What is Idiopathic Mast Cell Activation Syndrome?
Idiopathic Mast cell activation syndrome (MCAS) is one of several mast cell disorders. MCAS occurs when there are a normal number of mast cells in a person's body but they over-release mast cell mediators causing random allergic reactions in multiple systems of the body. MCAS is incredibly common being present in an estimated 17% of the population.
Symptoms
MCAS symptoms are incredibly varied and always occur in multiple systems of the body. Anaphylaxis is common.

MCAS symptoms are specifically not allergies. the reactions may look like allergies but the two are not the same and MCAS is not a condition meaning "many allergies" While MCAS can have some consistent triggers one of the defining features of the disease is that reactions are random and happen unpredictably.
Anaphylactic shock is not a requirement for diagnosis.
Diagnosis
MCAS is diagnosed by an immunologist. It is in part a diagnosis of exclusion and requires ruling out both allergies and systemic mastocytosis as well as other conditions such as certain types of tumors.
Diagnostic criteria for MCAS is debated. Some immunologists follow the symptom-based diagnosis approach in which case the diagnostic criteria are:
Recurring and severe anaphylactic-like episodes that involve more than one organ system
and
Positive response to mast cell stabilizing or mediator medications anaphylaxis-type symptoms
Others follow diagnostic criteria based on laboratory findings. In this case the diagnostic criteria are:
Episodic symptoms consistent with mast cell mediator release affecting two or more organ systems evidenced as follows:
Skin: urticaria, angioedema, flushing
Gastrointestinal: nausea, vomiting, diarrhea, abdominal cramping
Cardiovascular: hypotensive syncope or near syncope, tachycardia
Respiratory: wheezing
Naso-ocular: conjunctival injection, pruritus, nasal stuffiness
and
A decrease in the frequency or severity; or resolution of symptoms with anti-mediator therapy: H1 and H2 histamine receptor antagonists, anti-leukotriene medications (cysLT receptor blockers or 5-LO inhibitor), or mast cell stabilizers (cromolyn sodium)
and
Evidence of an elevation in a validated urinary or serum marker of mast cell activation: Documentation of elevation of the marker above the patient’s baseline during a symptomatic period on at least two occasions; or if baseline tryptase levels are persistently >15ng, documentation of elevation of the tryptase above baseline on one occasion. Total serum tryptase is recommended as the markers of choice; less specific (also from basophils) 24 hour urine histamine metabolites, or 11-beta-prostaglandin F2.

Primary (clonal) and secondary disorders of mast cell activation ruled out.
These are not all proposed diagnostic criteria as the subject is heavily debated. Generally, a laboratory-confirmed MCAS diagnosis is considered more legitimate.
Treatment
MCAS is a very treatable condition. Generally treatment follows a path from antihistamines -> mast cell mediators -> biologics.
Epipens are given to MCAS patients with a history of anaphylaxis.
Antihistamines are divided into 2 categories: H1 antagonists and H2 antagonists. These categories are determined based on the histamine receptor each one targets.
H1 antagonists mostly deal with systemic and cutaneous symptoms. H1 antagonists are also further divided into first and second generation antihistamines. first generation antihistamines include diphenhydramine (Benadryl) and Hydroxyzine. These tend to cause drowsiness. With second generation H1 antagonists cause fewer side effects and include drugs like loratadine (Claritin) and cetirizine (Zyrtec)
H2 antagonists primarily affect the gastrointestinal tract and include medications like famotidine (pepcid)
Typically when treating MCAS a person will be put on both a second generation H1 antagonist and an H2 antagonist.
When antihistamines do not treat symptoms well enough the next step is a mast cell mediator. The most common mast cell mediator is cromolyn sodium which is available by prescription only. (this is technically available OTC but it is at 1/50th the dose used for MCAS) Mast cell mediators work by preventing the degranulation of mast cells in the first place.
When both antihistamines and mast cell mediators are insufficient someone with MCAS might be prescribed a biologic such as Xolair to treat their remaining symptoms.

i woke up in a Bad Headspace today and imma use by blog to vent about it like it's livejournal circa 2004.
tw for discussion of medical stuff
- I hate xmas. I'm jewish but my dad is not and the pressure to have An Perfect Xmas every year and the disappointment every year just grosses me out now. Not even going to talk about american xtian hegemony but there's some of that in there too.
- Today i began to suspect that the awesome new migraine med that actually stops my weather-induced migraines might also be interfering with the efficiacy of my prediabetes meds. or maybe im wrong and im Just That Fucking Exhausted.
- Spouse spent all last week recovering from a severe medication allergy and hives on 70% of his body. and steroids Do Not Agree with him so it was just. a wild time. terrifying af. stood over him with an EpiPen basically the entire time.
- 3 days before The Hives, i scratched my cornea while pruning bushes and that right there is a pain i do not ever want to revisit. had to go to the optometrist and get The Goo.
- 2 days before Cornea i had my first Botox For Migraine treatment. 31 injections to the face, neck and shoulders. it stung but it was over in like 4 minutes. takes 3 sessions to start kicking in. sessions are 12 weeks apart. so i guess we'll find out in August if it's working.
- day before Botox i had an ENT appointment and he stuck a camera up my nose and then diagnosed me with a weird vocal cord paralysis thing so now i have to go back to speech therapy for the first time in 26 years.
- Week before that, saw my neurologist and she diagnosed me with a weird intermittent lazy eye / motion lag thingamawhatsis so now i have to go to an ophthalmologist AND vision therapy.
- That week I also saw my PCP and explained to her about the intermittent abdominal pain I've been having since like 2021. She took me seriously!!! Which is good!!! But now i am scheduled for baby's first colonoscopy. And i have to keep a food journal, which i HATE because food is STUPID.
- All of the above all happened this month btw. December 1-23.
- My final appt in November was yet another ultrasound of my former left tit because there is an oil cyst at the site of my top surgery and they are VERY SURE it is a benign oil cyst but the rules require them to poke it every few months for 2-3 years.
- Before that I had a 48 hour ambulatory EEG which was the itchiest i have ever been in my LIFE. That same week our basement stairs collapsed and a contractor had to come rebuild them (up! to! code!). That same week i also went to the dentist to get my crown fitted.
- I think my MIL was in the hospital that same week, too. so that's a thing that's been going on the whole time since then.
- I spent most of October deathly ill with food poisoning thst was originally misdiagnosed as viral. I ended up with a CT scan and colitis. and, eventually, cipro. it was the sickest i have ever been in my adult life. i would rather have mono again. i fantasized about those cholera beds with the hole in the center so you didnt have to get up to have your horrid dysentery. nightmare.
- The day before that hit i had ONE golden day where i felt good and had energy. we went to temple and i got glomped by about 10 different people. my 80 year old bestie kept finding me to hug me again. Rabbi hugged me super hard.
- Before that was a root canal, and before that was a tooth infection that took 2 rounds of antibiotics to kill, and before that was the original cavity filling that started it all. the dentist kindly comped me the $172 for the filling against the $3,800 bill for the root canal + crown.
- Before that? IDEK man. I have lost track. Somewhere in there i got diagnosed with insulin resistance which explained my HORRENDOUSLY TERRIBLE fatigue and cloudiness and waking up starving every 3 hours. The meds they gave me changed that literally overnight. it was a miracle. which is why im freaking out about the new migraine med possibly counteracting that. i spent the entirety of last summer in a fog. several of my very good friends visited and all i could do was nap on them. i couldnt go anywhere or do anything. it was a nightmare. i don't want to go back to that. but also i don't want to have a migraine every time the wind blows. but i would rather have a migraine 50% of the time than be back to that fatigue fogged state 100% of the time. nope nope nope.
- and amongst all of this, still having the seizures. they were going down for a while but the last week or so has been 1-2 per day. so. another checkmark in the "gee do you think you're stressed?" column.
- it is going on 10pm and I'm tired so i very likely have accidentally omitted several other things. to be fair to me, there's Quite A Bit to remember.
- so if youre wondering why i havent updated my latest fic, its partly bc i am TIRED and partly bc if i gave jason even a third of the health bullshit i have dealt with the past few months, it would absolutely defy belief. TWO kinds of eye problems AND a speech problem AND food poisoning AND dentistry?? surely no one in the world has to deal with that much!
sigh. anyway. thanks for listening. i promise i am stressed out of my GOURD but, shockingly, have not slid into any sort of depressive space. mostly im just annoyed. i spent today watching dinosaur documentaries and reorganizing my craft supplies.
tomorrow will be better. today just sucked.

Hello! I'm not sure what proper etiquette is here so I hope I'm not overstepping, pls feel free to ignore if I am. You mentioned in a tag and/or post about having estrogen allergy? My wife is dealing with that, and it's... so difficult, most doctors don't know what it is and we thought we had a short term solution so she could finally get some sleep (she hardly sleeps at all anymore because of nightly itching). It's so hard to find ANY information on it and we're not sure how to proceed anymore. Just...wondering if you have any information at all? I'm sorry if this sounds desperate, it's because...it is lol.

Dw anon, you're not overstepping!! I've been in hormone hell since I was 13 so I know how much of a relief it is to find someone who understands for once. I don't believe that me or my mom (who's also affected) have the same allergic reactions that your wife is experiencing, but I'm hoping that I can help anyways.
Okay so first off, the doctors not knowing about estrogen allergies is bullshit. They may not be super common, but they've also been documented since at LEAST 1921. Just a cursory google search has yielded two scientific articles about it (that looked legit when I scanned through them before going to lecture, where I'm typing this rn), which I'll put right here:

I'd suggest trying to read through medical journal articles on it because despite the density of the information and the difficulty of the language, medical articles tend to be more comprehensive about the symptoms and causes than ones written for the general public. They also mean that you have the language to confront doctors about it if you wish- which you'll have to do, because if she's experiencing an allergic reaction like itchieness, then I'd also be concerned about potential anaphalaxis if her reactions get more severe. The intolerance that me and my mother had manifested in different ways (migraines, muscle weakness, disorintation, being hypersensitive to food/unable to keep food down, extreme drowsieness leading to being bed-bound), but those weren't quite on the level of being so itchy that you can't sleep. A more typical allergic reaction is something to be concerned about and I highly suggest not only stressing this to your doctor, but also getting confrontational about it and meeting multiple different doctors if needed before you get care. Force them to run tests, force them to listen to you. That's my biggest regret when it comes to my hormone issues, because they just got progressively worse and worse until I had to go to the ER for it. Turns out that I was not only estrogen intolerant, I also had hypothyroidism! And nobody in 10 years of appointments thought to check for that!!
As for what helps me and my mom, I found that progesterone hormone treatment was effective for us both (until it stopped working for me, but I still don't know why that is. It works for my mother). You might want to be cautious about trying this because in the articles I read, some people were just as sensitive to progesterone, but it's worth a shot. I used to get the DEPO-provera (or however its spelled) shot to offset the issues caused by estrogen, which is an injection once every few months. I also found that once I went on testosterone that a good chunk of my hormonal health issues straight-up vanished, but obviously that's not exactly the best option for everyone. I'm on a masculinizing dose so maybe you can do a really super low-grade dose, but I'm telling you about it mostly just to get the point across that HRT works. It's not just for trans people- cis people get hormone fuckery and need hormone therapy as well.
But there's also proposed solutions in the articles I linked, so I hope those are helpful or give you an idea of treatment options. Best of luck!

Traveling while Chronically Ill
I recently came back from a trip to a conference on the opposite coast. Because the arthritis and spasticity in my back has been altering my gait and balance, I had to use a cane for the first time in a long time. This drew the notice of concerned colleagues, although most were too polite to comment. But that led to me thinking about all the things I have to carry that they don't see. This trip alone, I packed the following: * 6 daily medications * 5 prescribed daily supplements * 1 injection requiring refrigeration * muscle relaxers * allergy-safe pain relievers * allergy-safe antihistamines * epi-pen for severe allergic reactions * allergy-safe soap, shampoo, conditioner, and lotion * allergy-safe lip balm * zinc soap for skin lesions * gauze and bandage tape for severe lesions * allergy-safe band-aids * 2 types of prescribed eye drops * wrist brace * cotton gloves for overnight skin treatment * alcohol wipes * allergy-safe plane snacks * six face masks
Also, as soon as I arrived I went to Whole Foods and stocked my mini-fridge with safe food, knowing I wouldn't be able to eat with everyone else most of the time. I was conservative this trip because I was only going for four days. For longer trips, I'd be carrying two additional refrigerated injections with a cooler bag, an electric heating pad, a refillable ice pack, and an infrared light device for skin therapy. And possibly ankle and knee braces, depending on how I was doing.
This list of items represents my daily care for psoriatic arthritis, Sjogren's syndrome, hidradenitis suppurativa, migraine, and fibromyalgia, and my preventative care for severe allergies. You see the cane and the brace, but there's a lot more going on behind the scenes. I guess my point is, if you can get ready for a trip by packing your clothes, toiletries, and electronics, consider your able privilege. If you can use the soap the hotel provides, consider your able privilege. If you can eat the food the conference offers, consider your able privilege. I love to travel, but it takes a level of planning and preparation for me that many people will never have to experience.
