At Yale’s Long COVID Clinic, Lisa Sanders Is Trying It All

Since the beginning of the pandemic, she — together with colleagues in the pulmonary and neurology departments — had been seeing long-COVID patients at Yale but often in an ad hoc way. Some of the doctors had become so flooded with people seeking help that they were having difficulty scheduling and treating their regular patients who came to them for everything else ...

Long-COVID patients, generally speaking, have been very miserable for a very long time, and because the illness attacks their brains, their hearts, their lungs, their guts, their joints — sometimes simultaneously, sometimes intermittently, and sometimes in a chain reaction — they bounce from specialist to specialist, none of whom has the bandwidth to hear their whole frustrating ordeal together with the expertise to address all of their complaints: the nonspecific pain, the perpetual exhaustion, the bewildering test results, the one-off treatments. “These are people who have not been able to tell their story to anybody but their spouse and their mom — for years sometimes,” Sanders tells me. “And they are, in some ways, every doctor’s worst nightmare.”

Long COVID has been pushing the limits of hospital systems everywhere, not just at Yale. As Americans emerged from the most acute phase of the pandemic, as mask and vaccine mandates lifted and life returned to a semblance of normal for the people who had contracted COVID and recovered, primary-care physicians started to say, “‘I’m not interested in long COVID,’ or ‘I don’t treat long COVID. Let me refer you to a specialist,’” said David Putrino, who runs the new chronic-illness recovery clinic at Mount Sinai. For their part, Putrino added, the specialists were saying, “This is not what my practice is. This is not an emergency anymore.” Patients all over the country reported monthslong waiting times for appointments at long-COVID clinics. All the while, scientists and pundits heaped skepticism on the very notion of long COVID, arguing that infection made people stronger, that new variants posed no threats, that the danger of long COVID was overblown — implying that what patients were suffering from was all in their heads.

Forgotten in this debate are the 65 million people worldwide for whom the pandemic remains a torturous everyday reality.

In the late 1990s, patients with a galaxy of unexplained chronic symptoms — including fatigue, sore throat, joint pain, insomnia, dizziness, brain fog, and depression — began to gather into activist and identity groups, calling themselves sufferers of “chronic Lyme.” They had something, they argued, related to a previous infection from the bite of a deer tick, but their doctors were dismissing them as whiners and neurotics. This was in an era when the medical Establishment was rolling its collective eyes at patients who were querying whether diagnoses of “chronic fatigue syndrome” or “fibromyalgia” might fit their symptoms — ones that looked very much like those linked to chronic Lyme.

Sanders joined the chorus of debunkers. These patients had real symptoms and real ailments, she asserted in Every Patient Tells a Story. But the collection of symptoms was “hopelessly broad and overinclusive,” she wrote. “These are some of the most common symptoms of patients presenting to a primary care office.” She concluded that chronic Lyme was a “phantom diagnosis.”

“I completely regret that chapter,” she says now. “I would like to rewrite it.” Sanders explains that she was reacting to the doctors who were preying on suffering people by prescribing interminable courses of antibiotics that were not helping them: “But I completely misunderstood it. The patients were making the connection between their symptoms and Lyme disease.”

“We’re not paying enough attention,” she said. “We’re not. Doctors are still dismissing this disease as something that’s in your head. I have seen the exchanges on Twitter: ‘Long COVID is exaggerated, not real.’” Women are diagnosed with long COVID at roughly twice the rate of men, Iwasaki pointed out: “I think if the situation was reversed, we’d pay even more attention to this disease.” - Akiko Iwasaki, immunologist at Yale [also known for her work nasal vaccine & long covid research]

But even Sanders was not prepared for how little doctors and scientists know about long COVID. There is no blood test. Health officials can’t even agree on how to define it. The CDC describes long COVID as “signs, symptoms, and conditions that continue or develop after acute COVID-19 infection” — or, in Sanders’s paraphrase, “You got COVID and then something bad happened.” Under the CDC definition, patients have long COVID if they are symptomatic at least four weeks after initial infection. The WHO defines it similarly but with a different time frame: occurring or lasting at least three months after initial infection. This discrepancy matters to Sanders because, as much as possible, she wants to identify patients who have long COVID and not those who may take a little longer to recover from their original illness. In her clinic, she uses the WHO definition.

Sanders, more than ever before, is dependent on the patient’s account — on detailed specifics — to establish her diagnosis. It’s a process of elimination and deduction. So she has “learned just to shut the fuck up and listen.”

while I understand what this person is saying….the difference in how their partners are treated makes no sense to me lmao. Long COVID Recovery Remains Rare | Time

An important article that, among other things, covers why different studies come to slightly different conclusions about how many people recover from Long COVID:

How can multiple studies on the same topic reach such different conclusions? The way they’re designed can make a difference, Putrino says. Some Long COVID research uses data drawn from patients’ health records. In these studies, Putrino says, researchers sometimes assume symptoms have resolved if someone stops coming in for care. But there are lots of other reasons someone might stop seeing their doctor: financial constraints, frustration that treatments aren’t working, health declining to the point that leaving home becomes difficult, and so on. “Recovery” can also be defined differently. Is it a complete resolution of symptoms, or improving enough that someone can function despite their ill health? Once researchers start splitting those hairs, Al-Aly says, they often find that someone “didn’t really recover; they adjusted to a new baseline.”

so when i was a teen i was like, very alt, very punk, like eyeball-deep in the local music scene (this would continue for the next decade), i was NOT a scholar by any stretch and quite frankly hated academics. and my friend introduced me to this band and it ended up being one of my favorites for years.

well i was talking to my friend who's into a bad that was big back in that day and i was like ahhh yes.... i remember those days.... neon and crust and studded leather.... and i listed off some bands, and remembered that one i had liked for so long. i went and looked it up and spotted the album i remembered the most, and gave it a listen and well

it would seem ive always been this way. apparently

#the older i get the more i realize that my life has been this strange paradoxical orobouros of self #one day i decided i would stop moulding myself to other's likings bc dulling my edges only helped those ppl hurt me #thus re-embracing a part of myself i'd learned to be ashamed of. and it was incredibly liberating #and then i realized it was my destiny to be a mortician #and i was sooooooooo afraid to tell ppl bc i was sure they'd be weirded out #but every person i told was like OHHHHH ok yes that makes complete sense. that's perfect #and the more i thought about it the more i was astonished it had taken me so long to figure that out lol #and realizing that i really am by nature a scientist has been really liberating too. i hate that my class bracket prevented me from #discovering that bc i was working all the time for a decade and had no time to explore myself or my interests or anything #and i honestly always thought i was stupid.... because of other people lol #and once i let go of other people's treatment of me as the lens through which i view myself #i realized #oh.... i'm a scary and smart little freak. huh #and i started living true-to-myself and it's been super fun and rewarding and ive never been happier #but like... i kept thinking. wow who could have ever seen this coming! this must seem like such a stark turn for the outside eye #like now that i finally recognize myself... others probably dont #and then i revisited this band and went #ah #no #im just oblivious #ive literally always been like that. even when i didn't know what it was or what it was like. just completely blind to a destiny that is no #SO crystal clear that it's changed how i see the world and myself and the way i live #crazy

I don't have a problem with people quoting "women are my favorite guy." I really don't think it's meant to be malicious, it's a joking meme phrase that's meant to elicit a smile due to the unexpected way it's worded. And I don't have a problem with people liking male characters (hell, I myself have had three emotional breakdowns over one of them in the past two days!). What I don't like is that a lot of people I've come across seem to be saying this phrase as some sort of "proof" that they Really Care about women. And many of them. Do not actually seem be making an effort to back up that claim. (And this isn't even just in the context of refusing to engage with female characters-it's avoiding media created by or for women, it's harassing actresses for playing characters they don't like or who "get in the way" of a desired story outcome, and most egregiously, it's the way so many people treat female victims of domestic abuse or other similar kinds of trauma or mistreatment.)

It's not the fact that this meme phrase exists, it's the fact that too many people are hiding behind it as a show of "support" without actually doing anything. And, honestly, it's the specific way that people use it to suggest that we don't still have a rampant misogyny problem, both in and outside of fandom. If women were your "favorite guy," you'd treat them better. (At the very least, you would spend some time actually thinking or talking about them.)

... i feel like the easiest solution to the whole ~but migrants keep coming here with nothing and then expecting us to feed and clothe them~ thing and whining about the cost to the coast guard of trying to find the boats they arrive on would be

offer free shipping? station a number of reasonably big ships near or at where migrant ships usually leave from, kind of a schoolbus version of a passenger ship, nothing luxurious or pretty, but staffed and equipped to transport as many people as possible in safety and dignity while being as cheap and low-maintenance as can be. and then get barracks set up as close by as possible, where people can stay until the ship sets sail, with the same low-cost, low-effort setup

and then anyone who wants to cross the mediteranean, instead of having to pay exorbitant amounts of money to some shady entity for a space on an overloaded ship, can just. report to the official coast guard ship, which will make the trip the same time every week, and if they missed it, they can get a bunk in the nearby barracks and wait for the next one

and then! everyone who shows up can get basic medical treatment and at least temporary papers if they haven't any, and there can be co-ordination with the european ports, so everyone knows when and where the next ship will arrive, how many people are on board, what immediate assistance they will need, and any other relevant information

hell, if there's something stressful or expensive or otherwise troublesome at one port, they can contact the coast guard and go 'yeah, we said we'd accept the next ship, but there's been [thing] and we're overwhelmed as-is, but the next port over is set up for your arrival', and then no one needs to deal with the unexpected arrival of several hundred injured, ill, or starving people that need immediate help in a million ways

just. we know migrants will cross the mediteranean. it's a thing they've been doing for decades at this point. we also know it's a source of stress and trouble for everyone involved. might as well accept it and make it as safe and controlled as possible, reduce the amount of emergency something-or-other to react to and ensure that the people who arrive are fed and clothed and ideally can actually migrate with their families instead of having to figure out who they can afford to pay the fare for and make their situation even worse

#a grievance against society #also no charging anyone for staying at those barracks #and no fares for the ships #and if anyone goes 'but what about wealthy people wanting to travel for free'#if they're prepared to spend several days in cramped and uncomfortable spaces #with a ton of strangers #just to save a bit of money #they should absolutely be allowed to do that #hell if you set it up like this #there might even be an option of setting this up near an airport #and if someone has been separated from their family #or needs medical treatment that goes beyond the bare minimum #they can make a deal with the airlines #to fill up empty seats on the planes #not ideal to transport hundreds of people i don't think #but every now and again helpful for getting a child and parent re-united #or making sure someone can recover from surgery in a place where they can build a future #instead of somewhere they have to be terrified their past will catch up #we should just apply harm reduction principles more often #if people keep doing a thing even though it's dangerous and expensive and difficult #instead of making the thing more so in hopes of stopping them #we should maybe make it as safe and easy and accessible as possible #and only when people aren't getting killed in absolutely foreseen and avoidable ways #we can try to find and address the root cause of their actions #but first we need to not needlessly hurt or kill peopl #also yes that means all european nations need to help finance this project #it helps the non-mediteranean states too!#and it's the right and proper thing to do
