'Ideal helpmate', at the time, was a synonym for spouse.
He is calling Watson his spouse.

I remember in the books when Holmes mentions his 'other friend' from uni and the readers just going W H A T ? Holmes had a friend before Watson? And then one reads on and ahhhhhh of course Holmes didn't come out of his rooms for anything but chapel and OF COURSE he didn't talk to anyone of his own volition the other bloke's dog bit his leg off or something and the chap HAD to come and visit Holmes while he was recovering and NATURALLY anyone forced into close proximity with Holmes for any amount of time becomes his bosom friend, as proved by Dr Watson.

i'm mildly upset about the fact that when sherlock first meets john, john says 'he held out his hand as he spoke, and i noticed that it was mottled allover with similar peices of plaster, and discouloured with strong acids.' and i have yet to see an iteration in which sherlock's hands are covered in band-aids. like, i don't know why i just really like the idea for sherlock's hands being absoultely covered in band-aids.
