Haya's Story

Since the genocide began, 18 year old Haya Orouq (pictured on the right) has been working ceaselessly to get her family evacuated to safety.

Her mother, Amal (left), has lupus and is a kidney failure patient. The hospital she got her treatments at was just bombed and she no longer has access to the life saving medicine she needs.

Her younger sister, Lama (pictured with her mother Amal above), just turned 11 during the genocide. She is so young and has lost everything -- her safety, her school, her friends, and her home. Now she could lose her mother as well. 💔

Her father, Saleh (pictured with Haya above) and older brother Ehab, who is only 21, lost everything as well. They have been forced to move from place to place since their home was destroyed, and have had many close calls and terrifying sleepless nights with the sound of bombing all around them.

Sorry for not sending last week's issue as I got covid again :/ I passed it, so here's the best things that happened last week :)

1. The IRS plans to crack down on 1,600 millionaires to collect millions of dollars in back taxes

The IRS announced on Friday it is launching an effort to aggressively pursue 1,600 millionaires and 75 large business partnerships that owe hundreds of millions of dollars in past due taxes. The newly announced tax collection effort will begin as soon as October. “We have more hiring to do,” Werfel said. “It’s going to be a very busy fall for us.”

2. The NGO African Parks announced it would purchase the world’s largest population of privately owned white rhinos

Hysteria isn't a fucking thing

ok fun fact: I'm rapidly becoming a cult favorite doctor among our local privileged elderly white ladies, which I have mixed feelings about, but the #1 reason is that I just don't leap to "anxiety" as an explanation for symptoms unless the patient tells me "I am anxious, and then I feel these symptoms, and when I am not anxious, I don't feel these symptoms."

The sheer number of women I've seen who've been told for years to decades that the only thing wrong with them is anxiety is fucking staggering, in this Year Of Our Lord 2023, and I just keep digging. We checked a basic lab panel, sure. CBC. No anemia. CMP. Kidneys are fine. (Electrolytes are basically always going to be fine if someone is well enough to walk into my office under their own power to talk to me. Exception is mild chronic hyponatremia.) And we check thyroid. TSH and free T4. We check blood sugar. A1c, if the fasting is a little weird. Fasting insulin, if I'm still suspicious. We check cortisol. Inflammatory markers--ESR and CRP.

And eventually, if the symptoms support it, or right away, depending on my level of suspicion, we check rheumatological labs for abnormal autoimmune function. Anti-nuclear antibody. Rheumatoid factor. There's at least a dozen you can check, and which ones you should check is always a matter of debate and also of expertise that I 100% lack. We are out in the sticks. There are no "local" rheumatologists for me to send people to.

But a couple of weeks ago I found a woman--she has bipolar disorder and has been told for decades that's all that's wrong with her--who has an anti-centromere antibody titer that's fucking through the roof. I found an anxious 19-year-old with an ANA of 1:1380. And yesterday I found out why a sweet elderly woman I've seen for a year or two now started feeling crappy months ago: her rheumatoid factor is over 90.

Rheumatological disorders are always difficult. Our understanding of them varies from "pretty good, actually, and here are useful treatments" to "Well I Guess That Exists." Labs aren't always a slam-dunk and even labs plus symptoms can give you misleading impressions. Your immune system can decide that virtually any short chunk of protein is an enemy, and the problem with that is that your body is made up of many, many, many short chunks of proteins, so the odds that you'll develop some kind of antibody against yourself just keeps going up over your lifetime. Immune disorders tend to travel in packs; there's a clear genetic element to it, so the more first-degree relatives (parent, sibling, child) you have with any kind of autoimmune disorder (including Type 1 diabetes), the higher your risk of any kind of autoimmune disorder is, and if you already have one autoimmune disorder, you're at higher risk for developing another one.

But I think it's precisely because they're difficult that a lot of mainstream primary care prefers to pretend they don't exist, rather than try to sift through the utter fucking mess that is Mixed Connective Tissue Disorders, a title that has fallen out of favor since I learned it in my third year of med school. And women are at higher risk for autoimmune disorders than men. And older women are at higher risk than younger women.

So if I, as a family doc, just keep digging, just keep poking at the tangled knot of symptoms, there's a decent chance I will uncover something interesting. Hopefully something treatable. Sometimes we have nothing to treat with, and I just get to offer someone more understanding of their disorder, which feels pretty paltry but is better than the casual dismissal of "You're just anxious."

Never, ever, ever take anxiety as a diagnosis for a symptom other than anxiety. Not even as a rule-out. Keep those symptoms as an open question mark on the patient. Don't say "anxiety" just so you can close the door. And damn sure don't do it to women.

I'm actively working on learning more so I can be more helpful, in our Rheum-less community, so if you have good lectures or books, please drop me a lead.

Monoclonal Antibodies

So I listened to a talk from a drug rep the other day who came into the clinic to promote Cytopoint, the anti-allergy injection for dogs.

It’s a great drug, overall. Super useful for many, literally life changing for a few, and now a mainstay of the allergy treatments we offer. But I had to laugh at something the rep said:

“It’s not really a drug. It’s a bio-mimicry molecule! It’s replicating normal molecules that would be found in the body and just providing more of them!”

And I’m sitting here thinking: that is still totally a drug. A drug is any substance which when given to a living organism produces a physiological effect. 10mg of lokivetmab is most definitely a drug.

But if you are going to argue that Cytopoint is not a drug because it mimics a natural molecule found in the body, then by that logic neither are insulin, thyroxine, cortisone or just about every single hormone.

For goodness sake. I have to wonder what planet marketers live on some days.

We also had a bit of a debate because we’re widely told that Cytopoint shouldn’t have any side effects. Partly this is because monoclonal antibodies aren’t well known in veterinary medicine and they haven’t been around that long.

They’re certainly much safer than the equivalent drugs in other classes, but they’re not ‘side effect free’. They’re a protein, so it’s theoretically possible to be allergic to it. Like the Kesimpta I take it seems likely that some dogs will be ‘off colour’ for a day or so. The most common side effect is being quiet the next say, so it is pretty mild for a typical dog and still worth it. But it’s not zero.

Having a headache for a day is much milder than having acute kidney failure, but I wonder if we’ll get more reports in the future of extremely sporadic weird side effects as we gain more experience with this class of medication.

Medical advancements are wonderful and exciting. But don’t let marketers do all the talking!

And remember to report side effects to the manufacturer.

Prayers and good thoughts for our friend’s friend Raisa. She just had her 5th round of chemo for breast cancer, and the side effects are really hitting her hard, and she is struggling. She has 1 round of chemo left, but the doctors told her the last 3 treatments would be the hardest, and they have been.

Prayers and good thoughts, Emergency prayer request: a two-year-old girl was accidentally run over by her mom. She's in surgery right now and in bad shape. Please pray that God's will be done.

Prayers and good thoughts for our friend’s niece. She just had a healthy baby girl, but she is having some medical issues still.

Prayers and good thoughts for our friend who has had foot surgery and is in my pain. Also has faced trials of emotional, financial stress. Also her husband is not well and having problems with his kidneys.

She will have to live with a colostomy bag permanently. The doctors are waiting for her to be strong enough to submit her to chemo and immunotherapy to reduce the liver tumors. She is still young (78 years old) and also because she spent her life taking care of her children and their father who now has Alzheimer's

Prayers and good thoughts for our friend who is suffering from ringing in her ear. It is constant and she can’t sleep. Neither her doctor or dentist can find the cause.

Prayers and good thoughts for our friend’s husband who is in the hospital with a blockage, he’s been there for 2 days, and his wife is hoping he’ll come home on Monday. She will has had her surgery and is in terrible pain. Her husband is losing his job at the end of June, and her heart doctor said to be prepared for heart surgery once she heals from her foot surgery. She has a leaky aorta valve.

Prayers and good thought for our friend’s brother who was just diagnosed with terminal stomach cancer, secondaries in his liver and lungs. We pray for our friend and her sister to have their trip go smoothly as they plan their trip to be with their beloved brother. Also, we pray that their brother will be pain free and that he will be at peace.

i've recently been doing a lot of research into intersex conditions ((to be better knowledgable and supportive of a good friend who is intersex)) and i heard about turner syndrome. and the more research i do, the more i think i might have it ((particularly Mosaic form TS))? i have many of the most common symptoms ((very short stature ((4'9 at 15)), small lower jaw, shield chest, very mild lymphadema, low hairline down the back of the neck, very irregular periods ((i have about 3 or 4 periods a year, ever since starting my period)), high/arched palate, lots of moles, and probably some smaller stuff i forgot abt)) would it be worth it to bring it up to my doctor and get tested? expecially if i don't want to receive feminizing hrt. i don't have any other health issues really, so would that just be stealing testing and/or resources from those that really need it?

usual disclaimer that we can't give medical advice or diagnose.

It makes sense why you would be curious about turners/mosaic TS after looking into the symptoms and seeing that they seem to line up with some of your experiences. And I think that Turner's is one of the intersex variations that it can be important that if you're suspecting you might have it, to actually try to seek a diagnosis if possible.

This is because there are a lot of other health issues that are associated with Turners that can develop at any time in your life, including heart problems, kidney problems, osteoporosis, scoliosis, and sometimes diabetes. Even if you have Turner's, that wouldn't automatically thing that you would also have these other issues, but it would mean that you would be at higher risk and that you might want to undergo different types of monitoring throughout your life. I don't want to scare you unnecessarily because I know plenty of people with Turner's who don't have other health complications, but it is a significant enough associated risk that I think it can really be worth trying to find out that information about yourself if you have other reasons that you're suspecting Turner's.

You wouldn't be stealing testing or resources away from other people--there isn't a shortage of testing in that way. The main obstacles to getting testing and care for intersex people is more due to discrimination and uneducated doctors, rather than a resource shortage. In terms of getting forced on estrogen, you always have the right to deny treatment that you don't want, and to explore alternative options if there's a reason you need some type of hormones for your bone health.

Of course, it is always, always up to you whether you want to try to seek a diagnosis or not, and you'll be the expert on whether this feels like the right thing to do at this point in your life or not. I just wanted to provide some reassurance that you wouldn't be overstepping, and share some of the reasons why Turner's is an intersex variation where a formal diagnosis might be more important.

🌼be conscious of what's going on your skin. the skin layers can be over-cleansed, imbalanced, and aggravated when the skin microbiome and pH are disrupted. if you wipe out the beneficial bacterial populations with harshly formulated products, you may find that you break out (i.e. harmful bacterial populations begin to thrive). use gentle, simple skin formulas such as castile soaps, natural oils & butters (i recommend jojoba oil for mostly everyone), natural soap bars, and clays. most commercial skin care products disturb the skin cells (introduce too much foreign material to the body, unsuitable pH ranges, imbalance the microbiome, etc). they sell because it's convention to buy them, the brands are well-known, and their sales pitches are enticing. but really, no cream or serum will ever be the magic potion your skin was asking for. it will only be a band-aid, at best. truly healthy skin comes from diet, and the products are just for any other necessary maintenance. if you get a pimple, you should look first to why the pimple emerged, not what treatment will remove it.

🌼keep your lymph flowing. simple ways to do this are through good lifestyle habits. things like doing exercise & sports you find fun, breathwork, dry brushing, eating lots of fruit, avoiding wearing bras & other tight clothing items, lymphatic massage/gua sha can all be helpful.

🌼focus on hydration via fresh fruits and veggies. the water within plant foods is more useable to your cells, and your skin will cease being overly oily or dry overtime (especially if you reduce/eliminate your salt & oil intake). it will also help to encourage lymphatic flow, so you can see problems like blemishes, cellulite, old scars diminish after great consistency.

🌼allow your skin to breathe. this is especially good if you spend time in stuffy (poorly ventilated) indoor environments, like offices, hospitals, planes, etc. one great way is via herbal facial steaming (i recommend it 3x a week or so). make a hot infusion of an aromatic tea, cover it and allow it to cool a few minutes, and, with closed eyes, allow the pores to open and receive the herbal medicine (this is a great time to meditate, manifest, and just feel pampered). getting more fresh air, working up a sweat, and going to a sauna are also ways to allow your skin to breathe.

🌼avoid steroid prescriptions creams at all costs. they are very deleterious to health in the long term, and they can be very painful to come off of. it's not a real fix. it's not worth hurting your kidneys & adrenals for. evaluate your diet instead. do you eat dairy? choose plant-based dairy options instead (they're simple to make yourself too). do you eat a lot of fats? try oil-free cooking methods instead (opt for sautéing with a bit of water, baking, steaming, air frying, etc). avoiding eating out as much.

🌼be aware of what you're putting in your hair as well. your shampoos, conditioner, leave-ins, gels, moisturizers, oils, serums, etc all tend to touch your face too, when your hair touches your face. if they wouldn't be good enough to put on your face, don't put them in your hair. a lot of the best skin cleansers are also suitable as shampoos. a lot of oils and butters can be used for both. plant-based gels like aloe vera or flax seed are simple to DIY, and are dual-use once again. basically, this will simplify your life.

🌼understand the water that runs through your pipes. if your shower and tap water are like most people's around the world, they aren't optimal for our skin. for example, they contain added chemicals to sanitize the water but that can prevent the proper microbiomes from developing on your skin. a weak skin microbiome is prone to skin issues. so, try to use distilled water on your face (if you're not able to get a water filter, a gallon jug at a store is affordable. they're about $1.30 USD in my area).

Some of you may or may not know, I am a two time cancer survivor (pancreatic and stomach cancer) & I am currently going through severe liver failure, beginning stages of kidney failure and severe malnutrition (I am 92 pounds currently, my doctors had to sadly remove my feeding tube because it almost killed me on 3 separate occasions).

Throughout my 5 years of being ill, I’ve lost my hair many times but ever since I beat cancer the second time around (9 months ago) my hair has FINALLY for the first time in 5 years grown past my ears! Well, lastnight my hair started coming out in clumps. And I mean HANDFULS, so back to being bald I go.

There is nothing that would lift my insanely crushed spirits more right now than being able to get myself a really nice wig so I can at least feel a little pretty when I am well enough to get out, go to treatments, or even when I’m just around the house, because I truly have never felt more upset and insecure with my body in my life. Right now all of these illnesses combined, it’s wreaking absolute havoc on my body. and I also want to ask for help because I also want to go to my grandparents who lives in Poland before my surgery and a help for my upcoming surgery.

Mechanic VS Glamrocks

It's dark and damp where you are cornered, the only light you can actually see with coming from the bright overhead light in the protective cylinder. Your breathing is quick, and your pulse quickens. You thought that working with the Glamrocks would be fun. Cool, even. This is something from straight out of your nightmares.

Three of the main four have you surrounded in Parts and Service. Roxy and Chica are standing guard on either side of the door to the cylinder, but the angry green gator is the one that has you trapped here. Freddy is nowhere to be seen, but you know if the fatherly tempered bear was here, you wouldn't be subjected to this!

You wipe the sweat forming on your brow and squirm. You just wish for this, this torture to end!

"Monty, please!" Your plead and it falls on deaf ears… microphone? Whatever the animatronics use to hear with. You could almost cry. The unshed tears of strife were stinging, and threatening to fall, but you had just enough pride to keep them at bay…for now.

You couldn't keep doing this, you couldn't take it!

Monty looks at you cruelly and bellows a laugh that echoes within the metal of his casing and the cylinder itself, amused by your apparent discomfort. Chica and Roxanne are cackling at your expense, as if your pain were funny to them. It probably was, thinking about it. They wouldn't have been so amused otherwise, you think to yourself.

You're getting to a point where you are uncomfortable. You thought the Glamrocks were supposed to be nice. Kind. Cared about the feelings of others. The treatment you're getting now is nothing short of cruelty, of barbaric-ness, of insanity!

It was like they had all gone mad, and their only objective was to kill you.

Monty blasts Take Me On louder from his speaker. You bite your lip.

"This is so stupid! You always pick the songs I can't resist!" You whine, your arms elbow deep in robotic gator guts. He had fallen, again, into the marsh in Gator Golf and had filled up with dirty, disgusting water. Monty was now propped up on the huge chair in the protective cylinder and you were doing your damnedest to dry him out and save him from any lasting water damage.

"Go ahead, Mechanic, you're gonna lose anyway." Monty says cockily with a laugh.

You and the Glamrocks were playing the usual game of 'IF YOU SING OR DANCE ALONG TO THIS SONG, YOU LOSE A KIDNEY!'. Except no actual kidneys are in danger. Only your pride.

"You're getting a free show, stop complaining so much." Roxy quips after she rights herself. She begins primping, fluffing her fluffy white hair and using the glass of the cylinder like a mirror. She began effectively ignoring everyone in favor of admiring her reflection, losing her amusement in anything outside of herself.

Chica squawks loudly after a couple minutes, and flaps her arms like her real life counterpart, knocking over cans of oil, paint and air duster, startling everyone. "Ohh! Oh, Monty play this one, play this one!" She stills, sending him a link through the animatronic only network, you assume. It was the only time they go unnaturally still like that, unless they are charging or powered off. You just got done checking over Chica, so you know she had full battery and no cause to just randomly turn off at the drop of a hat.

Monty nods, wicked smile forming on his minimally moving maw. You don't think you're going to like what's to come.

Music and theatre have always been your thing. You were still a theatre kid at heart, dramatic and over the top, and you sung like a canary, especially when you were alone, down in Parts and Service by yourself, flittering around after closing. Trying not to disturb the public with your 'off tune screeching' as Moon liked to call it.

You would just belt out whatever song was wracking around in your brain. It was almost like you had a playlist in your head, cocked and loaded for you to pick and choose from. The Glamrocks, on stage and off, have only added to your growing list. You still love it though, and it's one of your favorite things about working with literal rockstars.

It's also you're least favorite.

Even if you don't really know a certain song, if it has a good enough beat, you kind of have to just vibe with it. It puts you in the mind of Freddy and his programming to jam out on DJ Music Man's dancefloor. You have joked that you have the same programming.

Did that mean you were any good at it? Absolutely not. You admit, you are mid at best. Try telling your heart that, though. You heart believed itself to be a rockstar of the same caliber you're working on.

So now, you were wiggling in place, trying to stay still and stay focused while getting the water out of Monty's deep chest, and dry out his wiring before he started short circuiting. That meant very delicate work, which meant that you had to take your time which meant that your head had to be next to his speaker, where he could blast every song you loved and you wouldn't be able to get away, lest leaving him to rust.

You flex and curl your toes and do controlled breathing to try and work through it.

You were a third of the way done by the time he finds the next song.

"Damned copyrights." Monty curses while the song loads. He, along with all the animatronics, are hooked up to the Plex's WIFI and can access any music streaming app there is. The only catch is that they may access any music but can't play any music. Labels being afraid of the Band stealing songs for their shows or something. So, Monty had to go and find some unofficial lyric video or fan-imation project so he could play the song.

You start to laugh, calming down enough to effectively work. "Yeah, you almost had me the-"

Careless Whisper plays. Loudly.

You scream. You cry. You gag. You beg.

You lose yourself to the tune of Careless Whisper. Twice.

You slam Monty's chest plate shut. It took you an hour and a half to finish a twenty minute job. You are still a little bit proud of yourself, given such harsh working conditions.

The Glamrocks go to leave, filing out one-by-one and you go about cleaning up your mess. Monty pops his head back into the cylinder. "Hey, Mechanic, you want some singin' lessons? You were a lil' flat back 'ere." You throw the shop towel in your hand at the gator. He catches it flawlessly.

"Yeah, yeah, whatever! Don't try to fall into the swamp water next time." You quip with a smile.

The towel hits you in the back of the head forcefully enough to knock you forward.

I am Kelly, My family is in danger. They are trapped in a city in Gaza called Rafah. They fled there because my grandmother was diagnosed with cancer and the only chemo treatment she could receive was there. Due to a militia that is increasingly taking over the country and bombing hospitals, all of the disabled, sick and elderly had to flea to Rafah to get medical treatment. Hundreds of thousands of Gaza people fled out of Rafah earlier this week after the militia overtook the Gaza military. My family was unable to flee because my grandmother is elderly and sick.

As of yesterday, my grandmother, disabled aunt, aunt who is sick with Malaria and my two teenage girl cousins were trapped at the top of an apartment building that has been overtaken by militia. They are helpless.

My aunts are sick and they have been injured. My two cousins are just children. Please help my family.
