Finally got a diagnosis for what’s been going on with me for so long! I have hyperthyroidism. We think it may have started around four years ago so it’s been left untreated for quite a while which is a big concern. I’ve got to start medication and be re-assessed in three months time. There is a possibility I may have to have surgery or some kind of iodine therapy but we don’t know for definite yet. I’m really, really hoping the medication works & I don’t need surgery because just the thought terrifies me! However it feels good to know the symptoms weren’t all in my head & that there actually is a diagnosis for what’s going on in my body. Fingers crossed things with my health start improving now🤞

When I went to the hospital yesterday, I was half expecting to get told "your heart is bad because you're a bit fat" and then being sent home. But they... Actually treated it seriously? Like... They did everything that they would do with a thin person?
And when we discussed diet, I was told "Don't constantly weigh yourself, don't constantly count calories. Focus on how your body feels, and spread weight loss out over months and months to create sustainable habits."
Have I found the one non-fatphobic hospital?

Because my most popular post is about weight loss and how it's a crock, I get a lot of questions about various things, including bariatric surgery--just posted the link to the post I did about that--but also Ozempic/Wegovy, the once-weekly injectable semaglutide medication that was developed for diabetes but was found to have independent benefits on weight loss.
I always said that weight loss was like Viagra: when a medication came along that actually worked, it would explode. We'd all hear about it. Fen-phen in the 90s worked, but it was bad for your heart. Stimulants, like meth, may cause weight loss, but they do it at the cost of heart health, and raise your likelihood of dying young. Over the counter weight loss supplements often contain illegal and unlisted thyroid hormone, which is also dangerous for the heart if taken in the absence of a real deficiency. Orlistat, or "Alli," works the same way as the Olestra chips Lays made in the 1990s--it shuts off your ability to digest fats, and the problem with that is that fats irritate the gut, so then you end up with fatty diarrhea and probably sharts. Plus Alli only leads to 8-10lbs of weight loss in the best case scenario, and most people are not willing to endure sharts for the sake of 8lbs.
And then came the GLP-1 agonists. GLP stands for glucagon-like peptide. Your body uses insulin to make cells uptake sugar. You can't just have free-floating sugar and use it, it has to go into the cells to be used. So if your body sucks at moving sugar into the cells, you end up with a bunch of glucose hanging out in places where it shouldn't be, depositing on small vessels, damaging nerves and your retinas and kidneys and everywhere else that has a whole lot of sensitive small blood vessels, like your brain.
Glucagon makes your liver break down stored sugars and release them. You can think of it as part of insulin's supporting cast. If your body needs sugar and you aren't eating it, you aren't going to die of hypoglycemia, unless you've got some rare genetic conditions--your liver is going to go, whoops, here you go! and cough it up.
But glucagon-like peptide doesn't act quite the same way. What glucagon-like peptide does is actually stimulating your body to release insulin. It inhibits glucagon secretion. It says, we're okay, we're full, we just ate, we don't need more glucagon right now.
This has been enough for many people to both improve blood sugar and cause weight loss. Some patients find they think about food less, which can be a blessing if you have an abnormally active hunger drive, or if you have or had an eating disorder.
However, every patient I've started on semaglutide in any form (Ozempic, Wegovy, or Rybelsus) has had nausea to start with, probably because it slows the rate of stomach emptying. And that nausea sometimes improves, and sometimes it doesn't. There's some reports out now of possible gastroparesis associated with it, which is where the stomach just stops contracting in a way that lets it empty normally into the small intestine. That may not sound like a big deal, but it's a lifelong ticket to abdominal pain and nausea and vomiting, and we are not good at treating it. We're talking Reglan, a sedating anti-nausea but pro-motility agent, which makes many of my patients too sleepy to function, or a gastric pacemaker, which is a relatively new surgery. You can also try a macrolide antibiotic, like erythromycin, but I have had almost no success in getting insurance to cover those and also they have their own significant side effects.
Rapid weight loss from any cause, whether illness, medication, or surgery, comes with problems. Your skin is not able to contract quickly. It probably will, over long periods of time, but "Ozempic face" and "Ozempic butt" are not what people who want to lose weight are looking for. Your vision of your ideal body does not include loose, excess skin.
The data are also pretty clear that you can't "kick start" weight loss with Ozempic and then maintain it with behavioral mechanisms. If you want to maintain the weight loss, you need to stay on the medication. A dose that is high enough to cause weight loss is significantly higher than the minimum dose where we see improvements in blood sugar, and with a higher dose comes higher risk of side effects.
I would wait on semaglutide. I would wait because it's been out for a couple of years now but with the current explosion in popularity we're going to see more nuanced data on side effects emerging. When you go from Phase III human trials to actual use in the world, you get thousands or millions more data points, and rare side effects that weren't seen in the small human trials become apparent. It's why I always say my favorite things for a drug to be are old, safe, and cheap.
I also suspect the oral form, Rybelsus, is going to get more popular and be refined in some way. It's currently prohibitively expensive--all of these are; we're talking 1200 or so bucks a month before insurance, and insurance coverage varies widely. I have patients who pay anything from zero to thirty to three hundred bucks a month for injectable semaglutide. I don't think I currently have anyone whose insurance covers Rybelsus who could also tolerate the nausea. My panel right now is about a thousand patients.
There are also other GLP-1 agonists. Victoza, a twice-daily injection, and Trulicity, and anything else that ends in "-aglutide". But those aren't as popular, despite being cheaper, and they aren't specifically approved for weight loss.
Mounjaro is a newer one, tirzepatide, that acts on two receptors rather than one. In addition to stimulating GLP-1 receptors, it also stimulates glucose-dependent insulinotropic polypeptide (GIP) receptors. It may work better; I'm not sure whether that's going to come with a concomitantly increased risk of side effects. It's still only approved for diabetes treatment, but I suspect that will change soon and I suspect we'll see a lot of cross-over in terms of using it to treat obesity.
I don't think these medications are going away. I also don't think they're right for everyone. They can reactivate medullary thyroid carcinoma; they can fuck up digestion; they may lead to decreased quality of life. So while there may be people who do well with them, it is okay if those people are not you. You do not owe being thin to anyone. You most certainly do not owe being thin to the extent that you should risk your health for it. Being thin makes navigating a deeply fat-hating world easier, in many ways, so I never blame anyone for wanting to be thin; I just want to emphasize that it is okay if you stay fat forever.

If y'all who actually care about my obsessive doberman posting ever wonder why I'm cagey about who I recommend nowadays, and why I don't name people who are "known greats" in the breed, consider:
Person C has a dobe produced by Person A and Person B. B and C are good friends. B has made some highly questionable breeding decisions including using dogs with known cardiac health problems and covering up thyroid problems by representing the dogs as "thyroid normal" but not including the detail that they're only normal while actively on thyroid medication.
Person C posts that their dog from this pairing is severely low thyroid at a fairly young age, and that they've decided not to breed their dog as a result. Person A publicly lamblasts C, calls it not that big of a deal, and swings by C's house to collect semen anyway. C deletes previous post, reports the photo, and the new caption makes no mention of thyroid disease but instead talks about how the dog passed all the other health certs with no issue, implying that the dog is perfectly healthy.
Consider:
Person Z has two dogs they have titled to impressively high levels in protection sport. Just before breeding, Z gets very bad results from heart testing on both dogs. One dog definitely has DCM, the #1 breed killer that takes over 50% of dobes from us by way of sudden death with no other symptoms, ie the dogs just collapse and die one day or die in their sleep with no other warning (unless you're doing rigorous heart testing). The other dog has concerning results indicating that DCM is on the horizon but not yet diagnosable.
Z puts both dogs on heart meds designed to combat DCM. The dogs' next test results return to normal. Z uses this normal result to say the dogs have healthy hearts and breeds the dogs anyway. Z has made no mention to the buyers of this litter that the parents both effectively have DCM, albeit well controlled with medication thus far.
If I did not have all 4 of these people on Facebook I would not know this about them. If someone were to friend them currently right now, they would not find evidence of this, because the posts with evidence have been deleted. There is no way to know this about them without knowing in advance.
When I say that the people I personally would get a dog from is a very small, exclusive club nowadays, I don't mean just because I'm particular about temperament, though that is certainly part of it. It's because I've watched this behavior from a LOT of otherwise popular, well-respected, well-liked breeders and I know if I named names their fan bases would crucify me, but I've also seen the wrongdoing with my own eyes or even in person.

Just woke up. Can’t wait to finish getting my degree(s) and work in cognitive psych research and wherever else life takes me in my mission to expose how psychiatry, and clinical pharmapsychology, is a money-grab pseudoscience that has everyone by the balls. The doctors of TikTok will be so mad, the feds will be listening, I will keep publishing. Y’all are gonna have to kill me. Everyone has a mental illness now, not even realizing how much it’s all a byproduct of advertising. “De-stigmatizing” my ass, you’re a customer. Psychiatry is inherently fascist and cultish. They never did a test for the “chemical imbalance” they said you have, did they? They “just know”, because their Bible tells them so, and so on and so forth. Once you are named, the authority to hold you against your will is handed over as your privacy is violated. You are never given clear instructions for recovery either, despite the “YEARS of research proving your illness”— kindness and responsibility towards yourself involve prioritizing your daily dose, presumably for life. Mischief managed. Kidney failure or thyroid disease because of all the lithium you take? Solution: another doctor, another cocktail.
Diagnoses work like activation phrases. There is a mental illness for believing you have a mental illness when you don’t— imagine that. A doctor once told me, when I was at the height of crisis, that it is difficult to know whether highly intelligent people are sick or not— that’s when I knew, I took control ever since. With that being said, a lot of what people experience is real, that isn’t what I’m trying to say. We just aren’t as helpless as they make us seem. Whatever. So much to do. I’m gonna be the female Szasz of my generation. We’re gonna get fucking freeeee! My psychiatrist has no explanation for my remission, I’m bi-winning and sometimes I flush my Adderall down the toilet. Sometimes I take it when I need to be a zombie, for the sake of others, of course.

The Best Workout that Changed My Body (Model Workout)
Growing up, I was a chubby kid. As a teenager, I was “skinny-fat” - I had a tummy but lean arms and legs. I have tried everything to burn that belly fat off; right from detox teas, waist clinchers, sports, Pilates, limiting my calorie intake, weight lifting, strength training, eating only health food for months, lemon water, even K-Pop stars’ diets… you name it and I’ve probably done it.
Then I finally found something that worked out for me. I stumbled across it on Reddit, and I shared it with my friends, not thinking that it would actually work.
Except that it did.
A friend who had gained 10 kgs because of her thyroid lost nearly 8 kgs in a matter of a couple of months. Another friend - who’s quite skinny and petite and struggled with weight gain - saw her body get more toned in just a couple of weeks.
So I decided to try it out as well - and I could actually see a difference in just a week’s time (as crazy as this sounds). A few pants I had that I always felt a little insecure about wearing actually felt LOOSE at the waist! I couldn’t believe it!
Now, for a little disclaimer. The person who created these workouts happens to train models. They are specialised in model training- reducing waist cm, hips is their specialty. HOWEVER. They were accused of filming their clients in their gym bathroom. Their spouse eventually took over the company. But that still doesn’t sit right with me. So that means that I will not be buying any of the company’s products, even though the work outs are really effective and require minimum equipment.
So I did a little bit of sleuthing - and found 5 out of 6 of their workouts for free on Reddit.
I’m going to share the link that I found on Reddit. I am NOT responsible for uploading the files. I have no role to play in it. These have been available on Reddit for literally years. I have no idea who the original distributor is, either. Also, these videos are quite old. I’m sure that the new ones are different from these.
The workouts range from 20-60 minutes (most of them are at 40 minutes though). The best part is that you can do these anywhere.
The only equipment you really need:
- yoga mat
- Slide discs/ wash cloths
- Ankle weights
- 3 pound dumbbells

The nutrition guide:
(I have not come up with this. Do not hold me responsible if you do not agree with what the trainer says).
1. Calorie intake: 1600 calories
2. No juice, rice, pasta, potatoes, bread 2-3 hours before you workout. If you must eat, it has to be low calorie protein or lean (eggs, protein shake, lean protein, salad, vegetables).
3. No carbs after 3 pm.
4. Dinner should be protein and vegetable based.
5. No protein for 1-2 hours after workout. Fruit juice, fruits, vegetables and starchy carbs are fine.
6. Only treadmill for cardio. Slow jogs. No weights for lower body, outside of the program. No lunges, squats or deadlifts. Yoga and Pilates are fine. I personally do 12-3-30.
How I use this routine:
Monday: workout 1
Tuesday: 12-3-30
Wednesday: workout 2
Thursday: 12-3-30
Friday: workout 3
Saturday: workout 4 +12-3-30
Sunday: 12-3-30 / yoga/ rest day
I take 5 days off in a month during my period. These workouts are also not crazy intense (I’m used to playing sports and being active in general) so I’m fine doing it everyday.

The Rosicrucian Fellowship (TRF) ("An International Association of Christian Mystics") was founded in 1909 by Max Heindel with the aim of heralding the Aquarian Age and promulgating "the true Philosophy" of the Rosicrucians.[1] It claims to present Esoteric Christian mysteries or esoteric knowledge, alluded to in Matthew 13:11 and Luke 8:10, to establish a meeting ground for art, religion, and science and to prepare the individual through harmonious development of the mind and the heart for selfless service of humanity.[2]
The school's teachings hold that man is a Spirit with all the powers of God, powers that are being slowly unfolded in a series of existences (rebirths) in a gradually-improving body, a process under the guidance of exalted Beings ordering our steps in a decreasing measure as man gradually acquires intellect and will. For this purpose we live many lives[18] of increasingly fine texture and moral character. Through the "Law of Cause and Consequence" we constantly set new causes into operation which will create new destiny to balance and improve the old destiny brought from the past. All causes set into action in one life cannot be ripened in one existence but "Whatsoever a man soweth that shall he also reap".

The theological consideration of art and beauty is made complex by the secularization of the contemporary world, which raises the question of the relation of aesthetics to Christian “conversion.”

The effects of dancehall genre on adolescent sexual and violent behavior in Jamaica: A public health concern
Aim: To determine the extent to which dancehall music/genre impacts adolescent behavior.
Results: Of the 100 adolescent cases (50 male, 50 female), females (40%) were more likely to gravitate to sexually explicit lyrical content than their male counterparts (26%). Females (74%) were also more likely to act upon lyrical contents than males (46%). There was no significant difference where males (100%) and females (98%) subconsciously sings the dancehall lyrics even without hearing it; as well as inspire their dreams 64% and 62% respectively. However, more females (74%) than males (46%) acted on lyrical contents of the dancehall genre.
Dancehall Culture
Donna P. Hope defines dancehall culture as a "space for the cultural creation and dissemination of symbols and ideologies that reflect the lived realities of its adherents, particularly those from the inner cities of Jamaica."[41] Dancehall culture actively creates a space for its "affectors" (creators of dancehall culture) and its "affectees" (consumers of dancehall culture) to take control of their own representation, contest conventional relationships of power, and exercise some level of cultural, social and even political autonomy.

Favela (Portuguese: [fɐˈvɛlɐ]) is an umbrella name for several types of working-class neighborhoods in Brazil. The cocaine trade has affected Brazil and in turn its favelas, which tend to be ruled by drug lords. Regular shoot-outs between traffickers and police and other criminals, as well as assorted illegal activities, lead to murder rates in excess of 40 per 100,000 inhabitants in the city of Rio and much higher rates in some Rio favelas.[27] Traffickers ensure that individual residents can guarantee their own safety through their actions and political connections to them. They do this by maintaining order in the favela and giving and receiving reciprocity and respect, thus creating an environment in which critical segments of the local population feel safe despite continuing high levels of violence.
Drug use is highly concentrated in these areas run by local gangs in each highly populated favela. Drug sales run rampant at night when many favelas host their own baile, or dance party, where many different social classes can be found. These drug sales make up a business that in some of the occupied areas rakes in as much as US$150 million per month, according to official estimates released by the Rio media.

A total of 137 urine samples and 46 serum samples, corresponding to 154 self-confessed designer drugs consumers in Ibiza island, were analyzed for the presence of designer drugs: amphetamine and amphetamine derivatives (methamphetamine, methylenedioxymethamphetamine (MDMA), methylenedioxyethylamphetamine (MDEA), methylenedioxyamphetamine (MDA), p-methoxymethylamphetamine (PMMA), p-methoxyamphetamine (PMA), etc.), ketamine and γ-hydroxybutyric acid. Among this population, coming both from the forensic clinic and from the emergency room of a hospital, a total of 99 cases were found positive for some designer drug. This study shows the prevalence of methylenedioxymethamphetamine (MDMA) among designer drug users, sole or in association with other drugs. Also, the mixture of MDMA with other designer drugs, ethanol and/or cocaine is shown to be more likely to produce toxic symptoms requiring clinical attendance in a hospital emergency room. These findings along with the consumption history, the concentrations of drugs and metabolites in urine and serum and the toxicological significance for the interpretation of some MDMA metabolites such as 4-hydroxy-3-methoxymethamphetamine (HMMA) are discussed in this study.

Music as an Element of Tourism Innovation: Types of Nightlife Premises in Ibiza (Spain)
The island of Ibiza is a western Mediterranean destination known internationally for its nightlife. The aim of this paper is to make a proposal to classify the different types of premises in the Ibiza nightlife offer. This involves making a first definition that allows to delimit which businesses are parts of the sector. The methodology used is based on the case study and specifically, on the review of the promotional actions and activities carried out, completed with the visit to the premises. The classification has been made based on the offer marketed and not only on the legal forms used, as innovation goes ahead of the existing legal classifications. Although it is a particular application, due to the international importance of Ibiza, it is a good starting point to classify the nightlife offer of many other tourist destinations. The resulting typology divides the sector into two large groups: nightclubs and other premises. While nightclubs have musical parties as their main activity declared, the other premises have accommodation or catering as their main activity, with music being an element of differentiation. Nightclubs are divided into several subgroups, depending on their size and relevance. The other premises are subdivided into Beach Clubs, Hotel Clubs, Party Boats, Lounge Clubs, Disco Pubs, among others.
The impact of global nightlife in Ibiza: Youth tourism
The promotion of the island’s image through its parties with the release of record labels, elaborate shows (Bensa, 2002), and the transformation of buildings into large entertainment businesses (Rosa, 2002), made it possible to return to the already known name of Ibiza and to catapult it as a symbol of entertainment and partying (Allemand, 2010). This development was something that occurred not only at the national and international scale; rather, it was something that transformed the island into a global icon (Amirou, 2000). The process of reinterpreting the hippy alternative image in a contemporary global context (Jauréguiberry, 1994), came from local businessmen (from the island and from elsewhere). They were the actors who understood the authenticity of the island to convert it into a unique product that went beyond tourism. Given the party reputation of the island, the traditional sun and beach tourists began to stay away, which made it possible to attract visitors who were willing to spend large sums of money to experience the nightlife in Ibiza. There was a shift from tourism to the experience of shows and culture. In this manner, by the mid-2000s, Ibiza had become internationally renowned in the electronic music industry (its own record labels and invited DJs) and for its nightclubs (Goulding, 2011) (such as Space, Privilege, Amnesia, Café del Mar, Pacha, Ushuaia, and Esparadis); it also has become one of the most sought-after stages during the summer in show business (with parties organized by the Matinée group, La Troya, Supermarxé, and Circuit). Simultaneously, from the cultural perspective, the construction of the image of the island had a minimal physical impact and, on the contrary, made it possible to emphasize the natural Mediterranean environment of the island and the libertarian heritage of the reinterpreted hippy movement (Perrot, 1997). With its network of nightclubs (Sönmez, 2013), Ibiza began to generate an entertainment niche that attracted record labels and international DJs, which transformed Ibiza into not only one of the destinations in the global circuit but also a destination with international renown, almost exclusively during the summer period (Fontaine, 1996). The dissemination of the reputation of its parties crossed borders, attracting people from all over the world to Ibiza. Simultaneously, it became a world-renowned destination through the dissemination of its parties through video, the Ibiza-based record labels, and the lyrics to songs that include Ibiza as an icon and that have been emulated in other parts of the world, such as “Ibiza’s Party.” This highlights the island’s own style, which is defined by freewheeling parties and a relaxed environment that goes beyond labels or conventionalism. We find ourselves with the case of one of the few non-urban destinations that has managed to concentrate and attract nightlife (Fig. 5), even going so far as to displace some of the international record labels and entertainment events in the main cities.

Leverage items: components that are important for the company but sourced from low-risk markets with an abundant supply. For this type of component, the company tends to make the most of its bargaining power and the abundance of the offer with frequent negotiations.
Bottleneck items: components with a low business impact in economic terms but where supply continuity is at risk. The management of these components should be aimed at creating relationships of collaboration in the medium-long term between customer and supplier to guarantee the supply, with less emphasis on the cost.

Regarding the future of certain projects TLDR they are on hold due to my health
Hello everyone and good day/evening/night. Not that I'm super active on Tumblr anymore outside of my projects, but I figured I would explain that these projects are now on hold until further notice. My plan was to continue to translate SINo content. I have a few EoS character gacha messages finished, as well as some job/weapon stories. Summaries of Aoki's streams. Break down of lore exclusive to the JP server. Chapter 7 of Act of Desire nearly done for a few characters. I had planned to scan and maybe try to formulate a team to translate the official movie pamphlet and the light novel. I spent my time on Tumblr from the start of the game in JP gushing about this game, and it still means a lot to me, but in regards to editing, scanlating, translating, ect. It's gotta be on hold. Medical vent: For two years, I had drastic changes in my body. This included a cyst that needed to be surgically moved inside of my body. Eczema, suddenly, with no history of skin conditions. Hormonal changes. Two years of it being hard to swallow, my throat always hurting, and having a cracked voice. Two years of never having energy, always tired, constantly having throat and ear problems. A few weeks ago, I went to do a check-in in regards to the cyst removal almost two years ago. Apparently my thyroid had gotten so large that my doctor noticed right away. Blood work, ultra sounds, several tests later, we're looking at a papillary thyroid cancer diagnosis. It's a scary thing to hear, but it can be treated. Mine is contained to the thyroid. Sadly, I won't even be able to book my surgery until the end of April. This has understandably taken a toll on the ones close to me, so I ask that people who know my circle respect them. This is not the end or a goodbye from me. There's so much I want to do and share with you all, but it is beyond my capabilities for awhile. Thank you all for all the support over the years. This site from my dinky little preci0us m3tal days before SINo came out to now, to the people who supported my writing and art before I started tl projects. I thank all of you. Whether we are friends, followers, or just passerby's I got a lot of love and support over these years. I will be back better than ever soon. I will need a removal surgery of my thyroid first. I will share a few things I did manage to finish in the coming days [job/weapon story-wise].

Hysteria isn't a fucking thing
ok fun fact: I'm rapidly becoming a cult favorite doctor among our local privileged elderly white ladies, which I have mixed feelings about, but the #1 reason is that I just don't leap to "anxiety" as an explanation for symptoms unless the patient tells me "I am anxious, and then I feel these symptoms, and when I am not anxious, I don't feel these symptoms."
The sheer number of women I've seen who've been told for years to decades that the only thing wrong with them is anxiety is fucking staggering, in this Year Of Our Lord 2023, and I just keep digging. We checked a basic lab panel, sure. CBC. No anemia. CMP. Kidneys are fine. (Electrolytes are basically always going to be fine if someone is well enough to walk into my office under their own power to talk to me. Exception is mild chronic hyponatremia.) And we check thyroid. TSH and free T4. We check blood sugar. A1c, if the fasting is a little weird. Fasting insulin, if I'm still suspicious. We check cortisol. Inflammatory markers--ESR and CRP.
And eventually, if the symptoms support it, or right away, depending on my level of suspicion, we check rheumatological labs for abnormal autoimmune function. Anti-nuclear antibody. Rheumatoid factor. There's at least a dozen you can check, and which ones you should check is always a matter of debate and also of expertise that I 100% lack. We are out in the sticks. There are no "local" rheumatologists for me to send people to.
But a couple of weeks ago I found a woman--she has bipolar disorder and has been told for decades that's all that's wrong with her--who has an anti-centromere antibody titer that's fucking through the roof. I found an anxious 19-year-old with an ANA of 1:1380. And yesterday I found out why a sweet elderly woman I've seen for a year or two now started feeling crappy months ago: her rheumatoid factor is over 90.
Rheumatological disorders are always difficult. Our understanding of them varies from "pretty good, actually, and here are useful treatments" to "Well I Guess That Exists." Labs aren't always a slam-dunk and even labs plus symptoms can give you misleading impressions. Your immune system can decide that virtually any short chunk of protein is an enemy, and the problem with that is that your body is made up of many, many, many short chunks of proteins, so the odds that you'll develop some kind of antibody against yourself just keeps going up over your lifetime. Immune disorders tend to travel in packs; there's a clear genetic element to it, so the more first-degree relatives (parent, sibling, child) you have with any kind of autoimmune disorder (including Type 1 diabetes), the higher your risk of any kind of autoimmune disorder is, and if you already have one autoimmune disorder, you're at higher risk for developing another one.
But I think it's precisely because they're difficult that a lot of mainstream primary care prefers to pretend they don't exist, rather than try to sift through the utter fucking mess that is Mixed Connective Tissue Disorders, a title that has fallen out of favor since I learned it in my third year of med school. And women are at higher risk for autoimmune disorders than men. And older women are at higher risk than younger women.
So if I, as a family doc, just keep digging, just keep poking at the tangled knot of symptoms, there's a decent chance I will uncover something interesting. Hopefully something treatable. Sometimes we have nothing to treat with, and I just get to offer someone more understanding of their disorder, which feels pretty paltry but is better than the casual dismissal of "You're just anxious."
Never, ever, ever take anxiety as a diagnosis for a symptom other than anxiety. Not even as a rule-out. Keep those symptoms as an open question mark on the patient. Don't say "anxiety" just so you can close the door. And damn sure don't do it to women.

Hi Laura, I'm training for my second marathon right now and idk how else to say this but it seems like I'm gaining weight and while I know weight/what you look like isn't everything in life it makes me feel uncomfortable. I feel sluggish and heavy and not invigorated and excited. I run in the mornings, eat healthy, drink water etc. I will say that my job is WFH so I don't move much outside of those morning runs. I just don't feel good and was wondering if you had any insight?
Hi friend- I can only offer insight from my own personal experience. I gained weight involuntarily leading up to the 100 mile race and then even after the race was over. We were never able to figure out the issue, but I really think it was related to immense stress on my body and adrenal/hormone dysregulation from training.
You may want to go see your primary care provider and get some blood work done just to make sure your thyroid and testosterone/DHEA levels look okay, too. We checked this and this was one of the reasons I was misdiagnosed with PCOS.
The thing that helped me most? Taking a step back from training, walking more, doing very low HR runs, and just generally running when I wanted to, not because I felt like I had to. I also started taking cold showers, doing breath work, and now I do cold plunges to calm my nervous system down: It took me probably 8 months total to feel normal again but it was a pretty extreme case I think due to my 100 miler.
If you aren’t doing things that help calm your nervous system down, you need to! We activate our sympathetic (fight or flight) nervous system with running but you do have to counteract that to have balance. The cold showers, mindful breathing and walking, reading- can all help. My friend Hannah calls it “The Great Slowing Down.” 🥰
All of that to say you are soooo much more than your weight and your body can and will continue to do incredible things. I know we feel pressure as runners to fit a certain mold and body type. But I encourage you to follow runners with all sorts of body types as a reminder that runners come in all shapes and sizes. I love Latoya Shauntay, Mirna Valerio, Lucy Bartholomew, Alison Mariella Desir, Tessa Chesser. Just a few that come to mind!
