a couple people coincidentally reblogged my rare disease day post from [last year] so. here's how that's been going. (some of this is repeat info if you click the link)
I have mast cell activation syndrome (or MCAS), an immune disorder in which my signaling cells are way too jumpy and can trigger all kinds of symptoms (up to and including anaphylaxis) in response to seemingly harmless stimuli. this is distinct from allergies as whether or not I react to a trigger can change from day to day based on how many other triggers are present, and the chemical mediators released cover a broader spectrum than just the IgE release in a classical allergic reaction. plus oftentimes symptoms are a mix of episodic and chronic, rather than being acute episodes like allergies.
triggers can be commonly understood things like certain food ingredients or scent chemicals, but can also include things like hormone fluctuations, physical or mental exertion, and the sun. I have reacted to ginger, laundry detergent, and a natural gas furnace, for example. I also do a lot worse in summer because it is both sunny and warm, and I always have to be very careful how much energy I use up or I might trigger anaphylaxis.
it's common for people with MCAS to have some level of symptoms all their life, that go unnoticed or dismissed until some precipitating event causes a substantial downturn in their condition. for me it was mono, but other infections (especially covid) and significant stressful events have also been anecdotally reported to precipitate downturns if I recall correctly, or simply being uncontrolled enough for long enough can also snowball.
diagnostic criteria for MCAS were first proposed in 2010, so the true rarity is still very up in the air. (personally I believe a lot of common wastebin diagnoses (ibd, fibro, cfs/me, etc) should be scrutinized very carefully as potential "specialty silos" of MCAS, especially given how often they tend to travel together.)
firstline treatments for MCAS include cheap medications available over the counter like loratadine, aspirin, and famotidine. my condition has become severe enough that I'm currently on third-line treatment (omalizumab injections), which does finally seem to be getting me somewhere. (this is, of course, in addition to still taking all of the first- and second-line treatments. I take 24 units of 13 medications in a given day, it's a lot to keep track of. to the point where I'm not actually sure I've counted it correctly :v then there's my monthly injections and my emergency medications on top of that.)
(those injections btw? can cost thousands of dollars even with insurance. thankfully I'm able to use the company's assistance program, so I don't pay anything out of pocket. for that one.)
being sick is never fun, but if you have a disease perceived as rare it introduces a ton of new complications. (which get even more complicated when the disease is legit super rare!) it took me a long time to figure out what was wrong with me, and I had to do it almost entirely by myself, because doctors simply don't have the knowledge. they're not taught to look for it and there's no simple reliable test. but by yammering about it, it might make someone else's slog a tiny bit shorter <3

Abstract The COVID-19 pandemic highlighted gaps in infection control knowledge and practice across health settings nationwide. The Centers for Disease Control and Prevention, with funding through the American Rescue Plan, developed Project Firstline. Project Firstline is a national collaborative aiming to reach all aspects of the health care frontline. The American Medical Association recruited eight physicians and one medical student to join their director of infectious diseases to develop educational programs targeting knowledge gaps. They have identified 5 critical areas requiring national attention

not to be That Feminist but. You cannot read your way out of talking to other women. Feminism is a very broad set of actions and beliefs, and it manifests in many ways, including ofc artistic and literary and academic creations, which are obvi v v important to the cause, but at its core feminism is a social movement and as such we can only grown within it if we acknowledge the inherent social aspects of it. You can and probably should read feminist essays and books and watch feminist seminars and movies and support feminist manifestations but you can only grow as a feminist in a healthy and productive way if you talk to other women, all kinda of women, not only other radical feminists or even other feminists at all, you gotta talk and listen to women of all kinds of lives and backgrounds and beliefs, even ones you disagree with or judge or are against. You don’t need to agree w them, or learn from them, or educate them, or even particularly like them. But you gotta respect them as /women/, as women who are in the same sexual class as you are, because when we're talking about feminism we're talking about women's lives and women's liberation we're talking about women. You can call it what you want, outreach, firstline work, base work, sorority, conscientization, whatever, but we need to understand that connection to other women is at heart of the movements and beliefs we choose to be in.

Also not sure what all boxes to check on the complaint form. Quality of care for sure. Am seriously considering mental or physical impairment too, because I'm concerned that the fact that he's still bringing up the pericarditis that I was cleared of simply because it was mentioned in the cardiologist note as the reason I was referred might indicate confusion. I'm also considering inappropriate prescribing but less sure on that one. He prescribed me Levofloxacin for an extremely mild UTI where I had almost no symptoms at all (and it was my first UTI ever, not a recurrent one), which is an antibiotic with a black box warning because of an increased risk of tendon rupture and tendinitis. I'm not in the higher risk group (over 60), but everything I read (on actual legit medical sites, not FB) basically said it was indicated in complex UTIs when other treatment courses had failed, and shouldn't really be used as a firstline treatment for a mild case. I called the clinic to try and get the prescription changed, but the doc was already gone for the weekend, so I decided to go into an urgent care clinic and explain the situation and see if the doc there would call in a different antibiotic for me. When I mentioned which one had been prescribed, his eyes literally widened and he yelled, 'No!' and said that you absolutely should not use that as a firstline treatment in such a mild case, specifically because of the risk of tendon rupture and that it's not worth the risk when there are several other antibiotics that are generally very well-tolerated and have a much lower risk of serious side effects. That doc had a really strong reaction to it being prescribed in this case, but it IS indicated in the treatment of UTIs, so I don't know if it's technically inappropriate. He's also promoting the use of Ivermectin for Covid treatment and prevention, although he hasn't personally prescribed it to me (I am wondering if he would if I had gone to him for Covid treatment, though). And doctors in this state HAVE been disciplined for that; he spent part of my last appointment venting about a doc a couple of cities away being 'persecuted and prosecuted' for prescribing Ivermectin for Covid and also has told me multiple times that he takes it several times a week himself. So at the very least he's legitimizing using it for an off-label use that has already been pretty robustly proven to be ineffective.
Misdiagnosis of condition would fit because at the very least I don't have chronic fatigue syndrome since 'excessive fatigue' is kind of a required symptom for that to be suspected, and I don't have that anymore after several weeks on iron supplements. Which he would know if he didn't ignore everything I say if it doesn't fit his anti-vaxx theories, even when I'm answering a direct question he asked. But technically he hasn't formally diagnosed me with that, he's just told me he's pretty sure I have that as well as POTS, but we'll see what neurology says.
