I quite literally just finished the season 11 finale, so a lot of the show is a blur to me right now, so I think if I was to give an answer that perfectly lines up with canon I would have to go back and rewatch (which i'm gonna do, just not right now bc it's 3 am), so I'm just gonna give an answer that might not make complete sense in canon but is just what I'm feeling right now.

I think the first time someone they know personally sees them share a kiss, it's Carl. I think it happens right before Ian and Mickey establish to each other that, yes, they are a couple. I don't think it's a huge thing, I think it might just be a quick peck in the hallway or in the kitchen when they think no one is around, but then Carl pops up somewhere in the room and hit them with the "are you guys together or what?" which then makes Ian bring the same question up to Mickey later and then getting the soft confirmation that "of course we are"
