So in this verse, Tsunade never ends up teaching Sakura- as that role is fulfilled by Chiyo. (But she DOES return to Konoha eventually).
And Chiyo's first rule (for everything lol) is: Nothing is Free.
Chiyo always demanded something in return for healing, and unlike Tsunade, she considered medical jutsu a part of her arsenal. If it could kill, then it was always an option.
So Sakura learned from that mindset, trading her services and using her rare skillet as a powerful negotiating tactic, especially during Tea Country's civil war. Her healing effectively kept her, Enji, and Saeko in relative safety and provided her a card to play when she needed.
And, while Sakura is nowhere near as jaded as Chiyo is, and often enough of a bleeding heart to help people for damn near free, (a place to stay, food for the night, a new holster for her kunai), she is also VERY pragmatic about her skillset. The civil war taught her that sometimes, letting someone die is better. This puts her at odds with Tsunade, who has a very different mindset (and was, historically, bitter enemies with Chiyo).
Another issue that arises with Tsunade, is that the Godaime is a doctor- while Chiyo is more of a healer. Their understanding of the human body comes from a different place.
Tsunade can name every bone and muscle and perform open-heart surgeries. Sakura can heal bone and stitch back together muscles and flesh on a very basic level, but she's never going to be able to open up a human body and revolutionize medical history like she does in canon. (In canon, Sakura creates a Jutsu that allows her to do a four-man surgery alone; she can't do that here.)
However, she can do some stuff that Tsunade claims is impossible—like reconstructing/altering someone's chakra network, unblocking tenketsu, sometimes turning off someone's dojutsu from a distance, and lite-healing remotely (Tsunade is very upset about this one specifically). A lot of it comes from her mastery of Genjutsu.
Genjutsu, as I reworked in my comic, requires a complete understanding of someone's chakra network to control ALL their senses. Sakura's understanding and control are so prodigious that she can almost autonomously control people's bodily functions (a skillset that is GREATLY aided by training with Chiyo's puppet mastery).
She uses Genjutsu as a tool to understand the body and employs it as both support (helping an ally maintain a sense of gravity as they're pulled underground or flung in the air, etc) and offense.

"What? My body...It's breaking!"
I'm willing to bet you REAL money Bela listened to Alcina's warnings (like a good obedient daughter) and avoided the cold just like her mother said. When she said this line she was genuinely confused and scared at what was happening to her body which tells you that for almost a hundred years she never put herself in any situation where she was exposed to the cold because this is legitimately her first time seeing the cold's effect on her.
Cassandra, on the other hand, did say "I have to get warm" which means that she knows what's happening probably because this is not her first time dealing with the cold and its effect on her (it is probably a mix of rebelling against her mother's rules and finding out the hard way what the cold does to their bodies and also because of some maids' assassination attempts because they always want to start with the most dangerous sister thinking it's actually doable). Anyway, as I was saying, Cassandra wasn't surprised at her body's reaction to the cold because she's been there before and she was kinda used to it but Bela was dealing with it for the first time in her life and she was genuinely scared/surprised at what's happening.

AK-Amputee!Orel
I've already seen mobility aids hcs for Orel, but taken his initial physical trauma + shit show of a medical help he received for it, it's not impossible for his leg to start deteriorating in the future.
i think it's canon that he got a nasty femur fracture from that shot. they were hunting game at least as big as a deer, so the bullet Orel got was also scary big. there's no way he got away with just a crack. it probably was comminuted fracture, it requires more intervention than just a cast. so in the end, his limping was probably coming from incorrectly healed bone and tendon I imagine once he is able to seek competent medical help as an adult, it would be a bit to late to regain anything. In the end, above the knee amputation becomes the only other option.
Of course, amputation comes with it's own issues like nerve damage and phantom pain etc., but is still be better that nothing. Also, he would be able to resume tracking once he heals and learns how to walk and run with a prosthetic.

When she stood outside the bars, however, peered in at that loathsome little mirror on the wall, he was ready to face her. That is, he’d convinced the faint flutter in his chest that he was.
“You’ve placed a magical mirror in which I am already very securely contained—trust me, I’ve had plenty of time to determine that’s the case—within a literal prison cell,” he observed blandly, head cocking while a bitter smile tugged on his lips. “Is that not just a touch excessive, my dear?”
