Like if the words switched: "But still Dean Winchester. Still beautiful." It would emphasize Dean's external beauty.

But because Cas thinks "beautiful" and then elaborates that what he finds beautiful is "Dean Winchester" that means that what Cas finds beautiful is not Dean's outer shell but everything that makes up Dean.
