If my opponent has a Platinum Emperion out and I attack with Etali, Primal Sickness and they didn't block, would they still get the poison counters from Etali's ability even though their life total can't change?

Yes.

Damage being dealt to a player is more complex than just "lose that much life." Yes, that's the result of the damage, but the damage can have other results like poison counters or gaining life.

What this means is that damage is still dealt to a player even when their life total is locked by Platinum Emperion. It just normally won't have any visible effect on the game since its usual result (loss of life) can't happen.

In the case of Etali, Primal Sickness, however, the game sees that it dealt 11 damage to the player and so when the trigger resolves, they'll get 11 poison counters and lose the game.
