Merilwen’s Teleporting Bow
Bow of Kelelbrymbelglore Son of Aofelnore
Martial weapon, ranged weapon, very rare (requires attunement)
Once per day, while holding this bow you can use an action to fire an arrow up to 600 feet away. You then instantly teleport to an unoccupied space within 5 feet of where the arrow lands.
If the arrow hits a creature, then they take 5d8 force damage and are pushed back 5 feet. You then teleport into the space where the creature previously occupied.
Furthermore, if another creature is touching you as you fire the arrow, they are also teleported to an unoccupied space within 5 feet of where the arrow lands.

Homebrew 5e magical artifact idea: Herald of the Righteous. A Great/Long/Shortsword (size depends on attuned wielder) that is sapient, with 11 Int, 13 Wis and 16 Cha, and communicates telepathically to its holder up to a range of 60ft. It grants +2 to hit and attack rolls, as well as proficiency and advantage on Acrobatics, but only when it considers the attuned person’s actions to be sufficiently “Cool” and “Radical”. It offers unsolicited advice on how to meet these ever-changing standards often.
Functioning similarly to a Dancing Blade, the Herald can float magically at the command of its holder, levitating horizontally about a foot or two above the ground with a carrying capacity limited to the attuned person’s weight (which includes whatever they’re wearing/carrying, within reason). When balancing on the Herald (requiring an Acrobatics check each turn), the user has a doubled movement speed and hands free to cast spells or use other, less cool weapons. The floating blade can also “jump” an additional 10-20 feet vertical, depending on the size of itself and the rider. While moving or jumping, the user can speak the command word “Ollie” to spin the blade beneath them, dealing 2x it’s usual damage dice to anyone in melee range.
Displaying too much “bogus” or “lame” behavior while attuned to the Herald will result in its hit/attack modifier becoming -2, and any attempts to ride it will result in the attuned wielder being either tipped or flung off of the blade, depending on the severity of their recent actions. This change in relationship can be resolved only by following its advice, in both combat and social situations, until it considers your misdeeds sufficiently atoned. The path to once again becoming “cool” in the arcane mind of the Herald after losing its favor can be a difficult journey, but ultimately the reward of regaining this sacred weapon’s trust and loyalty is worth the effort.

Homebrew Magic Item: Judgement
Based very, very strongly on Atom’s Judgement from Fallout 4’s Far Harbour DLC, which is a nuclear sledgehammer given to you by a cult of the god of radiation in appreciation for services rendered. Because that is the sort of thing that happens to you in Fallout. I shifted this a little sideways for D&D, taking inspiration from the Sickening Radiance spell, because nothing says Atom’s judgement in 5e quite like that spell. I did go for good-aligned, closer to Fallout 3’s Cult of Atom, but decided to give the weapon an idiosyncratic personality, since the Cult tends to be kooky at their absolute best.
JUDGEMENT
Weapon (warhammer), Very Rare (requires attunement)
“Cur! Thou shalt be forever wearied by my strike! Only holy forgiveness shall grant thee rest!”
The weapon known as Judgement is a warhammer forged of a strange greenish-gold metal, hafted in white and perpetually warm to the touch. Patterns like overlapping circles and rays of light are etched into the metal of the head, and seem to glimmer with a greenish radiance. It is rumoured that the weapon was forged on the orders of a mad solar, a deranged celestial who sent the weapon out into the world for some unknown, and perhaps irrational, purpose of their own.
Properties. You gain a +3 bonus to attack and damage rolls made with this magic weapon. Judgement deals an extra 2d6 radiant damage on a hit. On a critical hit with this weapon, the target suffers one level of exhaustion. This exhaustion can only be removed by a good-aligned cleric or celestial casting Greater Restoration or other similar magic.
Sentience. Judgement is a chaotic good warhammer with an Intelligence of 12, a Wisdom of 16 and a Charisma of 20. It has hearing, darkvision and telepathy out to a range of 120ft. Judgement can also speak and understand Common, Celestial, Infernal, Abyssal and Deep Speech. Its speech often sounds archaic and quite florid.
Personality. Judgement is a broadly good aligned and very chaotic weapon. While its origins are lost to legend, much of the inference of its creator’s madness comes from Judgement’s own behaviour. The warhammer is very enthusiastic in battle, joyfully determined to strike evil, but often becomes more melancholic, distracted and whimsical in less clear-cut situations. While it is broadly good-aligned, and cannot be attuned to by a creature of evil alignment, its own definitions of ‘evil’ are occasionally … idiosyncratic.
Judgement has also been known across its history to have strange, abrupt whims on occasion, and become monomaniacal in their pursuit, believing that a certain course of action is suddenly vital and must be performed with all haste and determination. The reasons behind these whims are obscure, and the weapon often cannot coherently divulge them. Perhaps they are part of some far-reaching design, or facets of its creator’s madness, or facets of its own madness. Regardless, Judgement is usually a helpful, if erratic, companion, happily willing to aid the cause of good however it or its wielder see fit, and only becomes troublesome if one of its whims has been triggered. It must carry out its chosen course of action, and can decide to break attunement and vanish from its wielder’s possession if it believes they will not aid it.

Hey! I've been dming and making homebrew content for my games for years. Most of it isn't very balanced outside of my personal (Admittedly High Power-level) games, but I wanted to start posting a few things on here in the hopes of getting some new ideas and maybe making some friends.
I hope you like it,

Nope-Sudes – Potion, very rare
Although the nope-suds may look like an ordinary bucket of soapy water, it is in fact a potent potion and tool for combat. Sometimes found in skely-ton-infested dungeons and imbued with the magic of the dark goddess they worship, the contained liquid is rumoured to grant sinister powers. But who would voluntarily drink from a mop bucket…
🔮 If you like my work, kindly consider to support me on Patreon to gain access to monster pages, tokens & artwork of over 220 quirky creatures as well as dozens of potion & item cards based on their lore.

Bane's Mark

Bane's Mark Magic Item - Unique - Attuned
Arming Sword - 1d8 +1
You may use this item to inflict a form of bane on an enemy you hit with a melee attack a number of times equal to your charisma modifier (minimum of 1) per short rest.
A target struck by a melee attack by this item must make a charisma saving throw, on a fail they must roll a d4 and subtract that number from every attack, check, and/or saving throw they make until the end of their next turn.
