it changes, it displays new lines of code. And every single line of code it displays, tells us a lot about what is happening to Freddy.
Tumblr media Tumblr media Tumblr media
This is the first block of code that we get:
system32> Get-568_win heat_869%yTnu_bl8 lvl_b> 228.wst serial.dot_btb rec.556> dtd /
You might inmediately realize that the first line of code from this command box matches exactly the one from the first analysis. Here are both of them as comparison.
Tumblr media Tumblr media
Hopefully, you've already made the connection. This command box is the one possibly being run by the Glitchtrap/Vanny Virus. Whatever lines of code appear on the command box to the left, are the ones being executed by the Virus - and they affect Freddy in real time. However, the command box on the right ir Freddy's, so to speak.
Both CLI (command line interfaces) are being run at the same time - yet independently of one another. Keep this in mind.
I don't want to go too in depth with this first block of code. All you need to know is that it moves around some directories and runs something called 'dtd', wich could be a command or a program.
The next two blocks of code, however? Ohhh damn... This is it, fellas. This is what I've been waiting for.
Now, I want you to know that this doesn't quite resemble any 'real' code, at least not at first glance. I do believe that it is a very 'condensed' form of the Python programming language, since the syntaxing of the commands shown here somewhat resemble how a string written in Python would look like.
So, I have taken the time to try to decipher what each line means, and what they do. And well, let's just say it explains why Freddy wasn't affected by the Virus in the first place.
Tumblr media
def rule(x) return warning78 init; self_overdrive abort(3)RTLKt abort(5)XGE END
This is the second block of code that we're shown, so lets break it down.
def rule(x) return warning78 init;
This string would define "rule" as a function, specifying "x" as the parameter. Basically, this line specifies that if the command rule is inputted, it should return whatever value (or argument) x has taken.
Normally, define is followed by a return function, which is why I've shown them together, as well as the init; command.
The command return followed by "warning78", makes it so whenever we call upon the function rule, it shows us whatever warning78 may be - and judging as to what happens in the intro, it could be any of the multiple warnings that appear in Freddy's GUI. Or it could also easily be the big "WARNING!" message that can be seen the entire time near the top of the screen.
Lastly, the init; command isn't exactly a standardized python command - but it is a common abreviation of the initialization command, where in the field of programming, it means "the assignment of an initial value for a data object or variable". Basically, it's when you assign the initial values and variables to a program so it can start.
All in all, these three lines create a command that, when called upon, gives out the warning78.
self_overdrive
Again, not really a python function, but important nontheless. This command doesn't have a specific meaning, but we can try to understand what it does.
The term overdrive doesn't really exist in the field of computing/programming. However, it is asociated with overclocking - "the practice of increasing the clock rate of a computer to exceed that certified by the manufacturer" - Overdrive is also a term in the field of music, also known as distorsion, which is when you force an amplifier to output past its limits.
Both of these definitions go around the same concept, pushing a computer to its limits so that it works better, or faster - even while it possibly damages the computer.
We can then assume that the function self_overdrive is making Freddy's system run pasts its usual limits. Which is why I believe Freddy's integrity level plumits during the intro.
Tumblr media
abort(3)RTLKt abort(5)XGE END
Lastly, we have these three lines.
The abort function isn't a real python function - but I believe you can asume what it does. Both lines are attempting to kill something - a program, a process, another function... However, I am not sure wether these two are really functions, since they could very easily be error handling messages. Essencially, warning messages that the system returns when something crashes, for example.
The last line, END, specifies the end of this string of code.
Which leads us to the third and last block of code. The one which in my professional opinion, is the one that reveals to us why Freddy's cool with us during Security Breach! =]
Tumblr media
report.NULL gridlock [ax674] init_task>void alloc [overload] SW.failure return /
THIS IS WHAT WE'VE BEEN WAITING FOR, FRIENDS. THIS IS IT. This is the part where I had the most fun with this analysis...
report.NULL
Now, usually report, in the field of programming and software, it means to record or log something. For example an error log, or crash log. However, it being followed by NULL could also mean that this is an error handling message of sorts.
In computing, Null is, well, zero. It's nothing. It's the absence of value, when something that should be there, isn't.
From this, we could gather that this is a warning message that attempted to report something, yet failed to find anything to report back. No value at all.
...or, we could take this line literally. Taking into consideration that this code is being executed by the virus - this line of code could be taken as an attempt by the virus to stop Freddy's system from freaking out.
Remember that this entire code is being executed as Freddy's actively getting a big flashing WARNING! message. So, this line of code could be an effort to silence it, returning a null value to a warning message.
Both scenarios are plausible, so stick with the one you think fits best!
gridlock [ax674]
This one... man... this line was wild. It's where everything clicked for me. You will want to ignore the characters [ax674], what we truly care about is the first word: gridlock.
You see, a gridlock isn't really a term used in computing at all. It is a term refering to a "severe congestion of traffic, where continuous queues of vehicles block an entire intersection". HOWEVER, gridlock is also known as another term for deadlock.
A deadlock is what's known as a stalemate. A situation where two opposing parties come to a point where no progress can be made. In programming, it means basically the same thing.
A situation where two processes can't proceed, since both of them are waiting for the other to release a resource. Now, imagine this scenario. We have two processes, A and B, and two resources, R1 and R2.
Process A is currently using resource R1.
Process B is currently using resource R2.
Process A requests resource R2, but is blocked because it's held by Process B.
Process B requests resource R1, but is blocked because it's held by Process A.
Now... imagine this situation, but with Freddy, and the virus.
What we most likely have here, the line gridlock [ax674], is an error handling message, warning that a deadlock is ocurring. The string of characters beside it has no meaning, and could easily be but an error handling code of sorts.
init_task>void
I've explained before that init is the abreviation of initialization. So, we can gather that this is a command that is attempting to initialize a task. Now then, in the context of the previous line, this one could mean one of two things:
The virus is attempting to initialize a task (a set of instructions) called void.
The virus is attempting to initialize a task, however, due to the deadlock, it returns this line as an error message, indicating a void return. In programming, when a function returns the word void, it usually means that the function was not able to return a value. It is similar to Null, yet not the same.
Either of these could be a posibility, so I will leave it to your personal interpretation.
And here it when it all goes to hell...
alloc [overload] SW.failure return
Alloc is not a real function, but it can be considered an abreviation of the word allocation. In computing, the allocation is the assignment of memory and resources to the various processes the system may have.
Proper memory and resource allocation is very important in a computer. As you may know, a computer as a limited amount of RAM it can work with, and the same goes for it's processing power. But, for example, what happens when you try to allocate resources that aren't available?
Well, a lot of things may happen. Mainly, the program could hang, the process could freeze - or the entire system could crash!
Remember earlier, we saw that it was likely that Freddy and the virus were in a deadlock. Yet, the Virus tried to allocate more resources to itself... Which overloaded the system, and likely resulted in the next line.
SW.failure has no real meaning - but I've interpreted the first two characters [SW] as software. This would make this line an error message warning about a software failure.
So... What does this all mean? How does it all tie together? Why didn't Freddy get infected by the Virus? Because it got too greedy. It tried to allocate too many resources/memory, overloaded the system and crashed both itself and Freddy.
Tumblr media
Why only Freddy, though? Why didn't this happen to the other animatronics? That... I'm not sure. I believe this is more of a lore question rather than computer question.
In my opinion, I don't think this was a case of 'Freddy knew that he was getting infected by a virus, and fought back'. I'm leaning more to the posibility of it being a 'wrong place wrong time' type of situation... or maybe 'right place right time'? In general, a lot of factors and a lot of different things happened that lead to this specific scenario happening.
Anyways, this is it! This has been my analysis - or nerdy infodumping, if you please.
I do hope that I was able to teach you something today, and that this whole analysis helps you understand the animatronics a bit better - and helps you with future fanfics, comics, AUs, artworks... whatever!
One last reminder - if you have any more questions about this stuff, my ask box is open! I love talking about this stuff!!
Oh, and, coming soon...
DJMM's Bouncer Mode ! A theory by a computer science student as to why it's still present, and why it makes him so aggressive.

Favorite Homestuck Character: 27th US President William Howard Taft
(page 556-566)
8/29/2009 Wheel Spin: Dramatic Irony Verdict: Puns, The Lower Form Of Humor
8/31/2009 Wheel Spin: Parent Bad :( Verdict: Brother Truly Awful And Horrendous
Tumblr media
We begin these pages with John, followed by a quick cut to Dave. Both of them navigating the dangers of their home and the possibility of being watched, but in very different ways.
The Colonel Sassacre and Bathtub level ups are clearly just a setup for puns, so I’m not gonna read too much into the idea that everything can level up (yet). But Colonel Sassacre gained 9550 boondollars from his level, so he must be pretty far up the echeladder compared to the bathtub (490) and John’s meager 200. Sassacre’s new level is ONE MAN JULEP VACUUM. A julep is a chilled cocktail, today usually made with mint, bourbon and crushed ice, although historically used rose petals and was prescribed for health. In 1939, the mint julep became the official drink of the Kentucky Derby after being drunk there probably since its inception. For the ‘genteel, aristocratic southern colonel’ hoovering up a large number of juleps at the derby must be a sign of social status. A high achievement indeed.
Meanwhile, the bathtub’s levels are both references to apocryphal bathtub-related stories. ARCHIMEDED was the ancient Greek mathematician who got into the bathtub and realized that placing an object into a liquid would displace that volume of that liquid (and could therefore determine what metal an object was made from, via the relationship between weight, volume and density) and ran naked through the streets yelling “eureka!” So, his AQUACRADLE is the watery crib that nourished and inspired him. Meanwhile, William TAFT was the US President famous for being the heaviest person in office, who was believed to have gotten stuck, or JAMMED, in the White House bathtub. Although both of these stories are probably false, they’re famous and they’re fun references.
Finally, VAULTHALLA is a pun on Valhalla, the hall of slain warriors in Norse mythology. It’s horrific as a pun (complimentary) but I do think p.558 is the best looking page in all of Homestuck so far. The jewel tones streaked to create the sunset and the sea, the way the boat bobs on the waves and the fire flickers and is reflected in the water is way more beautiful than it needs to be to carry the pun.
LAD SCRAMBLE (p.560) is John’s equivalent of YOUTH ROLL (p.379) in terms of being acrobatic feats attempted and failed by these characters. Can’t wait for Dave’s Dude Scoot and GG’s Kid Tumble. But I am really glad this happened to our boy on the lowest flight and not miles above the house. The code violations on Rose’s building are scarier to me than the absolutely MASSIVE imp that shows up afterwards.
As predicted, this is the rook. It has a better moveset than the imps and is worth about five of them power-wise which is absolutely more than John can handle. The learning curve of Sburb continues to be really steep. I wonder if all the enemies are literally climbing up from below? It makes sense given that they’re part of the forces of darkness, and it sets the game up as a giant chase using architecture, where – at a certain point – just out-climbing the smaller enemies might be more effective than killing them, and only the higher level enemies will even reach the players.
Returning to Dave after a hundred pages away from him is like stepping from the normal world into the first layer of endless puppet hell. His mind is a complex and terrible place, but he – or the narrator – is getting closer to admitting some things. He feels ‘pangs of jealousy’ about his bro’s turntable gear and there’s a mention of getting worked up, and he acknowledges that his bro’s comic ‘get[s] under [his] skin’ and that he thinks it’s ‘just a little TOO ironic.’ He is also aware that ‘trouble’s a-brewin’ due to a missing sword. This feels like somebody getting sucked in too deep to a subculture that’s fine in moderation, but then somebody takes it way too far to the point that it’s harmful – but by then it’s too late to get out, especially when that somebody lives with you, has power over you and presents themself as an authority.
Today I learned that ‘Animal’ is the actual name of a Muppets character (along with Rowlf). This comic is of course puppet themed – Bro does not have a diversity of interests – being tormented by Jigsaw from the movie Saw, which I will watch soon. The art style is different to Sweet Bro & Hella Jeff, less pixelated and artifacted with some hand lettering instead of Comic Sans, darker in both tone and color palette. I’m sure this is post-post-post-post-ironic to some people but to me this is just every ‘what if we took a children’s story… and made it Fucked Up’ post that has been made on the internet in the last decade.
There’s also near-confirmation that Bro is hiding close by and messing with Dave’s mind via sylladex. On p.563, there’s a flash of the sword on the wall being captchalogued, and on p.564, the flash of Lil Cal being de-captchalogued – both from back towards the couch area if my mental picture of the room is right. Out of every character we’ve met so far, Dave’s bro is the one without a single redeeming quality; every new fact or insinuation draws him as an even worse guy.

ENSTARS SELFSHIP WEEK - DAY TWO
shine on the stage !
moodboard:
Tumblr media Tumblr media Tumblr media
drabble / oneshot:
- ibara x kaname x tomoya - fluff - words: 556 - ibara and tomoya are visiting (an awake) kaname in the hospital, and they sing together since kaname is still bedridden
“You’re awake!” Tomoya exclaimed brightly, walking into the hospital room, pulling me by my hand into the room. I pushed a strand of magenta hair out of my face, smiling fondly, as the two of us made our way to the hospital bed.
The bed was inclined to a Fowler’s position, the blankets messy. Monitors beeped steadily to the side of it, wires leading to the figure laying in the bed. The bluenette was sitting on top of the covers, legs crossed under him. Despite his frailness and the bags under his eyes, Kaname had a grin on his face.
“Tomo! Iba! I’m so happy you’re here!” Kaname beamed, leaning forward to accept a hug from Tomoya and a kiss on the cheek from me. Tomoya grabbed Kaname’s hand and began to tell him about the week since their last visit. I adjusted my glasses as I rested my hand on Tomoya’s shoulder, a warm feeling flooding my chest.
“I apologize that it’s been so long since our last visit. Moya and I had been meaning to stop by sooner, but idol work got in the way,” I sighed, running a hand through my hair. “Meetings, lives… you know. It’s been a lot. Not that it’s any excuse, of course.” I added the last part quickly, giving Kaname an apologetic look.
The bluenette just smiled again, eyes somewhat melancholy. “It’s alright! I understand how it can be. Well… yeah. You know.”
Tomoya tilted his head, running his thumb over the top of Kaname’s hand. “I’m sorry. We know you miss it… I wish you could be up on stage with Ra*bits, or Eden, or even your brother.” The blonde fell silent for a moment, seemingly deep in thought. “How about we turn this room into our own stage?”
I looked at Tomoya at that. “What do you mean?” I asked, meeting his gaze. The blonde grinned, before walking over to the door to close it. He pulled out his phone, typing for a moment before turning the screen to face Kaname and I. It took me a moment to place the words on the screen, but Kaname recognized it immediately.
“...One of my songs? But, how?” He asked, face lighting up.
“HiMERU! Well, your brother, I mean. He helped me find it, and I thought maybe we could all sing it together. If you don’t want to, it’s fine, but–”
Kaname thwacked Tomoya’s hand gently, smiling at him with adoration. “This is one of the nicest things anyone’s done for me, stupid. Thank you.” He kissed the top of Tomoya’s hand. “You knew about this, Iba?”
I shrugged. “Shiro mentioned it in passing, but I admittedly wasn’t fully paying attention when he did.”
“He’s been busy. Besides, I wanted it to be a surprise for both of you, in a way. I brought mics, too,” the blonde reached down to pull three karaoke microphones from his bag. “I don’t have music, but I still thought it would be fun to have the mics. Here, Bara, read the lyrics real quick–I color coded each part for us.” I took the phone that was shoved in my face, reading through it, as well as taking the microphone.
“Tomo, I… thank you,” Kaname leaned up to kiss Tomoya’s forehead, smiling at the two of us. “This is amazing.”
sorry if this is ooc or anything,, i haven't written in a while and sjdnsjh also don't mind the first person, it just felt right.

Hiii this is my first time requesting on your page and I absolutely LOVE your work! ❤️
Can I request a yandere alphabet a,d,j,t with An Shiraishi and Ichika Hoshino from prosekai? No rush take your time :D
Tumblr media
“True Beauty” -"Words are humans greatest weapon"
⋆˚࿔ Fluff skirt from Kuro𝜗𝜚˚⋆ :: a skirt to feel comfortable in
― ʚ summary ɞ ― yandere alphabet A,D,J,T with ichian
― ʚ words ɞ ― 556
― ʚ warning ɞ ― murder, isolation, hinting at bullying
― ʚ author note ɞ ― awww that's so sweet! I originally didn't have a yandere alphabet, but I do now! Hope you enjoy it!
Tumblr media
―𐙚 An Shiraishi
Affection
An shows it quite normally I say, cuddling and kisses and all that you wouldn't even think she's a yandere, An loves you a lot and whenever she wants affection from you she gets it mean who wouldn't want to give affection to someone like her? Full of passion and still going through the loss of Nagi-san, she just needs your love
Darling
Not really, An wouldn't go as extreme, as she doesn't want you to know after all, I don't think she would even abduct you unless you find out, and that pretty much won't happen because she's careful about what she does, she's not that stupid while getting rid of her rivals
Jealousy
Of course she gets jealous, who does this person think they are?! An wouldn't lash on you that's for sure, it's her reputation as normal girlfriend on the plate but she will still kill that person in an alleyway when nobody is looking, An would probably find a way to cope by saying you didn't want to talk to them and she's doing the right thing
Tears
An feels heartbroken, she understands you that's how she felt when Nagi-san died, she would comfort you as much as she can and if possible kill the person who made you cry, the same goes for screaming but she's more aggressive while killing the person as if she's letting out you anger, now if you isolated yourself she would try to get you out with stuff like watch her perform or go on a date to her father cafe
Tumblr media
―𐙚 Ichika Hoshino
Affection
Quite similar to An, Ichika likes affection but shows it a normal amount so you won't know she's a yandere, I mean think about it a girl that has a band with her childhood friends it's cute isn't it? And it's especially cute if you can cuddle to her while she plays the guitar, and even better it doesn't seem intense!
Darling
Just like An nothing big really, actually now that I think about it, it is big if you realize that a few of you friends just hate you out of nowhere why? Is it because you spend more time with Ichika or she just told them to back off or else something bad happens, it's not like you believe them Ichika is a sweet girl
Jealousy
Occasionally she does get jealous, though not because of Leo/need she knows they won't steal you from her, I mean everyone knows the best friend code don't they? About dating your best friend ex or flirting with your best friend lover, now let's say someone flirting with you even though you told them to back off, well just a missing person not important they definitely not tied to a tree god knows where
Tears
Ichika also feels down together with you, she even tries to cheer you up playing you favourite song or singing it to you, and it the stars are going to show that night she takes you stargazing, but now if she finds out who is it they will either get their reputation destroyed until they commit suicide or just get killed by her, she will definitely murder them if you angry because of them, now if you try to isolate yourself she will join you cuddling you and singing to you to help you get better

¶ … Recognition of Delirium: Literature Review The objective of this study is to conduct a review of the literature on the assessment and recognition of delirium. The study, which follows reviews literature, related to the recognition of delirium in older patients by nurses and examines the variations of recognition of delirium by nurses as it relates to their own personal views of age and aging related symptoms. Devline et al. (2013) There are few data in existence relating to the current assessment practices used by nurses for delirium. (Devline, et al., 2013, p.555) Delirium is reported to be characterized by "an acutely changing or fluctuating mental status, inattention, disorganized thinking and altered level of consciousness." (Devline, et al., 2013, p.556) Devline et al. (2013) additionally reports that while it is believed by many that patients with delirium are hyperactive they are in fact more likely to be hypoactive or have a mixed pattern. (Devline, et al., 2013, p.556) Delirium in the ICU is reported to be associated "with higher mortality, prolonged stay, and greater health care costs." (Devline, et al., 2013, p.556) Preexisting cognitive impairment is reported as the primary risk factor for delirium as well as are factors of "higher age, the presence of acute systematic illnesses or medical comorbid diseases, and the use of certain medications." (Devline, et al., 2013, p.556) Ramaswamy et al. (1993) The work of Ramaswamy, et al. (1993) reports a study that involved a "comprehensive and sequential intervention (CSI) aiming to effect change in clinician behavior by improving knowledge about delirium. The study design and methods reported include "a 2-day CSI program that consisted of progressive 4-part didactic series, including evidence-based reviews of delirium recognition, prevention and management, interspersed with interactive and small group sessions and practical case conferences " which were reported to have been "conceptualized in a consultation with a leading expert on delirium." (Ramaswamy, et al., 1993, p.122) It is reported that the design of pre- and post-tests were focused on testing the participants in their knowledge and confidence around delirium identification. Study results state that there were an average attendance of 71 individuals at each didactic session. Specifically reported is "…Among all responses, 50 pretests and posttests were matched based on numeric coding (6 MD/DOs, 34 RNs, and 10 others). Mean pretest and posttest scores were 7.9 and 10.8 points, respectively (maximum: 17), showing a positive change in knowledge scores after the intervention (2.9 points, p < .001). Improvement in knowledge scores was higher in the cohort attending 2 or more lectures (3.8 points, p < .001)." (Ramaswamy, et al., 1993, p.122) Tabet et al. (2005) The work of Tabet, et al. (2005) reports a study that examined an educational intervention for preventing delirium on acute medical wards and states that delirium is a common disorder is older people who are hospitalized and that intervention may not improve such cases. The study reports the objective of testing the hypothesis "that an educational package for medical and nursing staff would both reduce the number of incident cases of delirium and increase recognition of cases of delirium within an acute medical admissions ward." (p.152) The study reported was a single-blind case control study in two acute admissions wards in a busy inner-city teaching hospital involving 250 participants over the age of 70 years of age. The state methodology was use of an educational package for staff on one ward of the hospital comprised by a one-hour presentation that was formal in nature accompanied by a group discussion, written management guidelines, and follow-up sessions. It is reported that the follow-up sessions "were based on one-to-one and group" and had the objective of making the provision of "continuous support of staff through emphasizing learning and testing knowledge. Diagnosis and management of some discharged delirium patients were also discussed to allow staff to learn from previous experience. The main outcome measures are point prevalence of delirium established by researchers, and recognition and case-note documentation of delirium by clinical staff." (Tabet, et al., 2005, p.152) Study results state that the point prevalence of delirium "…was significantly reduced on the intervention compared to the control ward (9.8% versus 19.5%, P < 0.05) and clinical staff recognized significantly more delirium cases that had been detected by research staff on the ward where the educational package had been delivered." (Tabet, et al., 2005, p.152) Inoyue et al. (2001) The work of Inouye et al. (2001) reports that nurses "play a key role in recognition of delirium, yet delirium is often unrecognized by nurses." (p.2467) Inouye (2004) reports a prospective study involving 797 patients ages 70 and older who underwent 2721 paired delirium rating by nurses and researcher. It is reported that patient-related factors associated with underrecognition by delirium nurses were examined. The study results stated that delirium "occurred in 249 (9%) of 2721 observations or 131 (16%) of 696 patients." (Inouye, 2001, p.2467) Only 19% of observations involved the identification of delirium by nurses and 31% of patients compared with researchers. It is reported that sensitivities of nurses' rating for delirium and its key features "were generally low (15-31%); however specificities were high (91-99%). Nearly all disagreements between nurse and researcher ratings were because of underrecognition of delirium by the nurses. Four independent risk factors for underrecognition by nurses were identified: (1) hypoactive delirium; (2) vision impairments; (3) age 80 years or older; and (4) dementia. (Inouye, 2004, p.2467) The risk of underrecognition by nurse is reported to have increased the number of risk factors present from 2% to 6%. (Inouye, 2001, p.2467) Patients with three to four risk factors were reported to have had "a 20-fold risk for underrecognition of delirium by nurses." (Inouye, 2001, p.2467) The study concluded that delirium is often missed by nurses when present but that delirium when absent was rarely identified. Specifically it is reported that "recognition of delirium can be enhanced with education of nurses on delirium features, cognitive assessment, and factors associated with poor recognition." (Inouye, 2001, p.2467) McCarthy (2003) The study reported by McCarthy (2003) entitled "Detecting Acute Confusion in Older Adults: Comparing Clinical Reasoning of Nurses" states the purpose of presenting the results from a study conducting "to challenge and refine the theory of situated clinical reasoning generated from an earlier dimensional analysis investigation." (p.203) The study sought to examine how nurses reason and make decisions "in the context of caring for older patients who demonstrate acute confusion in the hospital setting." (McCarthy, 2003, p.203) The study investigated the nursing care environment including acute care, long-term care, and home care settings in order to discover "whether care environment might serve as a factor that could either assist or prevent nurses from making accurate clinical decisions." (McCarthy, 2003, p.203) According to McCarthy, "The theory of situated clinical reasoning explains why nurses often fail to recognize acute confusion or differentiate it from dementia in their older patients who demonstrate cognitive impairment during hospitalization. Moreover, it describes and explains the differences in nurses' abilities to appreciate the importance of confusion and illuminates how nurses' perspectives toward health in aging affect the ways they regard older people and ultimately the ways nurses deal with the elderly in clinical situations." (2003, p.204) McCarthy reports that analysis "undertaken in the original dimensional analysis study yielded a grounded theory that addresses the failure of nurses to differentiate acute confusion from dementia in hospitalized older patients." (2003, p.204) The theory of clinical reasoning holds that nurses "embrace different philosophical perspectives regarding aging and the aged that represent personal postures or sets of beliefs. These beliefs can influence nurses, directing them to "figure out" clinical situations and act in particular ways. The theory proposes that nurses apply these perspectives by using different reasoning approaches when caring for older patients who demonstrate confusion behavior. Rather than follow an invariable scheme of clinical reasoning nurses tend to demonstrate wide variation in the nature of the observations they make, in their ability to accurately interpret observations, and in the efficacy and appropriateness of subsequent actions they take while providing care." (McCarthy, 2003, p.204) The explanation for the variations among clinical reasoning patterns of nurses in the context of caring for older patients demonstrating symptoms of confusion "can be linked to the 'overarching philosophy" of aging that they embrace and operationalize in particular clinical situations. These overarching philosophies serve as perspectives that condition the ways in which nurses judge and ultimately deal with older patients experiencing acute confusion." (McCarthy, 2003, p.205) Three distinct overarching philosophies in the theory of situated clinical reasoning were identified upon the basis of previous research including: (1) the decline perspective (DP); (2) the vulnerable perspective (VP); and (3) the healthful perspective (HP). (McCarthy, 2003, p.205) Findings reported by McCarthy include: "Analysis of these new data showed that regardless of setting, the ability of nurses to recognize acute confusion and to distinguish it from dementia among older patients varied." (2003, p.209) There are reported to be four strategies to enhance the nursing practice including: (1) determination of nurses' perspectives on aging before they care for populations of older adults to assess their potential ability and enthusiasm; (2) Maximize the efficacy of care by assigning staff with positive perspectives to care for older patients or by promoting them to leadership positions, thereby enabling them to influence and shape the behavior of other nurses; (3) Establish organizational protocols to minimize the negative consequences of behaviors associated with the DP and VP by structuring clinical reasoning pathways or protocols for more effective decision making and intervention; and (4) Educate professional nurses and nursing students to reason in ways consistent with the HP -- for instance, teaching them to assume, unless otherwise determined, that an older adult's cognitive baseline is normal and that symptoms of confusion are indeed indicative of acute brain failure." (McCarthy, 2003, p. 211) Summary and Conclusion This study has examined the literature related to recognition of nurses of delirium in older patients. The literature reviewed shows that delirium is underrecognized by nurses and that education is needed to enhance the detection of delirium by nurses. The views of nurses related to aging and underrecognition of delirium is varied. There is a need for better identification of delirium as it related to the outcomes of patients.
