It's also great to see a humanized side of working in medicine, so thank you for your openness :) You mentioned recently not prescribing bariatric surgery to patients except in rare, specific cases. If you have time and energy, would you be able to share a little more about what you think about bariatric surgery when those particular conditions aren't present? When someone is fat to the point where they can't do daily activities of living like dressing themselves, walking, etc., then bariatric surgery probably has a place.
However, bariatric surgery has risks. Lots of them. To start with, there's the on-the-table risks. These are a lot lower than they used to be--anesthesia in this day and age is incredibly safe. Getting to bariatric surgery is challenging for most patients, as insurance in the US will typically only work with a few centers that have wrap-around teams including the surgeons but also other specialists, especially nutritionists. So lots of patients go to Mexico. I haven't had a single one of my own patients, since I started having my own patients four years ago, get from the phase of thinking about bariatic surgery to actually having it done in the US. I've had three patients go to Mexico and have it done. I will withhold judgment, because I haven't been to those centers, I don't know what those doctors and teams are like, but I do know the overall out of pocket cost for patients is about 5 grand, which is so much cheaper than it is in the US that it doesn't bear comparison.
Just-after-surgery risks include blood clots that can go to the lungs or the heart. There is always a risk of wound infection, which can be devastating. If a prolonged hospital stay is required, pneumonia is a significant risk.
Any time you have intra-abdominal surgery, your body develops scar tissue. Places where scar tissue fuses different structures together are called adhesions. Having a re-operation after that is more risky because of those adhesions. You are also at higher risk for intestinal obstruction, because your intestines can hang up on adhesion and twist so that they cut off their own blood supply. This is a surgical emergency. When bowel dies, it becomes leaky and lets dangerous intestinal bacteria into the otherwise sterile environment of the abdomen. That higher risk of intestinal obstruction never goes away.
People who have had bariatric surgery are also at risk for dumping syndrome. This is a condition where the small intestine becomes overly stimulated immediately after a meal, because the food is not moving smoothly through the stomach into the small intestine on the natural time scale. That stimulation leads to excessive insulin release in comparison to the amount of glucose absorbed, which can means hypoglycemia, which is life-threatening.
Rapid fat loss leads to significant amounts of excess skin. Many people who've had bariatric surgery go on to have skin removal surgery. This is actually a riskier surgery than the bariatric surgery itself, because you are tampering with the barrier between the inside of your body and the world outside it. And if it's done too early, you can end up needing your skin to stretch again, and having stretch marks in addition to the scars.
After bariatric surgery, you are also worse at absorbing good nutrients. You need lifetime monitoring for vitamin levels, including vitamin B12. If you don't have enough vitamin B12, your nerves start to die. This results in pain that starts in the feet, since the neurons running from the spinal cord to the big toes are the longest and therefore most susceptible in the body.
But perhaps the most upsetting aspect of bariatic surgery to me is that it is presented as a definitive solution.
Is it?
Not for 20-25% of people who have bariatic surgery, who struggle with significant weight regain.
So if the most extreme intervention we have--literally surgically altering your gut--isn't enough to make weight loss permanent, how is anything else going to do it?
You can be skinny. For a little while. But attempts to lose large amounts of weight, including surgically, have high failure rates. The 75% success rate for bariatic surgery is significantly higher than for any other method currently widely available, but the risks are also significantly higher. I don't think it's worthwhile for most patients, especially given how many patients are lied to by their doctors about how much their weight is likely contributing to their health problems. Most of my patients focus on their weight rather than activity levels, they beat themselves up about how they're not doing intense enough exercise but don't incorporate lower-impact exercises like swimming or walking, they try to eat less rather than eating a diet more rich in vegetables and fruits and lower in highly processed foods. You can do so much for yourself without ever framing it as being about weight.
And if you've done that--if you're struggling with being so fat that you can't live your life--then sure. Talk to your doctor about a referral for bariatric surgery. But don't be shocked if the results are not what you were told to expect. Don't be surprised when you find that you actively resent the people who suddenly find you tolerable, even desirable, now that you're not so fat. Don't let them sell you bariatic surgery as a no-downside cure-all, because it most emphatically is not.

Health Update
First, I want to say thank you to everyone who reached out or commented on the latest installment of Where's Mommy? to wish me good health. I am so grateful for all of you 💚
However, the health issue I'm currently dealing with isn't due to a virus or bacteria, and there's a possibility it won't get better.
A little history.
Back in 2022, there was a two week period where I felt like my blood sugar was dropping, and I was very symptomatic. There was a moment where I slumped down a wall at work because of it, and they had to dump sugar packets from the break room into my mouth to rouse me. It was a very scary time.
After those two weeks, I went to my Primary Care Physician who ordered blood tests and had me purchase a glucometer to test my blood sugar several times a day. However, during the two weeks she had me do this, I never got a reading below 70, and the same symptoms did not develop as they did prior. My blood work came back clean, and without a reading lower than 70, my PCP dismissed it and told me I was having anxiety attacks, lol. She told me to come back if the symptoms came back, and they never did.
Backing up a couple more years.
Without revealing too much of my medical history, I have a chronic illness called POTS (postural orthostatic tachycardia syndrome). It's a dysautonomia or a dysfunction of the autonomic nervous system, the system that controls all of the automatic functions of the body. It was caused by my battle with Lyme Disease in 2010 and is currently incurable. I was diagnosed with POTS in 2018, after being told for 6 years that I had anxiety, lol.
POTS is not a very well-known illness, but it's getting more attention these days. It garners a host of different symptoms, including tachycardia, chronic fatigue, brain fog, orthostatic intolerance, migraines, gut issues, syncope, dehydration, blood pooling, etc. Everyone's POTS presents differently, and most people with a POTS diagnosis live on disability. I made the choice not to.
Fast forward to 2024.
Fast forward again to this past Wednesday.
Well, I finally was able to get a POTS specialist in my state this year. A huge win! When I had my initial consultation, I had mentioned the low blood sugar episodes in 2022 and asked if it could be related to POTS. The doctor told me that they don't see POTS patients having low blood sugar issues, but we're concerned enough to refer me to an endocrinologist. Another big win!
I had my consultation with the endocrinologist, and he ordered more blood tests, some of the same tests as before, and some different (y'all, they took like 20 vials from me). He also gave me a CGM (continuous glucose monitor) to wear for 14 days so they can track my highs and lows to see if they can catch anything.
Well, the next night, my blood sugar dropped below 70, 20 times, and 55, 9 times. Which means I was woken up 9 times throughout the night. I got only about 2 hours of sleep, and still had to go to work the next morning. But, once again, it went back up by itself without any intervention from me.
Y'all, it caught A LOT in just the first day, actually night. My blood sugar dropped below 70, 11 times, and below 55, 4 times while I was sleeping. Now, because anything below 55 is considered critical and could be fatal, there is an alarm that cannot be overridden and will sound. It sounds like a smoke alarm. So, I was awoken 4 times.
The odd thing is that my blood sugar dropped, then went back up on its own. I didn't eat or drink anything. Blood sugar doesn't really do that, so I thought it was odd. This also begs the question: If I'm asymptomatic at 53, then what level was I at in 2022 when I had symptoms? Honestly, I don't want to know.

All of the red is considered low blood sugar, below 70, and anything close to that 50 line is considered critical low blood sugar. And again, I did not eat anything during the night.
There are four major anomalies with my low blood sugar occurences:
Most cases of hypoglycemia are seen in diabetics, I am not diabetic
Most cases of hypoglycemia seen in non-diabetics are sporadic, mine are consistent
Hypoglycemia is normally corrected by consuming sugar, mine auto-corrects
When blood sugar drops, it creates symptoms, I do not get symptoms
There are only a handful of things that can cause hypoglycemia in a non-diabetic and even less consistently at night time. The doctor has already ruled out insulinoma (insulin producing tumors in the pancreas), so that leaves even less, and also the good old "we don't know what's wrong with you".
I'm not going to lie. This whole thing terrifies me. There's no telling how long my blood sugar has been doing this, and it only takes one dip below 50 for me to slip into a coma and die in my sleep. Luckily, my blood sugar does this crazy autocorrect thing, and I haven't died yet! Humor makes this easier.
Right now, I'm emotionally, mentally, and physically exhausted. Adding this on top of my already difficult life with POTS has been hard to cope with, and I'm crying a lot.
Hopefully, I'll get results soon, and my endocrinologist can figure out why this is happening and how to manage/fix it if it can be managed/fixed. Maybe I've got a completely new illness, and you'll find me in a medical journal! Wouldn't that be something.
Anyway, thanks for the continued support. I have a lot of IRL support from friends and family, but while I go through this process, I may seem distant, my posting might be sporadic, I may not keep my fic posting schedule, etc. And when I have an update, I promise to let y'all know!

Outside The Office Chapter Twenty Five
TW: Reader faces consequences of her self injurious actions.
I tiptoed over the metal pathway that guided people over Vox’s massive shark tanks. Part of me knew it was fully  enclosed and totally safe. But the other part of me still feared falling in- the ending for that wouldn’t be pretty. Around me, screens lined the walls leading to his primary office. I stepped into the circular room.Only one chair sat smack in the middle- his. 
“Welcome, welcome, reader,” Vox said as he turned his chair around. He clasped his hands together. “So glad you could make it.”
That unsettled feeling rushed through my gut. Vox knew, he had to. Otherwise what was this? 
“I have something interesting to show you.” Vox said calmly as he stood up. “Com’here. Take a seat.” 
I took a step back and he shook his head. I heard the metal of the door slide shut behind me. 
Fuck.
“I mean, you could try to back out ,kid, but then I would have to go straight to Lucifer and Valentino and honestly, what would that get you?” Vox continued in a rather unbothered voice. 
He had a point. Hesitantly, I took the proffered seat, half expecting to be strapped down like a horror movie. That didn’t happen, thankfully. Vox hit a button and all the screens but the biggest went black. Data flashed in front of me. Pulse rate. Respiration rate. Blood oxygen Level.  Blood pressure. EKG chart. Temperature. Blood glucose level. Hydration level. Weight. A mix of numbers.��
“See, what’s interesting about these numbers,” Vox said as his hands pressed heavily against my shoulder. “Is that they’re drastically different from just a few days ago.” 
Another screen popped up. Higher numbers in some areas. Lower in the other. 
I swallowed as I looked at the vital signs on the two screens. I decided at that moment that playing dumb was my best option.  
“So? What am I looking at?” I asked. Even to me, my voice sounded shaky. 
Vox had the nerve to chuckle. “Oh, princess, you know exactly what you’re looking at. But if we’re going to play that game, I have another video to show you. Several actually.” 
Double fuck.  
I watched the flash. One showed myself at the gym that morning, knelt over the garbage can. Another showed me tossing away breakfast. Another of me played quickly as I went through that morning’s routine, going to the gym, coming home, showering, Vox and I in the kitchen, talking. Me, scribbling in my notebook the day before, outlining my plan. 
“You know what’s even stranger? I specifically asked you if you had eaten lunch and yet…I can’t find you doing so anywhere in the video. So either my technology is faulty, or you lied to me.” He leaned down closer, “And I promise you, my cameras aren't the issue.” 
God fucking damn it. 
Vox dropped the notebook into my lap. “Open in. Tell me where any why you think this little plan of yours is a good one. Come on, reader, I want to know.” 
His voice was dark. Dangerous. Terrifying. This wasn’t my Vox, this was…Vox as an overlord. I swallowed. “Vox, this is the plan verbatim for new recruits, I used to do it to…”
“I know all that.” He waved his hand dismissively. “That data has been in my harddrive for years, that isn’t what I’m asking. I want to know why YOU thought it was a good idea to put YOURSELF through it, knowing that your powers are growing, knowing there is a difference- especially for you- between what you could do in heaven and hell, and probably most importantly, why you KNEW it was so terrible that you felt the need to hide it and yet, still did it anyway.”
I didn’t think I could feel any smaller. “I wanted to be stronger. I just, I knew you guys hate the way angels do things so I thought I just…”
“Would do it anyway?” He rubbed his forehead in frustration. “Kid, we despise how they act because what’s normal to you is incredibly cruel in actuality. You’re honestly lucky I paid attention because if you let this go longer, Valentino’s anger would be the least of your worries. Look how little it took to fuck up your vitals. A few days of stress induced vomiting and here we are.” His voice softened. “The question is now, how are we going to handle this? Because I won’t let you keep doing this to yourself.” 
I looked at him in confusion. “Why are you asking me that? Aren't you just going to tell Valentino and Lucifer and let them handle it?”
He shrugged, “I mean, that’s an option you could choose. But I don’t think it needs to go that far, at least, not right now. So I’ll make a deal with you.”
I hesitated. “What kind of deal?” 
He squeezed my shoulders. “The kind that doesn’t harm you. I need a few things if this is going to stay between us. I need you to eat. I need those vitals to improve. And I need to never, ever see you push yourself that hard ever again. You can work out in moderation, and eat healthy but you cannot break yourself time and time again. The next time it happens, I won’t be the only one who knows. Understood?” 
I nodded in defeat. The crushing feeling of self loathing settled over me like a cloud of darkness. But another part of me, a very, very small part, was glad that I had been seen.
“Oh, and if your vitals don’t improve the deal is off. Lucifer will know and Valentino will know, and I’ll take you down to the hospital myself. And trust me, you won’t like what they do to you there.” His eye swirled for half a second and then he blinked. His tone lightened and he clasped one hand on my shoulder. “Let’s start with lunch, shall we?” 
I glanced back at him in both disbelief and fear. “Wait a second. Let me get this straight- you’re keeping this between us? You’re not tattling on me?”
Vox shrugged. “I see no reason to make this a bigger deal than it needs to be. I sincerely don’t think any long term harm will come to you because I caught you so early. You’re not fighting me on it, you acknowledged the issue. You have to trust someone in here, sweetheart. And for whatever reason, you don’t feel yet that you can trust your boyfriend's reaction. So at the very least, trust mine.” He offered his hand and he pulled me to my feet. “Now come on, let’s get you back on the right track. I know a place that has the most amazing salads. And I have one of my employees building an appropriate, data driven workout plan that should get you where you want to be in a much healthier way.” 
As I followed him out the door I wondered if this was what it felt like to have an older brother, or maybe a parent who cared enough to notice when things weren't quite right. And even moreso, who cared enough to listen and call me out on my bullshit when I backtracked. Whereas my father would have been thrilled I pushed myself that hard, Vox clearly wasn’t. But instead of the harsh punishments, he offered a compromise. He offered safety, and trust in a drastically different way than Valentino did. 
Lunch alone with Vox further proved my point. True to his word, the restaurant he took me to did have the best salads, and after eating a sufficient amount, he walked me back to the apartment. Before he left, he handed me a thin file- the workout plan he promised. 
“No one will be angry you’re trying to get stronger. But no one will stand for you to hurt yourself either,” he warned before embracing me in a hug. “It gets better, kid. You don’t have to be that perfect little angel ever again- you can let that go.” 
I didn’t know how to respond. 
Alone in the apartment, his words echoing in my mind, there  was one lingering thing still bothering me. Valentino. Vox was right, I didn’t feel like I could tell him when this all began. Maybe part of me feared his punishment. No, that wasn’t what I was afraid of. I feared his rejection. 
But the Vee’s were close. All it would take was one slip of a video from Vox to Valentino, or one accidental mention and all of this would come spilling out. Would his reaction be worse if he found I kept it from him longer than I already had? I decided I wouldn’t take that chance. 
I stepped back into the elevator and hit the button to Valentino’s studio. I needed to do this now, while I had the nerve.

A study published recently in The American Journal of Clinical Nutrition by researchers from King's College London and the Quadram Institute looked at the effects of replacing regular wheat flour with 'cellular chickpea flour' on feelings of fullness, fullness-regulating hormones, insulin and blood sugar levels in people who ate it.
The study is the first of its kind and is based on the design of a new pulse ingredient that is now being commercialized for food industry use as PulseON by Pulseon Foods Ltd.
Eating healthy pulses including chickpeas, lentils and beans is known to help support healthy weight maintenance and decrease the risk of heart disease. A lot of the benefits seen from these foods are due to the fiber structure of the pulses themselves, with normal flour milling generally considered to reduce the beneficial effects of fiber structure.
