The thing is that the ways in which dean and sam need each other are both compatible and parallel in their nature.
Dean needs sam to stay with him (let him take care of him look after him and be with him) and sam despite all his need for autonomy and freedom which he does desire of course (he needs dean to respect his choices and see him) also needs dean to need him—choose him. It’s always been that way—something we see from the very pilot itself. He goes with dean in the pilot after dean admits that he doesn’t want to do it without sam even if he is capable for it.
It affects sam profoundly when dean gets close to other people—especially men because it threatens the idea that dean could need people other than him (even the mere desire to want for others apart from him is distasteful even if he doesn’t want it to be—he just can’t help it, it’s the way they are.) Of course dean has never needed for anything more than sam, that sam and just sam has always been more than enough but sam needs that from him, constantly and actively.
In Sacrifice (8x23), when he reveals that the fact that dean chose to turn to people (an angel, a vampire) apart from him was unbearable to him was just so.. much. And it’s interesting because we know that sam too is friends with cas, has never been shown to consider him a rival in any sense (but just the mere possibility of sam and just sam not being enough for dean is devastating for him.) He doesn’t harbour any resentment or competition towards cas, it’s just that he needs to be the choice over everything else from dean. He needs it because he chose that too, even when he had a chance to get out—multiple times over. And yes, the circumstances shaping his choices are often not ideal, are sometimes not even entirely choices but he always stays because of the knowledge that dean will always choose him.
The conflict then is often caused by doubt—dean, deeply insecure about sam’s loyalty. For him, it’s a given—that sam will always be first, has always and forever been above everything else but he expects sam to know it too. Despite everything he puts on him and says to him, despite the fights and the anger and the mistrust. None of that will ever change this one fundamental thing.
But Sam doesn’t (!!) Maybe at one time he did, before the demon blood and the soullessness and the countless countless ways he thinks he fucked up but somewhere along the line it became clear to Sam that he could not trust it to always be Dean’s first choice, can’t know it for sure.
Doubt again, Sam—unable to know if Dean will always choose him over all else and Dean unable to verbalise that enough because of said insecurity (the fear that he needs sam more than sam needs him) and unable to understand why Sam would feel this way because he expects him to already know that Sam will always be above all else, at the end of the day despite whatever happens because that is who Dean is.

Why Being Manipulated into Letting Gadreel in a Huge Deal for Sam?
I know a lot of Dean Girls were very upset (and will be upset after reading this post) with Sam when Dean asked him if the roles were reversed if he would have done the same thing (letting an angel possess him to save his life). Sam simply says no! Some fans have gone to the extent of calling him selfish but no, Sam is not being selfish in that scene. It wasn't that he didn't care about his brother. He really does and that is why him saying no is actually a good thing.
Now see, Dean never had to go through the whole loss of bodily autonomy due to a possession issue (not until after he was possessed by Michael but that's way after the Gadreel plot) like Sam has been and disturbingly a lot more number of times than Dean's single rodeo with Otherworld Michael (or is it AU Michael? whatever!). Just to keep the argument simple, I am purely focusing on the loss of bodily autonomy purely from a possession POV.
When Meg possessed Sam in S2, she used his body to kill other hunters, to assault Jo and to beat Dean. Maybe she did a lot worse than that, we don't know. Who is left with the trauma of that knowledge? Sam!
The Gary-Sam body swap. This one though seems funny on the surface, it's actually kinda disturbing because Gary used Sam's body to have sex and the kid. Kinda gross enough already but that dumb kid could have easily handed over the vessel to Lucifer. Imagine, that's how it played and Sam was back into his body. Now who is trapped with Lucifer, the Devil? Sam!
Speaking of, understandably saying yes to Lucifer was Sam's call to put him back in the box but it didn't go down without blood on his hand. The moment Lucifer took control of Sam's body, he killed the demons from Sam's life that deceived him. I am guessing these were possessed people he killed in the process and not just wisps of black smoke. At Stull cemetery, he exploded Cas to bits, snapped Bobby's neck and beat up Dean to pulp! all by whose hands? Sam's!
Gadreel's possession did help Sam get better but at what cost? Kevin's death? How many nightmares did Sam have seeing his hands burning Kevin hollow? Now let me point out the aftermath of this possession which is somehow even worse than the actual possession: Crowley skewered Sam's brains with needles, hell, he even possessed Sam to wake him as if one possession was not enough. Sam literally had two supernatural beings possessing him at one time! Don't even get me started on the painful, torturous grace extraction process. Sam was willing to die in that moment because he believed his life wasn't worth saving, definitely not at the cost of Kevin's life!
Before this role reversal scene, Dean wasn't possessed by anything, so he doesn't understand how horrible it is to lose autonomy over one's mind and body. I don't expect him to grasp the gravity of it. He sees it as a healing from within. For him, if 'ends justifying the means' that's all that matters.
When every single possession has caused nothing but grave trauma to Sam Winchester, tell me why would he or anyone for that matter, in their sane heads do this on their own brother, especially when they love them so much?
Here's another very real life perspective for all those who feel Sam saying 'no' if the situation was reversed was a horrible betrayal and proof that he doesn't love Dean enough: Ever had someone you love on life support or gone through a situation where you had to put down your beloved pet? Why do we do this? is it because we don't love them? because we don't care? no! Because sometimes, it is better to let them go than to prolong their suffering by putting them through this pain. So next time you feel Sam was being selfish, or disloyal to Dean or that he didn't care enough, think about a loved one suffering through something horrible because you didn't have the guts to let go!

thinking about dean growing up and putting everything before himself. hunting and his brother and his dad and his dad’s revenge quest for their mom. he doesn’t matter. he is entirely irrelevant. thinking about dean internalising this as just what you do, just how people behave and how they should behave. him viewing selfish as the worst thing you can possibly be.
then thinking about sam growing up and fighting. brave enough to challenge their father and rebel against him and voice something different, brave enough to focus on what he wants. dean seeing this and it stings - he could never do that. how is sam acting like that? he can’t believe that’s the right way to behave. so sam must be selfish, just in believing he has any right to his own life.
dean sublimates himself for the family and expects sam to do the fame, and his resentment and jealousy that sam doesn’t turns into anger and making sam out to be the mean one, the one in the wrong. and this never goes away. this is always what dean levels at sam - that he’s selfish, that in wanting to make his own choices he’s rejecting their family, rejecting dean……. awful. toxic. evil evil message to send to sam. entirely in character. dean wants to prioritise sam, would save him over the world. but he doesn’t care what sam wants.
selflessness isn’t always a charming character trait. it’s not the same thing as a generosity of spirit and it’s definitely not the same thing as being caring. sometimes selflessness just means you’re incapable of prioritising your life and incapable of understanding how anyone else could or should prioritise theirs. sometimes it means you still act selfishly, you just convince yourself you were objectively in the right, because doing something actually for yourself is unthinkable. sometimes it means you think the very act of having wants and boundaries is selfish, no matter whether they’re yours or anyone else’s.
anyway… thoughts on dean’s specific brand of awfulness regarding sam. what does it matter to him what sam actually wants? since when did it ever matter in the winchester household what anyone wanted? dean had to deal with things he didn’t want for the mission (for john). sam has to deal with things he doesn’t want for the mission (for dean). augh. the cycles
