梅雨真っ只中、ムシッとした空気に覆われていますが、皆さまいかがお過ごしでしょうか。

私は先日、北海道（札幌）に出張で行き、一瞬だけですが爽やかな青空を楽しむことができました。

今月は皮膚科関連の学会に17年ぶり（！）に立て続けに参加してきたのですが、流石にこれだけ年数が経つと、座長や演者で見知っている先生はごく僅かで、時の流れを感じます。

今号は、月初に開催されたASCO（米国臨床腫瘍学会）からの最新情報を2本お届けします。

【記事1】CDK4/6阻害剤の使い方に一石を投じた「SONIAスタディ」

───────────────────────────────────

「ホルモン陽性・HER2陰性」は、乳がんで最も多いサブタイプです。

このタイプの乳がんの場合、がんは女性ホルモンを糧に増殖するため、術後療法にしても進行/再発時の治療にしても、ベースになるのは女性ホルモンを抑制する「ホルモン療法」になります。

ここに加わったのが、CDK4/6阻害剤というタイプの抗がん剤で、日本ではパルボシクリブ（イブランス）とアベマシクリブ（ベージニオ）の2剤が、2017年から18年にかけて、進行/再発のホルモン陽性・HER2陰性乳がんの治療薬として登場しました。

当初は二次治療以後で使われていたのですが、一次治療での有用性を示すエビデンスが出たことにより、現在では一次治療でもCDK4/6阻害剤+ホルモン剤（アロマターゼ阻害剤）がホルモン剤単剤の治療よりも推奨度の高い標準治療となっています。

ところが、そこに一石を投じるような試験結果が出てきました。

　■”Primary outcome analysis of the phase 3 SONIA trial (BOOG 2017-03) on selecting the optimal position of cyclin-dependent kinases 4 and 6 (CDK4/6) inhibitors for patients with hormone receptor-positive (HR+), HER2-negative (HER2-) advanced breast cancer (ABC)”「ホルモン陽性HER2陰性進行乳がん患者に対するCDK4/6阻害剤の最適な投与タイミングの選択に関する第3相SONIA試験（BOOG 2017-03）の主要アウトカム解析」（Journal of Clinical Oncology）

一次治療でホルモン剤単独療法、二次治療でCDK4/6阻害薬の併用療法を行う場合と、一次治療からCDK4/6阻害薬の併用療法を行う場合を、”ガチンコ”で比較した試験は今までなく、SONIA試験はそこを明らかにしようとした試験です。

1050名のホルモン陽性・HER2陰性の進行/再発乳がんの患者さんを、以下の２群に分けてその後の治療経過を比較しました。

・一次治療でCDK4/6阻害剤＋アロマターゼ阻害剤、進行後の二次治療でフルベストラント＜A群＞

・一次治療でアロマターゼ阻害剤、進行後の二次治療でCDK4/6阻害剤＋フルベストラント＜B群＞

結果、２つの治療を合わせた無増悪生存期間（PFS2）は、A群は31.0ヶ月、B群は27.8ヶ月で、両者の間に有意差はなし。

一方で、CDK4/6阻害剤での治療期間は、A群は24.7ヶ月・B群は8.3ヶ月と、圧倒的にB群が短く、Grade3以上の重篤な有害事象の発生件数も2778件vs1620件と、B群が少ない結果となりました。

色んな意味で負担の大きい治療はなるべく後回しにしたい患者ニーズは一般的ですので、アロマターゼ阻害剤単剤治療が一次治療でも効果面で明確な不利がなさそうという今回の試験結果は、患者さんにとって選択肢の幅が広がったという意味で朗報だと思います。

最後に、こんな試験を製薬会社がやるわけないよなと思って資金提供者をチェックしたら、「オランダ医療研究開発機構とオランダの医療保険会社」と出てました。

今後もこうした患者さんの負担面に配慮する「デ・エスカレーション」的な試験は、保険者が率先して行なう流れが続きそうですね。

【記事2】カペシタビン（ゼローダ）の手足症候群に救いの手

───────────────────────────────────

「手足症候群」という言葉、抗がん剤治療を経験されてきた方は、耳にしたり実際に経験されたことがあるかもしれません。

抗がん剤を投与することで、

・手や足がしびれる、痛むなどの感覚異常が出る

・手や足の皮膚が赤くなる、むくむ、しみが出来る、皮膚が硬くなる（角質化）、水ぶくれが出来る

・爪が変形する、色がつくことがある

などの症状が副作用として出てくることがあり、これら一連の症状が「手足症候群」と呼ばれています。

酷くなると、ものを持てなくなったりキーボードを打てなくなったりなど、著しくQOLが阻害されます。

「手足症候群」を引き起こしやすい抗がん剤の中でも代表的なものが「カペシタビン（ゼローダ）」。経口剤という簡便性もあって、胃がん、大腸がん、乳がんで、広く使われています。

このゼローダの手足症候群を予防するのに、「ジクロフェナク」という消炎鎮痛剤の外用剤を試してみた試験結果が出てきました。

　■”Randomized double-blind, placebo-controlled study of topical diclofenac in prevention of hand-foot syndrome in patients receiving capecitabine”「カペシタビン投与患者における手足症候群予防を目的としたジクロフェナク外用薬の無作為二重盲検プラセボ対照試験」（Journal of Clinical Oncology）

ジクロフェナクは昔からよく使われている消炎鎮痛剤で、ブランドとしてはボルタレンが一番有名です。

カペシタビン投与予定の乳がん/胃がんの患者さんに、ジクロフェナク外用剤を予防的に4サイクル投与する群（130名）とプラセボ群（133名）とで、手足症候群の出方に違いがあるか調べたところ…

グレード２以上の手足症候群の発症率：3.8% vs 15.0%

全てのグレードの手足症候群の発症率：6.1% vs 18.1%

と、有意差ありで、ジクロフェナク投与群が低い結果になりました。

手足症候群が酷くなるとカペシタビンを減量せざるを得ないので、抗がん剤の本来の効果を期待するという意味でも、ジクロフェナク外用剤の予防投与は意義があると考えられます。

さらに、ジクロフェナクは古い薬剤なので、コストが極めて低いのも喜ばしいですね。

今回の試験はカペシタビン投与に限定されたものですが、手足症候群は、カペシタビン以外の抗がん剤でも比較的よくある副作用のため、更に応用が進むことを期待したいと思います。

Sorry but not really educated about metals, it wasn't part of my curriculum in fashion school. We had a few classes about jewelerly so it explained how jewels were put into metals for necklace or ring but we didn't dig about the different properties of metal, their care, etc..

I also don't wear jewels (I just have a small cross necklace that u can see in my brand intro video lol). Maybe I'm insane but I'm not comfortable with the feeling of wearing metals (rings especially). I cringe whenever I see people washing their hands without removing their rings for example lol

It's funny that you're talking about knit apparel (beanies, scarf) because knitting are my favorite clothing (I'm often cold lol) but they are a whole different category compared to woven fabric. Basically woven fabrics can be cut and sewn, but knit have to ve knitted which needs an actual production line dedicated into making the specific products you want (knitting is quite complex in that you can have totally different results with the different type of stitch techniques, fibers that you use, etc... So it needs quite a long sourcing & prototype testing process) . Since @atelierpapirouge is a single operated company (me) it's impossible I can order the minimum amount of quantities for them to start off the production (some companies won't produce anything below 1000 units for example). That's that problem with today's clothing industry : the scale of production are now so big that it's impossible for small designer to find a range where they can operate in small(er) quantities at an affordable provide..... And the only companies making lower scale of production are......you name it : IN CHINA!!

So for now, I'll stick to woven fabric, but developing knitted products is definitely a long dream of mine🤍

lazarus, come out of your cave

Numel is a very strong Pokémon, able to carry heavy loads up to 100 kg. Its hide is so thick and it is so slow in registering external pressure that it is almost impervious to pain. Nolan is enamored and thinks Numel are both cool and cute. Hardcore, he calls them. So strong it can’t feel pain!

So obviously, when Nolan stumbles upon a whole herd of Numel during his travels, he immediately knows he needs one. But he also wants the "best one",  and he decides to camp near the Numel territory and do a stakeout, assisted by his Pokémon. He’s not beneath crawling on the dirt and hiding behind rocks and stalking the Numel like some sort of idiot. Some Numel probably wandered into his camp while he was out hunting for them and they ate his food.

Meanwhile, Boog goes off on his own quest to roast his Seel drumstick over the Numel’s hot humps. He harasses and eventually spooks them enough (with ice and cold boogers), accidentally causing a stampede to breakout which results in a mini rockslide sending Nolan running for his life. Boog is sad because he still can’t roast his drumstick.

Unknown to them, one Numel broke off from the stampede to hide in a cave, and the ensuing avalanche of rocks trapped it behind a large boulder, blocking the exit. It can't get out. It's very dramatic.

The Numel have grown too wary and skittish for Nolan to get close to them, scattering whenever they sense anything amiss and rumbling threateningly, prepared to attack at the slightest hint of provocation. Not willing to risk another stampede and running low on food, Nolan is about to throw in the towel and leave when he passes by the rubble and hears muffled, plaintive cries coming from behind a rockface. He learns that a Numel is trapped and quickly grows concerned.

Nolan tries to get it out but he and his Pokémon (at the time, he had Sabre, Princess, Lee and Boog, all unevolved) can't really do much to move the giant boulder. They do their best to shovel the smaller rocks and rubble out of the way but quickly realise they are out of their depth. Knowing Numels are super hefty and strong, Nolan tries to encourage the Numel by shouting outside the cave like an idiot to use its power to bust out!!! But the Numel can't. It tries, ramming against the boulder (you can hear thumping from inside) but it’s quickly worn out.

Despite this, Nolan keeps on encouraging it because he knows if it doesn't come out it’s going to starve and die inside and no one will know about it. He tries to get the help from the Numel herd but they don't really understand him and after their scare, they don't trust him and attempt to chase him off. So he camps outside the cave entrance and stays there for 3-4 days, trying to encourage the trapped Numel. Meanwhile his Pokémon keep worrying at the boulder like little construction workers. He also put some of his Pokémon in charge for scavenging for finding food (where they bring back anything from berries and fruits to leaves, rocks, bugs, straight-up trash and other prey Pokémon)

As the days go by, the Numel is slowly growing weaker and starving; a hungry Numel can't even stand. So it just keeps on crying from the inside. When the rain falls, it turns the cave a little drippy with water leaking through. Numel hate rain and water as it cools them off, slowing them down and making them super tired and uncomfortable. Soon, the Numel falls silent and Nolan is very worried.

Watching Boog digging and smacking at the boulder gives him an idea. He gets Boog, his multitalented Pokémon whom he doesn’t appreciate (underrated MVP) to dig a hole. Boog can't dig deep a big enough hole for the Numel to come through, but it’s enough for his Pokémon to enter and give the Numel some food. So Boog goes in because he dug the hole and Sabre follows suit with some berries and to keep the Numel warm with a fire attack.

Once Boog gets into the cave with the Numel, he just stares vacantly at it. They both stare blankly at each other (though Boog has a sort of ‘wat’ look and the Numel looks both wary and weary). No one moves. Then Sabre comes in with food and the Numel comes alive. As it eats, Boog touches it with one paw. Then smacks it with his booger (he freezes his snot, hardening it like a club and swings it to hit) and Sabre hisses at Boog. Boog sneezes at him and runs away, tunnelling back into the hole.

Boog has this "oh shit" run (that I find is hilarious). His eyes widen (he doesn’t seem to blink, honestly), his head rears back a bit and he just turns in one smooth motion and runs with his snot trailing behind him. He’s completely silent and unblinking. The only time he spoke was after he evolved and he learnt Roar from Red (Theo’s Typhlosion) …but I digress.

  Sabre stays and spits embers at the Numel, keeping it warm before retrieving more food and repeating the process until he’s worn out.

The rain is getting worse and the Numel herd have already moved on to seek shelter from the storm, but Nolan refuses to leave the Numel’s side. He’s wet, cold, tired, hungry, aching and so are his Pokémon. They can’t keep it up for much longer. The rain makes the hole fill up with water and Boog and Sabre are now also trapped inside the cave. Well, Boog can dig them out but he’s too tired.

On the last day, a desperate Nolan pulls out his “inspirational anime speech” card, telling the Numel (and the rest of his Pokemon) how he won’t leave their sides (but will come back for it!!! = Titanic scenario), how he plans to get help and return so please wait for him etc;  and he inspires and touches the Numel so much that it gathers all the Naruto friendship power in the world and learns Strength! 

It starts to push and shove and ram itself at the boulder. Sabre picks up the cue and pelts the Numel with the last of his fire to keep him going strong. Boog helps too.

Then finally, FINALLY, they break through the boulder…!!!

And of course, the Numel wants to follow after Nolan. And he does. 

It’s later revealed that Boog also knows Strength...though whether he knew it all along or he learnt it after watching Lazarus use it (he’s a very quick learner; truly an underrated MVP)...no one will know.

What is a boog boy?

Boogaloo boys, or Boog boys, are basically just a new strain of right-wing anti-government militia, but this time mostly comprised of extremely online 4chan millennials and zoomers. Boogaloo refers to the coming civil war (or race war, depending on how far-right the person using the term is) that they are preparing for, ie; Civil War 2: Electric Boogaloo. Pokeball choices

NOLAN

Nolan has The Basic Man Aesthetic™. I can't imagine Nolan bothering about appearance beyond basic functionality, although he does find certain ball designs cool. However they are usually too pricey and he’s quite content with his arsenal of standard Pokeballs. 

His favourite ball designs include the Great Ball, Ultra Ball and Quick Ball. 

He doesn’t care much for seals because he can't be bothered to customise (TOO! MUCH!! EFFORT!!!) He has better things to do.

Princess, his Skitty, came in a special ball – the Premier Ball – when he got her from a specialised breeder. It has a pearly enamel-coated shell. Though he doesn’t care too much about aesthetics, he does care about exclusiveness and with the Premier Ball being a rare, limited edition, he’s happy to have his grubby hands all over it.

Nolan found a Quick Ball and has been saving it ever since for a special Pokemon to capture. However, Boog, who has been stalking following him for several long weeks now, captured himself with it. He chose the ball because he liked it. His self capture completely defeats the use of a Quick Ball, much to Nolan’s dismay.

DAY

Day only got into the Pokeball craze after he got his first non-standard Pokeball – the Heal Ball. He didn't know Pokeballs could be different so he was pretty excited about the cosmetic changes. He cares about aesthetics.

Kirby (a Sylveon by then), was on board with changing Pokeballs because he wanted something special too. Vain trainer, vain Pokemon. xD Kirby didn’t want a plebeian ball. So into the newly acquired Heal Ball he went.

Ever since then, Day got into Pokeball collection and customisation. He would seek out special balls in local shops and online so he could find fitting balls to colour coordinate with his Pokemon. Sometimes he would find designs that he thought were very lovely but he had no need for. He would buy them anyway as a gift. He bought a Moon Ball for Jacob’s Lops but Jacob never used the ball.

Over the years, after much extensive searching and saving, he has gotten a Dive Ball for Mickey, a Love Ball for Pinkie, and a Premier Ball (artfully decorated with seals to resemble a Togepi’s shell) for Benny.

Like his other bestie, Ayla, he is a huge fan of seals. They both collect, share and trade and shop for them together, often getting them in a value pack and getting excited over it all. The seals were rarer in their region so they both had to put in a lot more effort to get their seals.

He used to be a little like Ayla who would plaster every inch of her Pokeball with seals (though not as much). He liked trying different combinations and was a big fan of the original Party, Star and Heart seals. His most treasured seal is the rare and exclusive Twinkle seal that releases a shimmering shower of prismatic sparkles upon release.

He and Ayla tried to enter their designs for customisable seal competitions but never won.

Kirby — Heal Ball (with a Twinkle seal)

Mickey — Dive Ball (with a Bubble seal)

Pinkie — Love Ball (with a Heart seal)

Benny — Premier Ball (decorated with seals to resemble a Togepi’s shell)

ZION

Instead of gachapon, in this AU, Zion collects Pokeballs (the literal capsules instead of the prize in them ahaha). His great-grandparents did so and he expanded on their collection.

He has one of each kind; special limited editions, rare antiques, collector exclusives, handcrafted treasures and even those no longer sold on the market. One or two date back at least a century and are no longer in production (courtesy of his great-grandparents). I bet he somehow has a Master Ball.

He displays them all in closet shelves in a special room, each with their own display cushion and case that he lovingly maintains every day. He has a strict look-but-don’t-touch policy. ...for himself. Apart from shining and polishing and dusting, that is. He doesn’t let anyone else know of them. They are incredibly valuable.

Of course he doesn’t use any of them. No, all his Pokemon are in standard Pokeballs (when he finally bothered to register them anyway).

Only Furin is in a special ball – a unique Luxury Ball – and that is because she was already in it when she was given to him after his great-grandparents (her owners) passed.

Ueki — Poké Ball

Hagame — Poké Ball

Fūrin — Luxury Ball

WENDIE

You would think Wendie was someone who cared about the aesthetic appeal of her Pokeballs, but no. She thinks seals are tacky and the various ball designs to be gimmicky and gaudy (i.e. Luxury Ball). She prefers the sleeker, simpler and classier designs but wouldn’t necessarily go out of her way to obtain them.

For the most part, special Pokeballs are expensive and harder to come by and she’s much more into function than design. 

That said, it’s also why she caught Freya in an Ultra Ball (that she won as a prize).

Anarchism will never work on a large-scale, for any long period of time

I used to be an anarchist, but over time I abandoned it, because there are just way too many variables that make large-scale anarchism impossible for any long timeframe. Still a socialist, but I think we need a state. There are a ton of problems with how anarchism would play out in the real world. I've never heard a good anarchist response. This is why I think anarchism has a reputation in the Left for being pretty childish. There are obvious issues. For example...

- How exactly do you win an anarchist revolution in the US context, when you have an extremely powerful capitalist state and millions of right-wing gun nuts? Do folks actually think a decentralized network of communes (loosely organized in a federation) will win against the might of the US state and all of the boog boys? Without a state or some strong centralization, how do you coordinate the flow of fighters, resources, info? How do you coordinate revolutionary action and logistical matters without a proto-state or some type of strong command structure?

- Even if you do somehow win an anarchist revolution in the USA, what comes next? Like, if you smash the state and you automatically create a stateless communist society, do you think that states like Britain, Russia, and France will accept that? Do you really think a decentralized anarchist society will secure revolutionary gains (let alone win) against the highly trained, disciplined military apparatuses of capitalist states? Will anarchist militias beat back the SAS?

The two points above explain why anarchism has never succeeded for any long period of time. The three big anarchist experiments of the 20th century (Catalonia, Shinmin, Free State in Ukraine) all failed in less than 10 years, often in just a few years. Anarchism will never succeed on a large-scale, for more than a few years, because socialism will always face tremendous opposition from capital, from right-wingers, from reactionaries. Without a state, you will get nowhere.

Hate to say it, because I was once anarchist... But anarchism is extremely idealistic, in that I've never heard a good explanation for how an ideal (stateless socialism) can come first, when you have to deal with material conditions of invasions, sanctions, counter-revolution, etc.
