Abstract
Long COVID is an often debilitating illness that occurs in at least 10% of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections. More than 200 symptoms have been identified with impacts on multiple organ systems. At least 65 million individuals worldwide are estimated to have long COVID, with cases increasing daily. Biomedical research has made substantial progress in identifying various pathophysiological changes and risk factors and in characterizing the illness; further, similarities with other viral-onset illnesses such as myalgic encephalomyelitis/chronic fatigue syndrome and postural orthostatic tachycardia syndrome have laid the groundwork for research in the field. In this Review, we explore the current literature and highlight key findings, the overlap with other conditions, the variable onset of symptoms, long COVID in children and the impact of vaccinations. Although these key findings are critical to understanding long COVID, current diagnostic and treatment options are insufficient, and clinical trials must be prioritized that address leading hypotheses. Additionally, to strengthen long COVID research, future studies must account for biases and SARS-CoV-2 testing issues, build on viral-onset research, be inclusive of marginalized populations and meaningfully engage patients throughout the research process.

No mental disorder meets the scientific definition of “disease” recognizable to pathologists: a departure from normal bodily structure and function (Szasz, 2001). This reality is clearly understood by the current and previous directors of the NIMH who acknowledge the speculative status of existing biological theories (Insel, 2011) and caution that DSM diagnoses are “heuristics” not to be misconstrued as “natural kinds” or “real entities” (Hyman, 2010). It is therefore confusing to observe these same individuals state elsewhere that mental disorders “are recognized to have a biological cause” (Insel, 2010; p. 5) and are “real illnesses of a real organ, the brain, just like coronary artery disease is a disease of a real organ, the heart” (Hyman at the 1999 White House Conference on Mental Health, quoted in Albee & Joffe, 2004). Use of the term “disease” in the context of mental disorder reflects an expanded definition in which cellular pathology is replaced with subjective report of distressing or impairing psychological symptoms, the presence of biological correlates, or the assumption of an underlying disease state as yet undiscovered by science (e.g., “…mental disorders will likely be proven to represent disorders of intercellular communication; or of disrupted neural circuitry”; APA, 2003b). From this perspective, any DSM diagnosis is eligible for disease status (Peele, 1989), and what constitutes a “brain disease” is subject to the vagaries of the individuals in charge of determining the disorders and symptom criteria sets that comprise the latest version of the APA's diagnostic manual. [...] Given the limitations of existing knowledge about the biological basis of mental disorder, declarations that mental disorders are “brain diseases” (Volkow, n.d.), “broken brains” (Andreasen, 1985), or “neurobiological disorders” (CHADD, 2012) are perhaps best understood as the product of ideological, economic, or other non scientific motives.

In a piece that’s very much giving stop buying avocado toast, Buzzfeed asks a financial planner for advice on how to get out of debt when you have ADHD.
Debt is framed simply as a result of “impulse spending”, and capital management expert Daniel DeWitt’s tips include things we’ve all been told a thousand times, like not using credit cards, cancelling subscriptions, and cutting back on that pesky expense, food.
It’s a classic American message of personal responsibility filtered through a diagnostic lens, the solution to economic hardship presented as learning better “self-management” skills to generate more wealth. I can’t help but think about the idea of false consciousness here — those structures or ideas that hide the real nature of a capitalist economy.
In this case, it’s the idea that a brain disorder is the cause of your money problems. The DSM is being used to obscure the real cause of economic hardship: exploitation.
Svend Brinkman has described diagnoses as “anchors” in a modern world that is constantly shifting and accelerating. They can be a kind of answer (see: the rock bottom to “it finally all makes sense!” trope in diagnosis stories), but they can also take all the politics out of our pain.
A biomedical view of ADHD explains economic hardship with a detached neurological lens that seems to exist somewhere outside of history. But debt isn’t caused by Bad With Money Disorder — it’s a tool of class warfare, and ADHD or not, we’re all being targeted.

In the early 1990s—at the peak of a 20-year growth in the US crime rate—the federal government announced the launch of a "violence initiative." Headed by the US Public Health Service and backed by senior psychiatrists such as Fredrick Goodwin (then chief scientist at the National Institute of Mental Health (NIMH)), this project drew on biological theories of crime which dated back to the nineteenth century Lombrosian concept of the "born criminal". It was proposed that a mass-screening programme of inner-city children would be undertaken across America to determine those biologically or genetically predisposed towards anti-social and violent behaviour. As a vaccine against criminality, once the "conduct-disordered" children had been identified they could then be administrated psychotropic drugs. Breggin and Breggin's detailed discussion of the violence initiative rightly demonstrates the racist ideology behind the supposed objectivity of this biomedical project; a focus on inner-city youth is blatantly a focus on minority and black communities. At the time, Goodwin allegedly made remarks at the National Advisory Mental Health Council comparing "inner-city youth to monkeys who live in a jungle, and who just want to kill each other, have sex, and reproduce".
Psychiatry's involvement in such projects is perhaps less shocking when considering their long support for racial theories of the mind. In 1850, physician Samuel Cartwright reported in The New Orleans Medical and Surgical Journal his discovery of two new mental disorders affecting slaves in the Deep South: the first, drapetomania, was a disease causing slaves to run away from their owners, while the second, dysaesthesia aethiopis, resulted in the slaves becoming lazy, showing a lack of respect for the rights of property and breaking work tools. The prescribed cure for both disorders was "whipping, hard labor, and, in extreme cases, amputation of the toes". This psychiatric naturalisation of slavery as normal, inevitable, and even healthy for the black slave has been referred to by Burstow as a blatant example of "social control medicalized." Yet as Greenberg reminds us, for the burgeoning community of mad doctors, the discovery of such mental conditions held out the promise of contributing to contemporary society through the establishment of new "scientific" ideas in the area.
The commonalities between slavery-era diagnostic constructions and psychiatry's recent focus on inner-city youth are what Breggin and Breggin describe as "the psychiatric labeling of resistive or rebellious activity in order to justify medical control." This process of enforcing the status quo through the biomedical pathologisation of the political has allowed the psychiatric profession to enhance their respectability, capital, and power in capitalist society. Though treated with suspicion by some colleagues in the north of the United States, Cartwright's ideas were widely supported by fellow physicians, local politicians, and slave owners in the south. Whereas the classifications were abruptly consigned to history by the civil war only a few years later, drapetomania, along with Kraepelin's biological theories on praecox (later relabelled as schizophrenia), were highly influential on medical researchers in the early twentieth century who contended that African Americans were "biologically unfit" for freedom.

Your blog has gotten me interested in antipsychiatry as an extention of bodily autonomy - however, at least some disorders do seem to be clustered in a way that makes sense for troubleshooting if someone needs treatment. Like, reading the definition of depression as a teenager made me realize that it's not normal to want to die 24/7, and that I should probably talk to a therapist. I don't know exactly where I'm going with this tbh, but I guess I think that some (at least my own) psychiatric disorders make sense and are pretty common ways that people can suffer?
yeah, i think it's pretty important to distinguish between diagnostics as a form of loose categorization (for example, i find ocd + anorexia both to be useful shorthand for experiences i have + share w/ many others) and diagnostics as a prescriptive form of Assigning Illness (such as the DSM). the experiences of ppl with depression are real + often similar to one another + it's extremely useful to have a culturally recognized term that summarizes a lot of painful experiences.
it's when those shorthand descriptions become prescriptive rather than descriptive (e.h you feel x way BECAUSE you have depression, not that you have depression BECAUSE you feel x way), medicalized (e.g this is because you are Sick, this must be Cured, this has a definitive biomedical cause), or alienated (e.g this experience sets you distinctly apart from Normal people, who have Healthy thoughts + brains).
even when you say thinking those things is not 'normal'- sure, most ppl probably don't- but is the concern that it's 'abnormal' or that it's making you miserable? what if everyone else DID feel the same way- would that mean you don't deserve relief from it? one of the biggest flaws in psych diagnostics is that it relies on the assumption that these experiences need to be stopped (and Stopped is usually their ideal goal, though they will settle for mitigated or reduced) not because they are painful but because they are 'abnormal' or 'disruptive' (often to functioning within systems which are inherently unjust like school, work, maintaining housing, etc). the concern is not suffering, but abnormality (with the assumption that there is even such a thing as a Normal Person).
take, for example, my dx of anorexia- when i experience torturous thoughts of hating my body, obsessing over what i eat, obsessing over if other ppl are judging me for what i eat, obsessing over my weight, etc, it is diagnosed as pathology. when a fat person experiences this, they are very, very often validated in these emotions because they are Supposed To feel that way according to fatphobic medicine + cultural norms. while tides are shifting in this regard, this was not just a silly little oversight of the anorexia dx but built in, because the assumption built in to psychiatric diagnostics is that extreme body hatred is only pathological if it's 'irrational'- if you are supposed to hate your body, as determined by norms, then you are 'normal' + there is no 'dysmorphia' present, therefore no care or concern.
in general, i think most systems of categorizing ppl fall apart + become harmful when they fail to recognize themselves as roughly constructed shorthand ways of describing deeply complex experiences- gender, sex, race, diagnosis (this gets foggier in certain medical diagnoses but still stands for the majority of them), identity in general rlly. but if we can see them for what they are- (fallible + limited) things we created to describe ourselves, not objects within us which control our behavior + thoughts, they are immensely useful for many.
