Today in the ED I saw a pt with a posterior radial fracture (a Colles’ fracture). The fracture was impacted so the orthopedist said to splint it and have her follow up with them. One of my co-residents made this helpful splinting handout.

Can you speak any on how your finger splints interact with any swelling you may get? I have mild hypermobility that's getting worse as I get older, but I also deal with edema in my hands on the regular.

For sure! So I can't post pictures right now, but I also have swelling due to POTS stuff, and my silver ring splints are adjustable, which means I can bend them in and out of shape to fit my fingers as needed.

I can make them looser or tighter depending on what I need. Some days I swell up too much to wear them, but most of the time I can wear them by pinching the rings close together, which makes them fit looser. In winter I pry them further open to make them fit tighter.

If I can get off the floor today I'll try and post a video for you.

April loved mythical critters and legends and mysteries. She was not expecting to meet any of them in person, but she’s not complaining! These are ACTUAL dragons! Or, well, two dragons and two little serpents. And they’re funny and nice and just fun to be around, not to mention absolutely adorable! She’s not missing out on that opportunity.

Splinter tries to stay away, but this little girl is very persistent. She finds them almost every time they come out.

Rise Ramblings #546

In the beginning of the mini-episode “Race,” April texts all the guys with an offer: free pizza to the one that can come by and watch Mayhem.

I can only describe what happens next as, the “Text Battle of all Text Battles.”

Based on the choice of pictures we can safely assume that we are indeed looking at Leo’s phone.

Well, let’s analyze the play-by-play, shall we?

ROUND ONE

Leo comes in clutch with perfect form and all caps. No extra frills, but with all the chills. Next, we have Raphael. Ooo, looks like he lost a bit of time there tripping up on capitalization. Nice try, Champ. Third is Mikey. It appears his spelling mistake cost him vital microseconds. No “dibss” for you, Mikey baby. Lastly, we have Donatello. Two explanation points? Too much time lost there, buddy.

Winner: Leonardo

ROUND TWO

Again, perfectly done by the boy in blue as he slides into first with all caps (plus an exclamation for effect.) Second, we have ol’ reliable, Big Red. By ditching the “I” he gained some speed but lost it all with the extra exclamation points. Maybe next time, Red, maybe next time. Third we have Orange. Typos? What’s a typo? Sadly, the time skipped in forgoing accuracy means nothing as he still took third place. Last is Purple, yet again. Maybe spend less time on correct capitalization and more time sending your message, Dondon.

ROUND THREE

And here we’ve emerged with the emojis. Swift, simple, and to the point, Leonardo is first place once more. His message is clear. Pizza? Absolutely. Get a clue, it’s all for Blue! Second is the big man himself, two slices for him please! Sadly, the extra slice equals no dice as he comes up thrice in the number two spot. Starry eyes, pizza surprise, but he’s not on a roll as Mikey is next to go. And dead last is Othello von Ryan. Yes Donnie, pizza is 100 percent fire, but with the knife and fork you’ve come up short!

Why you should wear your ring splints while crafting

Hypermobile hands suffer from a number of painful issues, the most common of which is swan neck deformity, which I have in index and pinky fingers. This is when hyperextension of the PIP (middle joint) damages the tendons, which pull on the DIP (top) joint, making that point downward.

It doesn’t cause me much pain (yet?), though sometimes it causes locking/snapping of the joint, so I often end up not wearing my ring splints on those fingers, even though I know I probably should for long-term reasons. So I was curious about how the ring splints would affect hand movement while knitting, and I recorded it with and without the ring splints.

The way this type of ring splint works is preventing hyperextension of the PIP. These photos show it pretty well in my left index finger. With the splint, I can still bend the top of my finger at the DIP (because I’m using it), but the PIP doesn’t collapse down. It doesn’t seem as important on the right hand with knitting (or on the thumbs).

(This type of splint doesn’t help a separate issue, which you can kind of see here, at the thumb MCP. I think it’s subluxating? It looks like an indentation in these pics but it only happens when I push the thumb forward. I have a thumb splint with an MCP stabilizer and extends down into the palm, but I tried it and it didn’t stop the subluxation.)

Related: holding a pencil. You need splinting on the DIP joint this time. Hot tip: my rings from my pinky PIP fit my index DIP, so when I’m writing I can just move it over.

Sharing because I went to a hand orthopedist about this and when I asked what I needed to do to reduce progression of deformity and preserve function, he literally was like 🤷🏻 I really don’t think they know what to do with young people in whom these problems aren’t associated with arthritis and hand weakness.

Remember.

Even though the poison ravaged his body and physically took him away from his children, it would seem spiritually, the warrior could connect with them. But not for long as his sons need guidance just to see him.

Growing up and, most importantly, alive, the father couldn't be happier to see them, even if he had to watch from a distance so as to not threaten their lives too much with his presence.

He'd be damned if he'd let hurt them once more.

Despite the unfortunate circumstances, he finds limited company in the familiar old rat that visits from time to time, the updates he shares always bringing him a spark of joy.

Oh, and his other ghost roommate that's made him question his existence more than once.

Perhaps another time, he'll be able to care for his children. For now, they have their fathers who will care for them more than he ever could.
