Opinions on writing about mental and physical illnesses?

im talking about more on things the author themself doesn't have nor do they know people with those illnesses.

i am asking because just about every single oc i have deals with some sort of mental and physical illness. im worried that people will give me crap for not having those illnesses but still representing them in my writing. i do a lot of extensive research on the heavier illnesses that aren't as well known + i dont know much about so that i do represent it right, but im still not entirely sure how other people would feel about it.

a lot of my ocs struggle with 'basic' things like depression and anxiety and anything else that can be put in 'simple' terms, which are things that i do personally struggle with and feel that i can represent well.

but i do have some ocs that struggle with things like, schizophrenia, auto-immunue disease (which my mom has so i knew a little more about it going in), psychological part mute, and anything else i may be forgetting that is bigger than just what their personality is. keep in mind these are already established things in their lore and shape who they are as characters. i have already done my research on these things (though i know that i could represent them better) and i know the basics and a lot of the in-depth stuff

im not asking if i should change said ocs--i dont plan on it. but like, i want to know people's opinions on writers writing about illnesses they dont have experience with. is there a way i should go about representing it? should i be more subtle about some, less subtle about others? also keep in mind that for my novel, the two main characters (who are the people the POVs are in) they dont have any of these bigger illnesses. the ones with those illnesses are the 'side characters' in that novel. this question is more for my any oc blurbs + practice writings that i do

The blood on his knuckles had dried by the time he had made it to the showers. He didn’t remember the walk there. Didn’t remember the fight. Not really, anyway. Just remembered the way his body moved with practiced precision. It was becoming almost automatic. The crowd had been louder this fight. But he didn’t know if they were cheering his name or his opponent’s.

He turned the water on and let it run for a few moments. He had blinked and the mirrors had already been clouded with steam. When he stepped under the stream of water he flinched at the heat that came in contact with his battered skin. But he didn’t move away from it. The feeling of it felt distant, like it was happening to someone else entirely.

Water ran through his hair, down his face, and over his chest. He stared at his slightly trembling hands. Not really paying mind to the way the blood slowly vanished from them. His breathing slowed, the edges of his vision blurring slightly. It sounded as if everything sounded too far away. Or maybe it sounded too close? He couldn’t really tell. It was all muffled to him. He didn’t move. He didn’t blink. He just stood there. No one really knew how long he stood there.

Whose perspective should my story start in?

This is shorter than my last post, just a quick ask. Should the story I'm working on start in Axel's perspective (a fighter) or Frankie's (a information broker that has to get information on him). If it were to start in Axel's perspective, it would be easier for the reader to piece together that she is going after Axel when she is introduced in Chapter 2. If it were to start from Frankie's perspective, they wouldn't know who she is tracking until they piece it together in Chapter 2, where I already have it set to where Axel's perspective starts with him taking a shower right after a fight.

Need help making a detail decision

So, the main characters in my novel are two twins. Axel and Frankie. They were split up when they were 12yo and in this time that this story takes place they are 23. Axel was born female--Knew he was trans at 10 but didnt transition until he was 14 (obviously illegal top surgery and had to do deals to get testosterone. his rich friend helped him do so).

Axel is a fighter, and Frankie is an information broker / hitman (rarely a hitman, only to people she believes are bad people). The story alternates their point of views.

My question for this detail is, a gang has asked her to find information on a fighter. The fighter is Axel, obviously. But I don't want Frankie to know that just looking at his paper and the picture.

So here are the three options I have in mind (keep in mind that he also goes by a different name for his job)

Axel is a masked fighter. This is the one I lean away from the most. Because when I introduce Axel's character, his face is battered from a fight. I don't see any mask that I can like… See him wearing that would allow that to happen.

He wears like costume makeup to hide his identity. Again, this one could work but he would probably sweat it off, unless it's waterproof or there is some sort of makeup that fighters specifically use

He is unmasked, but no one knows who he is other than his boyfriend. No one knows his real name, etc,. Frankie wouldn't recognize him due to him transitioning and looking WAY different--Which is why I needed to add that context earlier that he is trans. They do look familiar, obviously. But she thinks he's dead, and she thinks it would probably be a coincidence.

Reading my own fanfiction is basically just a rollercoaster of emotional whiplash.

20% of the time: “Hold on. I wrote this? This is fire. This is emotionally devastating in the best way. This scene is dripping with tension. I’m a literary perfectionist. Someone give me a book deal.”

80% of the time: “Straight to jail. Immediate prison. Why is everyone’s breath hitching?. I used the word ‘gaze’ three times in one paragraph like I was possessed. Did I think 'his eyes darkened' was profound? Why is everyone clenching their jaws? Why is someone whispering 'their name like a prayer' again?? No one talks like this. What is this dialogue. Why are there so many weird metaphors and em-dashes…”

Backstory Frankie and her twin brother, Axel, grew up in an abusive household. When they were 12, their parents disappeared, and the twins were separated. They both ended up homeless for a while. She was put into a mental hospital when they first got separated. It's how they got separated. Frankie has been an information broker for a while. She is only a hitman for people that she believes are bad people. She won't just kill any random person. She thinks that Axel ended up dead, contrary to Axel thinking that he will find Frankie one day. She uses information as her weapon to survive. She also now has a girlfriend named Avery.

Extra Stuff Struggles with suicidal thoughts, self-harm, and drug abuse. Does drug dealing on the side. Thinks that their getting separated was her fault. Knows Spanish due to the number of gangs she has to work with that are natively Spanish

Axel was taken care of by Taryn and their family, who are rich. He had known he was trans since he was 10--At 14-years-old, he begged Taryn to pay for his top surgery. It was done illegally and it was botched. He has a flat chest other than the indents in his body. They broke some of his ribs and damaged his lungs. He has permanent scars from it. At 18 he became an officer, but at 21 he realized how toxic the workplace was and left it. Shortly after that he became a fighter. He has boyfriend named Rayden now.

Extra Stuff He's been shot multiple times. When he was homeless and a when he was an officer. He's also severely depressed and struggles with subjects such as self-harm and suicidal/homicidal thoughts. Struggles with alcohol/drug (really just weed) abuse.

warnings while this isnt primarily an 18+ blog, keep in mind that there will be nsfw topics discussed. not just smut, but sensitive topics that may also fall under the nsfw category. there will always be warnings when those topics are discussed, in writing and out of it. i am also angst obsessed, especially when it comes to my ocs.
