Just a reminder: Not every OSDD-1 system fits under 1a or 1b. Yes those labels are helpful, however, not everyone fits under that, nor should we force them. All that OSDD-1 means, is that it’s nearly DID, but doesn’t fit the entire criteria. Saying anyone who doesn’t fit in one of two narrow labels is faking, is silly.

I'm pro-endo, but I do think that the term "sysmed" is transphobic. I know that anti-endos who force a medicalized view of plurality are harmful, but they will never compare to the harm that transmeds did, and will never even come close to the harm caused by medicalizing transness. "Sysmeds" aren't the result of a vaguely homophobic ideology that strips away the bodily autonomy of gay trans people due to our supposed "fetish" for "real" gay people. Sysmeds aren't what forces people to jump through so many extra hoops for necessary medical care like transmeds are. Sysmeds aren't what gives me a strong fear of the irl gay and/or trans community. As bad as some anti-endo might be, please don't call them a sysmed.
Personally, I feel a lot of the differences you highlight are more due to the scale of the communities.
I've interacted with and seen so many pro-endo and mixed origin DID systems who are scared to interact with online DID communities. Even posting in DID tags here on Tumblr is something many pro-endos with DID are terrified too.
And the only reason this doesn't transfer to IRL plural communities as much... is because there really aren't IRL plural communities yet.
Systems are fakeclaimed so much that most systems don't feel safe coming out IRL, and organization is difficult. But I believe that's going to change which only increases their potential for harming the community. But even putting aside the potential future harm, I feel you're understating the harm already caused to the plural and DID communities.
But to discuss this, it's important to acknowledge that there is a larger divide here than just being pro and anti-endo.
The hatred for groups like The Plural Association isn't simply born of endophobia, but of a larger medicalist philosophy. For this reason, the word "sysmed" is more than just about the comparison. For a few common philosophies included in that framework:
Parts Language is treated as objective fact, and systems who don't use parts language are shamed for it. For the few system medicalists who believe endogenic plurality might exist, this is used as a dividing line. "Endogenic systems are people but CDD systems are part, so therefore we're different." The problem with this is that it erases the many systems who identify as people. Remember: CDD systems identified as different people FIRST, and were later labeled as only parts by psychiatrists.
The biggest threat to CDD systems is people pretending to be systems. The myth that there are a ton of people faking out there faking DID, and therefore making it harder to be diagnosed, is everywhere in these spaces.
The Shame Criterion. Some questionable studies were conducted into diagnosed systems that were deemed to be "imitative DID." The claim is that one difference between "imitative DID" and "genuine" DID is that people with genuine DID are ashamed of their symptoms. These have been passed around in system medicalist spaces, and raised suspicion towards any systems who are too proud or overt. (This is harmful to any attempt at plural rights because it immediately casts doubt on any systems who aren't ashamed enough. It also ignores that people who are isolated without a community of people with similar experiences will be more ashamed than those who have people with similar experiences.)
Dismissing Mixed Origin systems as endogenic. This, while tied to the endogenic question, isn't about it directly. Individuals that believe in endogenic plurality and think it should be kept separate are sure to keep mixed origin systems on the "endogenic" side. They would prefer people with spiritual beliefs about their systemhood, even if diagnosed with DID, to not be allowed into DID spaces. This is a direct threat to the ability of these systems to seek medical care for their disorders.
For all of these reasons and more, the term "sysmed" is about a larger philosophical divide between them and the inclusive plural community than just whether endogenic systems exist or not.
More than just harassment and bullying online, if doctors are aware of and listening to medicalist rhetoric, this could further harm CDD systems who seek diagnosis, reinforcing the myth that there's a DID fad and systems seeking help are jumping on a trend.
The McLean Hospital video that fakeclaimed diagnosed DID systems on TikTok for not being ashamed enough of their systems was PROMOTED here on Tumblr and reblogged by system medicalists, as well as posted to sysmed hubs like r/systemscringe.
This was a video that was deemed so harmful by the institution that posted it that they took it down the next day, but that hasn't stopped others from using it and spreading it as an example of a wave of DID fakers.
This directly resulted in DID TikTokers facing harassment.
And it could make doctors even more hesitant to diagnose people with DID because of the perceived influx of fakers.
It's impossible to measure the impact of system medicalists spreading the video, or the impact of their other rhetoric, but the fact is that their beliefs and the content they spread further stigmatizes all systems who don't conform perfectly to what's perceived as the medical model, and supports ideas that make it more difficult for many systems who would benefit from medical treatment to get it.

I found this comment on Reddit & I think this is about all that needs to be said regarding sysmeds, transphobes, and any randos who feel the need to chime in on the topic of YOUR identity:

the demedicalization of disorders is going to kill people.
Anon, tell me, what made you jump to such an extreme conclusion and feel the need to drop the unlikely into the inbox of a person just trying to exist peacefully in their own space?
What is making you act out in these ways, to someone who has explained their points and beliefs already?
The de-medicalization of things such as personality based and system based disorders won't kill people. Same with autism or adhd.
Yes, there are limits to what can and cannot be de-medicalized, that might have severe problems if it was tried. Yeah, duh. But how about you go back and re-read our posts about us and how we live life.
Also, it's clear you ran out of argument points because you resorted to a very extreme situation to try and win the battle you're losing.

Boy howdy is it fun(/s) frantically checking the blog/pinned post of any url w 'system' or the like in it to make sure the op of any post/rb Im seeing isn't a fucking sysmed.
Also, time and time again someone engaging in the reduction of plurality to only DID(ex: calls being plural inherently 'having DID', calls plurality as a whole DID, calls not-exclusively-DID plural experiences DID, etc..)ends up being a dogwhistle for sysmeds....
I don't blame ppl who accidentally rb/like/etc. a post from a sysmed/anti endo but the main reason I send in asks abt it is if I know the blog/user that happened to rb it is pro endogenic and also bc I don't want any content made by sysmeds(even non syscourse content!) on our dash if the account is one we follow.

plurality is not defined by switching.
plurality is not defined by headcount.
plurality is not defined by an inner world.
plurality is not defined by distinct headmates.
plurality is not defined by communication.
plurality is not defined by getting along internally.
plurality is not defined by an origin.
plurality is defined by multiple consciousnesses/identities in one body. if you say you’re plural, we and many, many others will always believe you. no two plurals are the same, and no one can tell you what you are or aren’t 💞

Reminder: the "sysmed is transphobic" talking point was invented as a means of revictimization
Transgender Endogenic/mixed origin systems: *exists*
Sysmeds: being a system is a mental disorder. You can't be a system without a disorder and anyone who says otherwise is just doing it for attention. Everybody, bully the ableists!
Transgender Endogenic/mixed origin systems: *refers to them as sysmeds, comparing identical talking points as well as the hate and harassment they've received from sysmeds to what they suffered from transmeds.*
Sysmeds: Now the nasty transgender endos are stealing terms from the trans community! Everybody, bully the transphobes!
Abusers love pretending to be victims. Transmeds employ the same manipulation tactics, alsi accusing anyone who disagrees with them of being transphobic.
Another thing I see is claiming sysmed is a slur. This particular tactic is borrowed from TERFs who claim the word TERF is a slur and demand to be referred to as "gender critical" instead.
To anyone who thinks there's a notable difference or that the comparison isn't valid:
In my time dealing with sysmeds, I've been harassed and fakeclaimed for my identity. I've been sent more suibait than I can count and dealt with attempts to get me banned by mass false reporting. I've been maliciously misgendered numerous time as a means to hurt me, the way transmeds have done to non-dysphoric transgender people, including by the admin of a major anti-endo server called The Survivor's Network.
Sysmeds repeatedly employ the talking points of transmeds, the tactics of transmeds and the abusive behavior of transmeds.
Then when compared to transmeds, they call the victims (who are often trans or otherwise GNC) who call them out transphobic for making the comparison.
Sysmeds don't call their victims transphobic because their victims are transphobic.
Sysmeds call their victims transphobic to silence their victims.

Being SYSMED harms TRAUMAGENIC SYSTEMS!
I am an OSDD-1b system who used to be a sysmed/traumascum, and it harmed me. I encourage other systems to avoid being sysmed.
Sysmeds believe all systems suffer from dissociation, should fuse into one, and that the host is most valuable. They often see being a singlet as superior to having alters, and may believe being professionally diagnosed is necessary. They think systemhood can only start from trauma at a very young age- yet if you're 15 or younger and know that you're a system your experience should be doubted. Being a sysmed can lead to hurting your system by abusing alters and seeing them as burdens or less valuable then you as a host.
When I became pro-endo(again), I learned to respect and appreciate my system, while as sysmed- I abused my alters, especially persecutors, because "the host is most important" mentality sysmeds share.
Embrace your DID/OSDD system, it's a part of you that won't go away. Being pro-endo can help shift your mindset from seeing your system as a burden to a positive aspect of who you are as well as gain their support and love. Accept support from those who understand, even if their experience doesn't perfectly match yours.
Endogenic systems deserve respect even if you think they are "confused" or "un-knowingly traumagenic" so let go of any hostility towards them. Trust that their experiences are genuine because frankly- its their right to know what experiences they had, not yours.

"Plural" is inclusive of ALL SYSTEM origins. If you don't like "endo and endo supporters :((((" using the term "plural", that's a you problem. It doesn't matter if you don't believe in non-traumagenic plurality-- plurality, as a term and a community, accepts non-traumagenic systems.
Stay mad about it, we don't fucking care.
