SEES members react to getting anesthesia: headcanons from a real anesthetist

Makoto: the guy who secretly smokes weed, chews through roc every 15 minutes, needs over 1.5 MAC maintenance sevo. (Laymans terms translation: smokes weed so he burns through a ton of muscle paralytic agent (rocuronium) and anesthetic gas (sevoflurane) needed to keep him relaxed and deeply asleep)

Yukari: had her hair and nails done the day before surgery, wakes up from anesthesia asking if she said anything dumb and apologizing if she did. (Complimenting patients on their nice nails is part of my small talk to attempt calming nerves when they're rolled into the OR)

Junpei: would try to fight anesthesia and count past 10 seconds, tries to cheat by counting fast (he loses anyway) (It's so amusing when patients try to challenge anesthesia. Some put up a good fight, but in the end, anesthesia always wins.)

Mitsuru: takes 300 mg of propofol on anesthetic induction, scares the shit out of OR staff when she still reaches for the airway device as the anesthetist tries to insert it. (Redheads tend to need more anesthetic than average. For context, the induction/knock-you-out dose for propofol is about 2 mg/kg. For frail old people, I halve that dose. Most people don't need more than a single 20 ml syringe/200 mg of propofol. I push 200 mg for big tall football/basketball guys. I've seen redheads take at least 2, even 3 syringes. Mitsuru would be a tough one to knock out.)

Akihiko: the extremely athletic ASA 1 guy with baseline bradycardia bordering on need for anticholinergics. Will most definitely wake up swinging fists and knocking out teeth and trying to jump out of the bed if he didn't get enough sedative on board beforehand. (Healthy athletic young patients (HAY patients, I call them) tend to wake up violently and delirious from anesthetic gas. To mitigate this, there's a sedative called precedex that helps smooth out emergence from anesthesia. Good to give for little kids, teenage girls, and big strong-looking guys. As soon as I see I'll be getting an Akihiko/HAY type patient for an upcoming case, I already know to draw up and dilute precedex to have at the ready.)

Fuuka: actually a very pleasant and compliant patient, but has family history of malignant hyperthermia and allergies to practically everything, apologizes for all the trouble. (Malignant hyperthermia is a very rare, but very deadly anesthetic complication if not treated promptly. Many anesthesia providers go through their entire careers without ever seeing MH, but we're trained to know what to do if it ever happens. Anesthetic gases and a muscle paralytic agent called succinylcholine are MH triggers. The anesthesia machine must be completely removed of the gas canisters and flushed through with high flow oxygen for an hour or so, to really make sure none of that stuff is exposed to an MH patient. I like the idea of Fuuka turning out to be a patient requiring an extensive anesthetic plan when she totally wouldn't mean to)

Ken: the rare kid who's cool with getting an IV in preop. (Pediatric patients typically do not get an IV placed before being rolled back to the OR, as most kids are terrified of needles. Induction of anesthesia in the OR must instead be achieved by delivering high flow anesthetic gas through a mask. Once the kid is unconscious from the gas, then an IV can be placed to give medications throughout a case intravenously. Amada seems like the type to be fine with getting an IV placed when he's awake because that's what adults have to do.)

Aigis: is a robot, physically can't process anesthesia. (Probably goes without saying)

Koromaru: Mitsuru or Akihiko, as the oldest members of SEES, act as guardians to sign anesthesia consent forms. Holds out his front leg and rolls over to offer his chest so staff can put on the blood pressure cuff and EKG stickers. Adored by the vet and vet techs for being so smart and adorable.

Shinjiro: the guy you think would smoke weed and drink a lot but actually has a history of post-operative nausea and vomiting (PONV) and prolonged emergence from general anesthesia. (Somehow I like the idea of Shinjiro turning out to be a delicate flower when it comes to anesthetic requirements)

I dont have a clear idea in my mind but ever since I sketched that dumb AU of Shinjiham in HS i have the feminine urge to make up a HC of them lmao i doubt anybody be reading this but i like talking to myself and reread this the next morning so ima just write this down lol

Like I said I dont have a clear idea but basically Shinji extended his study and ended up in the same year as Kotone (11th grade) because he actually got into a coma. Im playing around the idea that the story revolves around why is he in the same year as she is? Why did he fell into coma and how did he get into coma in the first place? This also means that Kotone didn't meet him until he enrolled into her class. Aki is still her senior and they're friends because idk I havent think of this yet lol but anyway one day Aki asked if she can help him keep an eye on Shinji and told her about him going to enroll in her class soon and he knows Shinji will struggle because he's pretty much an outcast and some people speculates and make rumors about his reasoning for his extension and Aki really cares for him and doesnt want him to fall behind or fail so he asked her a favor to help him out.

Shinji is hard to befriend at first and tell her to buzz off but when she mentioned Aki asked her to keep an eye on him and she won't take no for an answer, he sighed and said how annoying he is for setting him up like that lol At first he is kinda frustrated at how persistent she is on trying to help him and how she gets into everyone's business in class but he's aware of how contagious her presence can be and knows how people around her always be at ease when she's in class so he slowly opened up to her and accepts her kindness.

I dont really want him to be as depressive and suicidal as canon but I can see him having a self-worth issue and having a hard time accepting goodness around him. I kinda imagine at one point Shinji and Aki had a huge fight about him pushing everyone away and screaming at him that he's not as mysterious as he thinks he is and he's too blind by his own pity party to see how people still loves him and it took him a fist on the face and Aki crying to get that through his thick skull.

After the fight, he didn't come to school for a few days which actually worries Kotone because despite how uninterested Shinji always be in class, he never delibrately skip class and oddly enough he always pass his homework on time so she visited his place with his homework packets (and after bribing the office clerk for his address) and when she sees him all bruised up in his face she was like SHINJI WHAT HAPPENED WHY YOURE ALL FUCKED UP WHO DID THIS TO YOU AWAFSJQGRAJALA and he hissed at her to shut up and after she apologised, he admits that Aki punched him and after reassuring her it's technically his fault, he asked if she actually liked him and just as she's about to stammer with red on her cheeks, he quickly corrected himself that what he meant is if she likes being friends with him because he is such an ass to her and opens up about what he fought about with Aki and it's not until he sees tears in Aki's face that he's not the only one hurting and him pushing people away to not hurt them actually hurts other people too. Kotone assured him that if he realized that, its not too late to change and he has friends to help him get back on his feet. When she mentioned "friends", he looked at her and asked "Kotone, are we friends?" And she beamed at him and hook his pinky around her and whispered "Always been." and that's when he really accepts her and sees her as a genuine friend and not some annoying girl in class.

Idk how do I make them ended up dating but I just like the idea of a slow burn relationship so in the meantime they're just friends with budding feelings towards each other hsksksksksk

Headcanon about how the FeMC (who I will refer to by her canon name Kotone cuz its just easier) specifically can survive sealing Nyx when saving Shinjiro:

So this came to me the other day while crying over that romance path but it's not required to be a romantic thing, just that the pocket watch gets involved and that you manage to save Chidori (and didn't like The Answer)

The way I see it is that Kotone's ability to get the Universe card and ultimately cast the Great Seal comes from her boundless kindness paired with her deep, deep desire for connections

She's no chosen one or anything, she just happened to be there when Aigis needed someone to seal Death in and she just happened to be the kind of person who is just fundamentally and boundlessly good while also having the kind of determination and overwhelming ability to love that, given the power to do so, can literally change fate

That's where the pocket watch comes in. Shinjiro was fated to die, there was no avoiding that...until Kotone got involved. She got to know him as a person and came to wish for his happiness the same way she does for everyone else she encounters. She cared so much that she literally heard him off handedly mention the pocket watch once and immediately went out to look for it. No clues, nothing to go on but she went and found it anyway.

And in my headcanon that determination to bring someone happiness affects things around her, including the pocket watch. But in its case, ensuring that meant to literally avert death. The same way Kotone's ultimate goal is to avert Death.

The pocket watch came into contact with her and its purpose and her final act were the same; to change fated death.

So I headcanon that Theodore enlists Elizabeth to find a way to save Kotone and they come up with the theory that if they can find some way to imitate Kotone's powerful desire to protect the world, ensure life, break fate they could possibly use it as a sort of proxy to keep the Great Seal stable while also freeing Kotone

After some time they realise that Shinjiro shouldn't be alive, his fate is strange and playing out in a way it should not so they investigate and find out about what Kotone did with the pocket watch. But the watch alone isn't enough because its not alive.

Then they learn that Chidori came back from the dead and that that was not in the cards (pun intended) either. She shouldn't be alive...but because Kotone kept nudging Junpei to visit her she is. Kotone got involved and indirectly changed fate again

So Theodore, in his typical awkward way, pops into Kotone's hospital room and stays there until it causes a commotion and every member of S.E.E.S. shows up to confront him and protect their precious leader

He explains that he has maybe found a way to save her and that he needs the shattered pocket watch and every left over flower from Chidori's stay at the hospital

It's a desperate thing and no guarantee it'll work but they all want her back. Shinjiro of course has the watch on him, he never goes anywhere without it to remember who gave him this second chance at life. Because even if he can't explain it, he knows that only Kotone could have found it and only because she was the one who gave it back to him was it able to stop that bullet.

There are also some flowers left, not all of them had found a place on Chidori's coffin, and Mitsuru has no problems getting the folks at Kirijo to hand them over to her

And at the stroke of midnight the entire group is gathered around Kotone's bed, the watch and flowers gently placed in her hands, and on Elizabeth's sign they all focus hard on all the good that Kotone brought into their lives, all the warmth she's given them, their love and appreciation for her, their wish for her fate to change.

At first nothing happens...but then the ground shakes, the sky flashes, for a second they see the same thing as that night on top of Tartarus...but then it's gone again, the Great Seal stabilises, the world remains safe...

And, responding to her friends' voices, Kotone opens her eyes.

The pocket watch and flowers are taken away by Theodore and Elizabeth. They are now safely stored away in the Velvet Room. Igor pretends not to know anything.

Besides, who is he to stop the Universe from changing the unchangeable?

He holds a couple of shares, yeah. Not a whole lot as of right now, but he has a few from his mother.

The only ones who know about this are Kotone and Mitsuru. Minato tells no one else about it. Where and how he gets his funds is no one's business. If anyone asks, he just says that he's good at saving, which isn't a total lie.

They had gotten some strange looks from the others in the dorm, but Aigis seemingly did not care.

(Junpei) "Whaaat?! That's like, every guy's dream!"

(Yukari) "More like it's yours..."

Aigis adjusted her position and gave (Y/N) plenty of space on her lap.

Everyone in the dorm winced in pain. They could feel that impact.

(Minato) "That sounded like it hurt."

(Y/N) "Ow, it did!"

(Aigis) "Are you injured, (Y/N)?"

(Y/N) rubbed their head while Aigis continued to stare at them, albeit her eyes seemed to be focusing on their head.

(Junpei) "Oh shit! That's right, I forgot you were a robot!"

(Yukari) "How do you just forget an important detail like that?!"

(Aigis) "It is why I warned them it would not be comfortable."

(Y/N) "And I appreciate it, Aigis but...Now that I know what it's like, maybe I won't just drop my head like that."

(Aigis) "Would a pillow suffice?"

Y/N smiled.

(Y/N) "That'd...be nice, yes."

Minato wordlessly tossed (Y/N) a pillow from the couch and allowed them to fulfill their wish without suffering a concussion.

“so if you press this button, the footage should start playing,” the leader points to the console, directing you on how to play the recordings. other than for meetings and before missions, you’ve never been in the command room before. hell, you didn’t even know there were cameras in the building until now.

the specific recording your leader wanted to show you is labeled ‘september 23 - lounge’. as you press the play button, the lights of the command room dim down as the video begins to play on the screen.

as expected, the recording shows the first floor lounge. shinjiro and junpei sit on the same couch, but at different ends.

“so, aragaki-senpai, what’s your ranking of all the girls in the building?” junpei leans back, hands behind his head. there is a considerable amount of distance between them, both physically and interpersonally, and junpei decided talking about the ladies is the best way to bridge that distance.

“why would i care?” shinjiro scoffs, his crossed arms not helping the already-tense air.

“oh, come on! with so many beautiful ladies here, you’ve gotta have a ranking by now! like, who’d you rather be alone in a room with? or see in a bikini?” junpei begins listing off the girls in an attempt to convince shinjiro to dig deep into his thoughts and desires. “there’s kirijo-senpai, with that air of elegance and maturity; yuka-tan, a pretty chick very popular among guys our age; fuuka, an all-around cutie; [name], who is… hm…” he didn’t even reach aigis before running out of adjectives. trying to think over his very limited dictionary, he briefly hesitates.

shinjiro visibly perks up upon hearing the last name listed. noticing this, junpei exaggerates his struggle to think.

“hm… what is there about [name]…” junpei rubs his chin, staring at his senpai as his face morphs from an expression of indifference to one of annoyance.

“you’re kidding me, right?” aragaki scowls, sitting up slightly. “you seriously can’t think of anything for [name]?”

“well, she’s just… eh…” junpei trails off. “i wouldn’t rank her very high.”

“i know you’re a moron, but i didn’t know you could be this much of a dumbass,” he leans forward. without even raising his voice, the simple action caused an air of intimidation around him that could be felt even through the screen. “the hell do you mean ‘you wouldn’t rank her very high’? are you fucking blind?”

“nonono, man, she’s attractive-“ junpei frantically tries to explain himself, backtracking on his original plan of getting a reaction out of shinjiro because finding out his senpai’s type was not worth getting his ass beat. but, aragaki continues.

“don’t tell me you’d rank her lower than a goddamn robot!” this was possibly the most passionate he’s ever been, and it was over a casual conversation of ranking the girls in their dormitory building. “she better be in at LEAST your top three or i’m mopping the floors with your ass.”

“no, dude, she’s in my top three, i swear!” junpei’s attempts at damage control were getting more desperate. “she’s probably number one!”

“…number one?” aragaki repeats, as the two sit in a heavy silence for a moment. “you don’t deserve to have her in your number one spot,” he mutters.

“…what?”

“i SAID you don’t DESERVE to have her in your number one spot!!”

“OKAY THEN SHE’S NUMBER TWO!!!” junpei raises his hands up in an act of surrender. “she’s second! [name] is second!”

“…”

unable to listen to anymore, you hurriedly hit the pause button. you feel hot, and as the lights turn back on, you make a futile attempt to cover your burning face with your hands.

your leader clears his throat. “so, should i set you two up on a date, or-“

“no!” you exclaim, cutting him off with a wide-eyed expression on your face. “no! no.” you calm yourself down with a deep breath, trying not to imitate junpei’s desperation shown in the video. trying to collect yourself, you add, “that won’t be necessary. shinjiro didn’t even say his own ranking, so all of that probably could have meant nothing. absolutely nothing. right.”

“i mean, if you finish watching-“

once again burying your face into your hands, you yell into your palms to cut him off. a typical response from a teenage girl finding out her crush laid his pride on the line to advocate for her attractiveness.

the leader, ignoring your wishes, presses the play button.

“where would you put [name] then?” junpei asks, his signature shit-eating grin on his face. the video seems to have skipped ahead, as evidenced by junpei being much more calm than earlier.

shinjiro hesitates.

the quality is a bit fuzzy, but you can see junpei having a perplexed expression as he leans in to get a better look at shinjiro’s face, who turns his head away.

“are you…” junpei squints, then his eyes widen in surprise. “are you blushing??”

“the hell? no way i am!” shinjiro turns his body away.

“oh man, you should’ve just said you like her!” junpei grins, trying to be a supportive bro!

“i-i don’t even think of her like that! just lay it off!”

“so, what about her, huh?” his excitement shows in his voice. “she’s pretty cute and all. oh, those eyes are gorgeous-“

“i said lay it off!” shinjiro exclaims, and junpei jumps.

“ok man, ok! that’s my bad!” junpei backs up, and the awkward silence returns once more. without saying a word, shinjiro gets up and leaves.

at this point, you’ve sat down. the leader turns around to look at you, your face buried into a pillow. your ears are practically glowing red.

“my offer earlier about setting you two up still stands-“ he begins, but as you did before, you cut him off.

“shut up!!!!” you scream into the pillow, kicking your feet. you lift your face from the pillow to meet his gaze.

he can’t help but laugh. “should i get yukari? she probably knows more.”

admitting defeat, you nod.
