Batman: The Brave and the Bold v2 #16
Because technically he didn't die, right? But he was in this place "The End of the Road" that was basically hell, and he was transformed into this "unfallen" creature, which doesn't look very alive to me. (In fact, it looks basically the same as Deadman, who is a dead man.)
Of course, he's soon saved by Deadman and the deity Rama Kushna before he actually dies. But I personally think that as long as he was in that skeletonic form, he wasn't alive.
