This is not a hot take but personally I don't feel that Ghost is necessary self-conscious of his face—nor does he really need it for remaining anonymous anymore. Like, his family is dead and the Riley Murders aren't the kind of infamous that every-day passers-by will know about. (Unless you're a local or a true-crime buff.)

At first it might have been for anonymity, yeah, or because he was self-conscious of the scars and stuff, but now I think it's just for comfort. He got used to wearing it and now it's just Part Of The Uniform, he feels naked without it. The people he cares for don't recognize him without it—and that became sort of a running gag. Simon "Ghost" Riley, never seen without his mask, what must be hiding under there? He likes to play along, thinks it fun, and then the rumors start which are fun too but also just... what the fuck.

He's really not all that mysterious—mentally ill, absolutely, but he makes no effort to hide the fact that he is a person with a sense of fucking humor. Yes, he is actually a little frightened of skulls, but he expected for people to riff off of it rather than take it seriously.

What Simon doesn't immediately realize is that when his 'sense of humor' starts with threats of violence, with a record like his, people air toward assuming he's serious (and are not, in fact, playing along for the bit.)

He also does not realize exactly how quiet he is to anyone that doesn't directly work with him. Of course he's happy to shoot the shit with Price, Gaz, Soap, maybe even some recruits, but he's far from social. Combined, that sort of behavior sets up a reputation for a man.

Tldr: I think Ghost is just Some Dude who was at first genuinely surprised to find out people are genuinely frightened of him beyond the "Tall Buff Dude" stuff he was already used to.
