Can I have top surgery and be overweight? The Truth.

I am here to clear things about being overweight and top surgery (aka double mastectomy).

Can I have chest surgery if I am overweight? Blanket answer, yes you can.

HOWEVER, let's talk about some things first.

As soon as we peek under my blanket statement, most doctors will not want to touch you if you are too far outside their BMI index. Sounds like a preference, right? I mean, true, but they are a business and, like any other business, can refuse service for any reason. Especially when your life is in their hands.

The BMI index is followed to a degree by most plastic surgerons. This index is not to body shame you in this case. It is strictly for medical reasons. There are many medical reasons to refuse the surgery.

As you lay flat on your back, your own weight presses on your lungs and heart. This can actually cause damage your heart and lungs and also restrict your breathing even further.

Anesthesia issues are a lot deeper than most people realize. It is not just a harmless gas to keep you asleep. Anesthesia is poisonous to us, and it does more harm to your body the longer you are under its direct effect. Also, the more you weigh, the more they have to use. The best anesthesiologists in the world probably would have a hard time with this, especially depending on how long you can potentially be under. The more you weigh means more tissue they need to work with and extra time can mean complications not only while under the drug but can cause many complications after the procedure is done, and even when you wake up. Issues include headaches/migranes, intense nausea, vertigo like symptoms, having a hard time passing urine, lack of gross and fine motor functions, brain functions, paralysis, coma, and death.

Blood flow issues for people who are overweight are a real risk. The skin has stretched over time , and more skin, along with viable tissue, is thinner, with less of a blood supply being delivered to the skin and tissues itself.

Being overweight also means your skin is stretched and no longer has the same elasticity as if you were not overweight. This thin skin is easy to pull apart/tear.

Time in the operating room, more is not better. The longer you are being operated on, the more complications can occur during the procedure and during the healing process. (This is also a general risk)

Upping complications means more likely needing revisions. Plastic surgerons take this into consideration for your health. Insurance probably won't shell out for the same operation twice because of the views on transitioning lead to a lot of "this is just cosmetic."

Diabetes exponentially increases risk of infection and potential necrosis and gangrene (death of tissue).

You are a living canvas for the plastic surgeron(s), your results reflect highly on them. They want the best results for you, but they also have a reputation to upkeep.

If the surgeon gives a condition they consider reasonable and the patient does not produce results near their actual target BMI, which tends to be more liberal than the numbers that are thrown around, the surgeon can question the patients ability to be able to take care of themselves and adhere to the strict rules given for post operative care. A surgeon can also think "does this person want it bad enough if they can't do this request?". They may not say that out loud, but they may think it.

The heavier you are, the harder it is on your body to heal because the extra work your body needs to do to do daily tasks, let alone extra tasks yet also rest.

You do not realize how much and how often you use your chest muscles. Getting out of bed without being able to lift yourself up is really a thing. Using those muscles in some cases can cause rips and tears of stitches, tissues, and muscles.

Surgeons will give you realistic, cis-like results with your chest, this may mean you will not have a flat chest, they may perform more of a reduction than a double mastectomy. Surgeons will make your chest look proportional to your body, hence another surgery if you do lose weight.

Do they want you to have bad results? This is a physical correction to make a positive mental impact if you have bad results post-op. This may not really fulfill the function of procedures purpose to help you like your body.

Doctors want to know you want this surgery, so if they give you a time frame and you don't meet their goals, that's fair. You may have seen your day to day but the surgeron didn't and doesn't care. To them, you didn't meet their goals/ requirements. No surgeon wants to lose their job, role, or status.

Surgeons only want what's best for your health. It's not a shaming conspiracy, it's literally just biology.

Waiting sucks, we know. But what sucks more is that having bad results, you will have forever and potentially risking your life.

1. what drives them? what’s their ultimate goal?

Ramiel’s driven by an all-consuming sense of self-importance or like. The sense that there’s something your supposed to do but you haven’t done it yet, and you can’t remember what it was- but gods does it eat at you, keeps you awake at night, leaves you never feeling truly satisfied with your accomplishments.

If he can claw his way to the very top of the ladder surely THEN he’ll be satisfied with the people he’s proven wrong and the respect he’s earned and the power he’s gained. (This goal is NOT going as planned tho, for Ramiel)

2. what was your original concept for this character? how did playing them change that concept?

Ramiel actually hasn’t changed much at all from his concept and actually playing dnd! Mostly he’s just been like. Streamlined. For my dnd campaign- we actually ended up rebooting it after like. three-ish(?) years of playing! IT WAS GREAT TO BE ABLE TO TAKE MY FIRST DND PC AND BE LIKE ‘time to make him even BETTER now that I know how dnd works and in general have a much more thorough understanding of collaborative storytelling.’

Ramiel’s personality and character motives/flaws/tropes have all stayed really consistent between Godskeep 1.0 and Godskeep 2.0- but his backstory changed a lot and I was able to develop him to be much more rooted in the world he lives in! In 2.0 he started with a handful of relations to important npcs which has been a lot of fun…

Honestly what maybe changed the most is he went from having a 100% bonafide god-complex in 1.0 to having an ‘unusual amount of arrogance and self-importance for someone so clearly insecure’ in 2.0.

10. what inspired this character’s creation?

I listened to this specific PMV on loop while i drew the FIRST picture of Ramiel…. It was suggested I make a paladin to fill the martial class slot- and I knew I wanted to make an ambitious, self-assured white and gold dragon-kin. “Young God” BY HALSEY, FALLEN ANGELS, TOXIC RELIGION, GLADIATORS, AND TIGERSTAR WERE ALL HUGE INSPIRATIONS.

THIS ONE IS HARD TBH. Honestly, I loved talking with my friend (who dms the game) and like. Fleshing out the little things that establish that Ramiel was never going to be ‘nice’- but even with his ruthless upbringing and bullying persona, he has a genuine capacity to love and do good :’). IDK PUTTING THE LITTLE THINGS IN THERE IS PROBABLY THE STUFF THAT MAKES ME HAPPIEST. Hange finally looked at you, you couldn’t believe what was happening, until they looked at Levi and said “don’t ever lay a finger on her again”. Before letting you drag them away from there.

Levi just smirked, knowing what this was about.

You dragged them to their office and closed the door behind you, before turning back to them.

“What the fuck is wrong with you?” You said, your heartbeat still going at a thousand.

The reason he snuck out with his old Irish friends and went to box in the WVBA was to get away from his parents. He really doesn't want to come back, and ESPECIALLY doesn't want to talk about them.

soda popinski, bc he can handle his... quirky personality. Plus, they go to clubs to dance together, along w/ disco kid. speaking of which, he surprisingly gets along pretty well with disco kid, they bond over their music tastes, even if theyre radically different. they also collab on music together sometimes, with aran writing the lyrics and disco doing the instrumentals. they have a lot of creative differences however, but despite that, they remain friends.

The only person I can think of that may have been more-or-less of a friend to Aran ever is Soda. They've been great drinking buddies since the dawn of time, and can be real with each other only when they're drunk as all hell. They don't want anything to do with each other when they're sober tho.

He, Glass Joe, and Don Flamenco are besties. They annoy the ever-living daylights out of each other, but they're hilarious when they hang out together.

I ship it all ways it can be shipped, platonic, romantic, familial, queerplatonic, alterous, sexual (<- though won't talk about that much, wait 1 more month until my 18th birthday)

What made you ship it?

I have a thing for rarepairs, if it wasn't obvious by the loceit & analoceit shipping. I got into SaSi at the beginning &, at that time, analogical was very much a rarepair since Prinxiety & Logicality always took the forefront. I used to read prinxiety fanfics and replace Roman with Logan, it was that hard to find analogical fics. (There were analogical antis. It was wild.)

I don't remember what specifically started the shipping, though I am 100% sure My Negative Thinking had SOMETHING to do with it.

I don't like "opposites attract" since... that type of relationship dynamic of "my partner is completely different from me & we have nothing in common" would... never work IRL (I know the point of shipping is to see cool dynamics, not be the "most accurate to real life", but I'm a realist, I just can't enjoy it). Analogical completely chews up the "opposite attracts" dynamic and spits it out into the trash. They are so similar and that's what I like about it. Two characters that can relate with each other and find comfort in that similarity (hmmm this sounds a lot like my feelings on demus... do you see a pattern /s).

In My Negative Thinking they both react with disgust towards c!Thomas bringing up romance and as a romance repulsed aro, it's perfect. I love two guys that hate romance in a romantic relationship 🧡

A lot of people think that affection in a romantic relationship has to be grand and super romantic & I absolutely hate that. I enjoy Analogical because it's two characters that would not be grand in their affection. I love exploring different boundaries in a relationship. I'm unsure how 2 explain, but it means everything to me to see a relationship that isn't "normal". A relationship that isn't the "perfect", in the eyes of the media or amatonormativity.

I'M RUNNING OUT OF WORDS, but it's just....... give me a romantic relationship where the characters don't kiss or say I love you as often. A romantic relationship where their entire dynamic is inherently not ""romantic"". Same way I want platonic relationships that are sexual or romantic, while still being platonic and it doesn't change the feelings of anyone. Escape the confinements of societal standards!!
