Hi~ Trans man putting in his two cents. The amount of customization and inclusivity is limited to the amount of coding you can do. It is a lot of work indeed, and is something you have to think about depending on the length of the story. You could have just have man/woman/nonbinary and add xe/xem pronoun besides they/them. Then goes cis and trans option, and something I saw in other IFs and think is a good idea, is putting a separate option for genitals right before the first intimate scene, otherwise I don't see the need of mention it in other context. Regarding trans options, is up to you to leave just "MC is trans" in a vague way or being incredibly specific and customized regarding hormones, surgery and other transition options. This again, will increase the coding work tenfold.

And since you say it's okay to give you recs, allow me to spam you with IFs (and authors) who has given people wonderful trans content/representation. It's not a complete list, just the ones that come to mind:

i have not had the pleasure of playing most of these so they’re on my list for this weekend and next. truly appreciate it!! also, thanks for sharing your ideas on ways i can make my game more trans inclusive. i am new with the coding but im confident i can incorporate at least some of what you’ve mentioned on top of what I’ve already done. and then as i improve my skills, I’ll work to improve customization options.

For Morien cuz I woke up and chose violence

You find Morien half collapsed on a table in the tavern. The loud chattering, the screaming and the sound of scraping almost ceases at your passage.

Without giving anyone any mind, you walk forward and pick Morien up by an arm, making them rest most of their weight on you. You keep them still when they let out a moaning grunt, slumping a bit forward.

Leaving some coins on the table, you drag the castle physician outside.

The truth is that you had a fight with them three days before. Morien wasn't talking to you, but if the previous nights the knights you had sent after them at the tavern were able to keep an eye on them until they had stumbled home, this time they hadn't even been able to speak coherently.

You half carry Morien towards their quarters, ignoring their broken protests.

Once you have laid them on their bed, making sure there is nothing around them they can hurt themselves with, and you are about to start cleaning up the mess they have left the room in, the physician's eyes open.

Rarely has Morien reached this state - you have to say. Their alcohol tolerance is so damn high they can talk and yell at you to fuck off even after six or seven drinks.

Morien's eyes are filled with a naked kind of resignation. Of anger, and disappointment, but above all something vulnerable has come to the surface.

"Will you..." they rasp out, throat parched. "Will you pass me the bucket?"

You do and they proceed to throw up.

You look after them the whole night. And when morning comes, and you groggily open your eyes to find an empty bed, you release a sigh and get up.

Morien is just there, past the door, when you turn.

The vulnerability in their gaze is still partially there when they ask, "do you see the fucking problem now?"

You see no problem. Only a deeply suffering person trying to dull their pain in any way they can. Distancing themselves from anyone as soon as they get close because they are scared they will be hurt again.

Morien's eyes shine as if already defeated, already faced with rejection and regrets, "will you give up? If you won't be able to break past my stupid walls?"

Naja brainroooot

Tall MC carrying Naja around like a teddy bear🧸 They need their emotional support spouse! With an arm always wrapped around their waist. Puts Naja on their lap unconsciously, to the point they won't react if Naja themselves sit on their lap in public, just wrapping their arm around them in an almost pavlovian response.

Naja clinging to the Monarch's neck while carried, simply enjoying the ride. Naja would probably feel too self conscious to sit on the Monarch's lap in public unprompted, but if MC puts them on their lap naturally Naja enjoys it a lot, especially if the Monarch noticed Naja was getting anxious or spacing out.

Staying so close to the Monarch grounds them, it soothes them in a way nothing else can.

"I'm sorry, Liz, I am." He takes a small step towards his wife, hating the distance that had been put between them. "What can I do to make this right? There has to be something."

Cognac brown eyes slip shut as if she was trying to block out the desperation her husband, the love of her life, was showing her. "I don't know if there's anything left you can do, Oliver," she murmurs, all the fight leaving her. "I'm so tired of expecting more than you're capable of giving. I'm so tired of fighting by myself. I'm so tired of pretending that I've only lost my child and not my husband too."

She opens her eyes, fresh tears causing them to glimmer like stars.

"And I'm so tired of thinking that I'll ever be enough for you."

Thinking of a potential scene where MC wears the ROs tokens, especially Cherena's, and someone from their circle of "friends" try to embarrass MC by saying something like "where did you find that mirror? In the thrash?" and Cherena waiting till the laughter dies down and quietly comments "it's mine, actually. I gave it to them."

That would be so sad, it would make Cherena so upset. And it would be something of an eye-opener for them, because while Cherena is aware they are surrounded by superficial friends, this treatment of MC makes them very dejected and so hurt.

Cherena wouldn't do much, but their voice and face alone are expression enough that something is deeply wrong

Oh God now I'm stuck with the image of MC owning a fake front cafe that connects to a side building where Grey and Co. do all their activities.

Better yet, sweet MC offering sweets to someone who knocked on their door looking for Grey "for business" (who are actually looking to kill them) and the visitors drop dread cuz the sweets had poison.

MC calling Grey saying they have a "package" at home and when everyone arrive the crew is surprised and mildly concerned.

They would be so proud. Also slightly concerned about the muffin they just ate.. The thieves learning to keep their hands to themselves because MC doesn’t have anything labeled.

Arthur groans, face flushing a brilliant red which he buries in his hands to try and hide from your prying eyes. “You’re so embarrassing, how did you say that with a straight face?”

Morgana’s lips tilt into a gentler version of her signature smirk, before she leans in close to whisper in your ear. “Why don’t we do a taste test to confirm that?”

Lance chuckles, sounding a little breathless as he reaches over to brush away some stray crumbs from your lower lip with his thumb. “What am I going to do with that mouth of yours, Merlin?”

Gwen chokes as her mouthful goes down the wrong pipe. “You-? Me-? Sweet??? Oh no, you’re the sweet one! I mean, not that I’ve tasted you… Wait! That’s obviously not what you meant. I- if you’ll excuse me a moment.”

ROs reactions (+ James and Mother) to MC being a literal Disney princess with the ability to attract any kind of animal? I don't mean just their pet, but whatever animal crossing their path. They would be able to reduce a snake or a bear to a harmless puppy with only the kindness of their eyes, the softness of their voice and a few caresses.

The court is walking through the forest, close to the castle, but far enough for a lovely picnic. Even the Queen has made an appearance. An hour into the peaceful little event, MC realizes that animals have begun to surround them. A bear, a snake, even a raccoon among the many...

MC hops up and wanders toward the animals, eager to show them a bit of love. "Well, hello there sweet creatures," MC says as they calmly approach a bear standing on its two legs, watching with a tilted head.

James: The Prince's eyes widen as he watches his twin come to a stop in front of a massive bear. Mouth agape, he feels torn between running and tackling them before it swipes with its paw and watching to see what magic unfolds before him.

Vivian: The Queen keeps an impassive face as she watches her youngest child approach dangerous, wild animals. She takes in a subtle breath, wondering what Percival would think. MC reaches out and gives the bear a gentle caress on its head. Vivian's lips twitch as her child softly compliments the animal on its fur. Pride swells through her at the actions: both kind and strong.

Constantine: Constantine slowly but surely moves to MC's side. Just in case. His heart is beating wildly and he wonders how MC can continue to impress him with their hidden abilities. MC gestures for him to pet the bear. He hesitates but does so after a moment, a goofy smile finding its way onto his face not long after.

F: F watches in wonder as MC interacts with the animals, their quiet voice calming the creatures. MC's gentle laughter works its way into F's mind, sending butterflies throughout their body. F shakes their head, amused. Then, a rabbit darts across F's legs on its way to where the other animals are with MC, and they cannot help but join in on the quiet laughter.

Margaret: A bird flutters past Margaret's face and lands on MC's shoulder, eliciting a rare and true smile from the guarded woman. If MC can affect animals in such a way, I look forward to watching them bend the nobles to their will without even trying. Margaret watches MC with soft brown eyes, filled with fondness.

Felix: Felix's mouth opens in shock when MC reaches out to give the now prone bear a belly rub. Then he smirks a bit and shakes his head. Well, I won't be bringing MC to the forest for any dates - don't need them showing all the animals affection instead of me. Still, Felix decides to join MC with some of the leftover food and they feed the animals, side-by-side.

Arthur feeling safe for the first time of their life, waking up unusually rested and without anxiety (Arthur often wakes up very early in the morning with their heart beating wildly in their chest and anxiety making them shaky, until they either fall back asleep or stay away until morning). Enjoying the moment as one of the most peaceful of their life
