Real scientists unmask the anti-transgender Cass Review as methodologically flawed and misleading

The Cass Review, a widely cited report on gender-affirming care in the U.K., has been heavily criticized by researchers for its methodological flaws and unsupported claims.
A new peer review published in BMC Medical Research Methodology found that the review lacked statistical rigor, misrepresented evidence, and excluded key studies without justification.
Headed by pediatrician Dr. Hilary Cass, the Cass report dismissed gender-affirming medical care as unreliable, recommending "exploratory therapy," which critics argue is akin to conversion therapy.
The review applied biased analytical methods, misquoted previous studies, and selectively adapted assessment tools to justify anti-trans conclusions.
It also advocated randomized controlled trials (RCTs) for puberty blockers, which experts denounced as unethical. Despite its flaws, the Cass Review influenced policies, leading to a U.K. ban on puberty blockers and contributing to restrictive health measures in the U.S.
Chris Noone and his colleagues write:
Using the ROBIS tool, we identified a high risk of bias in each of the systematic reviews driven by unexplained protocol deviations, ambiguous eligibility criteria, inadequate study identification, and the failure to integrate consideration of these limitations into the conclusions derived from the evidence syntheses. We also identified methodological flaws and unsubstantiated claims in the primary research that suggest a double standard in the quality of evidence produced for the Cass report compared to quality appraisal in the systematic reviews.
Experts urge policymakers to reject the report, calling for research centered on patient autonomy and accurate scientific analysis.

Prof. Mia West
Overview and Project Objectives
This work originates from the publication of Prof. Lawson’s paper Breast Size and Cognitive Ability last month (Lawson, Breast Size and Cognitive Ability, Northwestern Journal of Science, 2025) and the ensuing discourse it has elicited both in the scientific community and society at large. Not only do we find it socially and politically harmful, but either intellectually dishonest or frankly substandard in both reasoning and the basic principles of experimental science.
Prof. Lawson posits that breast size in females is negatively correlated with academic ability and general I.Q. As this work will show, not only is Lawson’s methodology ruefully unsound, but his conclusions are so implausible one can only assume that, by his own logic, Professor Lawson must himself possess an impressive set of mammaries to believe such scholarship deserves serious consideration.
It is the objective of this paper to, entertaining such ludicrous premises, empirically disprove Prof. Lawson’s thesis on its own terms and hopefully set the matter to rest permanently.
Methodology
A research team comprised of four highly accomplished female experts (see Appendix A: Team Background and Initial Cognitive Tests), including the author, will take it upon ourselves to test Dr. Lawson’s hypothesis in a controlled, verifiable and reproducible basis. Having established a baseline for cognitive ability, we must consider the second element of Dr. Lawson’s proposed correlation; as we’re sure he’ll lament (given his manifest interests) all members of the team possess what can commonly be referred to as a modest chest size (see Fig.1: Team’s Initial Measurements).
In order to modify this factor gradually, we will be using Dr. Joanna’s Marsh experimental Focused Hormone Enhancement System, or FHES, which has shown remarkable results, even in its early stages of development (see Marsh, FHES Preliminary Observations, Oberlin, 2024).
Weekly breast measurements, as well as tests on cognitive ability, will be performed to track any changes in the research team, or most likely, lack thereof (at least in the intellectual side of Lawson’s correlation). Additionally, this author will provide any observations and comments that might be relevant to the purview of this paper. 
Observations: One Week Under the Effects of FHES
Initial results show that the effectiveness of Dr. Marsh’s compound not only meets the expectations established in her paper but, in the present use case, surpasses them (see Fig.2: Team’s Measurements, Week 1). All four team members have been forced to acquire new brassieres to work comfortably. 
A common reported side effect among the team is a degree of difficulty maintaining focus on complex tasks for even moderate periods of time- what is referred to as “brain fog” in common parlance. While not debilitating, this phenomenon has increased the difficulty inherent in writing a clear, concise overview. It does not, however, seem to be accompanied by physical exhaustion- activities which require movement have not been reported by the team as feeling more taxing.
We attribute this “brain fog” to the adaptation to the new hormonal load, and we expect it to subside soon. This also explains the slight decrease in the result of the team’s cognitive tests (see Appendix B: Cognitive Tests, Week 1).
Of additional note is that the team has experienced a slight but constant emission of clear, vaginal fluid. Testing shows it to be harmless and indeed to be the kind of fluid generated for lubrication normally during intercourse or arousal in general. We believe this to also be merely a temporary hormonal adjustment, but will keep monitoring it in the following weeks.
   Observations: Two Weeks Under the Effects of FHES
The effectiveness of FHES continues to astho asst surprise the team. So the flavour of the day is Bad Fandom Stats, and it turns out that not only is centreoftheselights using a wildly inaccurate method of counting ship popularity during a given year for the Year In Review fandom stats, but also that nearly every column on the chart is a lie. @5ummit exploded the whole methodology issue last year, so I'll just link their post and dive into other stuff.
One would think, looking at a list of popular ships with their fandoms listed next to them, that the named fandom is simply the one within which the ship exists.
The ship at the top of that list is "Evan "Buck" Buckley/Eddie Diaz (9-1-1 TV)" and the fandom is listed as "9-1-1 (TV)"; that's pretty straightforward, there's only the one TV show and those characters are from that show.
But then we get down to ships with characters that exist in multiple versions or subsections of a canon which have their own fandom tags on AO3, and things start getting janky
One of the first things I noticed was weird about this year's chart (aside from the numbers themselves being just straight up wrong) was that the fandom for Kirk/Spock was listed as "Star Trek: Alternate Original Series (Movies)", the AO3 tag for the reboot movies. That felt wrong, because while I know the reboot movies are big on AO3, most of the K/S stuff I've seen recently has either been expressly original cast or not specific to any one cast or iteration of canon.
I thought that the list might have been saying that Reboot Kirk/Spock alone was big enough to make the list while Generic and Original Kirk/Spock were separate fandoms that hadn't gotten onto the list. That would be absurd for new fic count, but there are stranger things on this list and the methodology favours newer ships, so I went digging.
A search for Kirk/Spock fic posted in 2024 and a glance at the sidebar gives us this, out of 2,255 total fics

Searching for Kirk/Spock fics updated, rather than newly posted, in 2024 also puts the All Media Types and Original Series tags above the Reboot Movies.
But searching the entire unfiltered ship tag gets us this, out of 22,426 fics

This means two things.
1.) Despite the listed fandom on the year end chart, Kirk/Spock (and by extension, all the other ships from fandoms with more than one iteration of canon [and some Batman ships are on there, so we've got far more complex things than simply reboot movies and TV/anime adaptions of novels/manga]) is a generic ship tag.
2.) the fandom listed was taken from the unfiltered tag, not from this year's data.
And centreoftheselights confirms that this is indeed her methodology.
The fandoms are, quite simply, wrong.

Since we do not have access to her data and her methods are not replicable, we can't check how many of the ships might have been struck with a mismatch between which version of their canon is most popular overall vs this year.
I can't even be certain that there is a mismatch for Kirk/Spock. It's possible that the fics that actually make up her numbers have more reboot movie fic than otherwise. No one will ever know.

It is interesting to dig into the differences. I find it utterly fascinating that Kirk/Spock had a period that pushed the reboot movies to the top of the list overall and has since settled into Original Cast being more popular, all while the ships remained consistently popular enough to regularily end up on Top Ship Lists.
Centreoftheselights' data does not allow us to dig into those differences, or even to meaningfully speculate about them.

And yet more! As I explained in this reblog, the "type" column is not harvested from the data itself, but is a subjective interpretation of what centreoftheselights believes the characters' genders is or could or might be. So not only are all the columns wrong, they aren't even wrong in the same ways.
Over the whole sheet, the columns are: ⚠️Rank: Well, it accurately lists the order of the inaccurate counts, so I guess the column technically isn't inaccurate ⚠️Change: This is indeed the change in rank from last year's chart. guess what api-less website expressly forbids scraping of any kind, meaning I'll have to manually look up every last fucking character on the goddamn list do you understand how many that is aaaaahhhhh
not me finding out that fandom wikis do not have an api (which is like a specialised ui for programmers that a lot of websites have where you can more directly access resources off the website with programming tools, f.e. if one piece wiki had one, you could tell your coding languages like "hey get all the strawhat pirates' names and heights off their character pages and put them in my database here please" and it would be able to get that info off the wiki for you) so I will have to learn web scraping to get data off there for visualising ship stats project I have planned for my data analysis portfolio instead now smh

The laboratory was always a place of order. Every instrument had its designated space, every experiment followed a precise methodology, and every result was expected to be recorded without deviation. Dottore thrived in this structured world, where logic dictated every decision and unpredictability was a flaw to be eliminated.
You, his child, however, are the embodiment of unpredictability.
At first, he had thought it was simply a phase—an inability to focus, an aversion to certain sensations, an inexplicable resistance to what he deemed perfectly reasonable expectations.
You fidgeted constantly, your attention drifting between his words and the unseen world in your mind. You would recoil from certain fabrics, cover your ears at loud noises, and refuse to make eye contact for prolonged periods. Your interests were deeply fixated, but not on the subjects Dottore had hoped you would inherit.
No matter how many times he introduced you to the marvels of biology, chemistry, or engineering, you simply didn’t care. Instead, you were drawn to colors, textures, and shapes. You would spend hours sketching, completely immersed in your art, but the moment he placed a scientific text in front of you, your focus dissolved.
Frustration grew between the two of you like an unspoken wall. Dottore tried to enforce structure, believing discipline would instill the focus you lacked. It didn’t work. The segments had mixed reactions—some found your unpredictability amusing, others were visibly irritated. Pi, ever the perfectionist, struggled with your chaotic energy, while Gamma found you fascinating, often sitting beside you as you scribbled away with reckless abandon.
The tension came to a breaking point one evening when Dottore, in a rare lapse of control, snapped.
"You need to learn discipline," he said, voice sharp as he snatched away a piece of paper from you that is covered in vibrant, swirling colors. "Art is a hobby. It will not serve you in any meaningful way. You must focus on what matters."
You froze in place, expression unreadable as you got up and abruptly stormed off. You didn’t yell or cry. You simply left, your small hands balled into fists. That night, you refused to come out of your room, even when the segments tried coaxing you out. Eta, the youngest segment, who often acted as a bridge between you and Dottore, shook his head solemnly. "You hurt them."
Dottore had spent his life dissecting, analyzing, and manipulating the human condition, but the realization that he had hurt his own child in a way he couldn’t immediately fix was a foreign and unwelcome feeling.
It wasn’t until he spoke to Beta—one of his more observant segments—that he truly began to understand.
"They don’t think the way you do," Beta explained. "Their mind works differently. What you see as distractions are part of how they function. You can’t force them to be like you. You have to adapt, or you’ll lose them."
Adapting was not something Dottore did often. He demanded the world shift to his design, not the other way around. But the idea of losing his child, of pushing you so far that you no longer trusted him, was unsettling.
So he changed. Slowly. Awkwardly.
He learned that you needed noise while working—so he allowed you to hum or tap your fingers against the desk while drawing, even if it grated on his nerves. He learned that certain textures made you uncomfortable, so he adjusted your clothing accordingly. He learned that eye contact wasn’t necessary for communication and that sometimes you needed to move around while talking to him. He stopped trying to force you into rigid routines and instead built flexibility into your days, allowing you to take breaks when your focus waned.
Most importantly, he began to acknowledge your interests rather than dismiss them. When you drew, he studied your work, asked you about your techniques, and even provided you with higher-quality materials.
One evening, he found himself watching you sketch a complex diagram of the solar system, eyes alight with excitement as you explained planetary formations in a way that rivaled any scientific explanation.
“You like astronomy,” he mused, watching as your pencil danced across the page.
You nodded enthusiastically. “It’s like… science, but with colors and movement and so many possibilities.”
Science, but with possibilities. That, Dottore could understand.
The segments adapted as well. Gamma became your willing test subject, letting you doodle on his arms with ink. Eta took to making up stories about the things you drew, turning your creations into fantastical narratives. Even Pi, who had once been the most resistant, learned to engage with you in small ways, challenging you to replicate complex patterns in your art.
One day, you timidly approached Dottore with a new sketchbook in hand. You hesitated before handing it to him. “I… I drew something for you.”
He opened it to find an intricate illustration of his lab—meticulously detailed, yet softened by artistic flourishes. The test tubes shimmered, the machinery gleamed, and in the center, surrounded by all his segments, was him, drawn with an unfamiliar gentleness.
Dottore stared at it for a long moment before finally speaking. “…It’s exceptional.”
Your face lit up, relief evident in your eyes. “Really?”
He nodded. “Really.”
From that day forward, your world found an unexpected harmony. Dottore still didn’t fully understand the way your mind worked, but he no longer tried to force you into a mold you weren’t meant for. Instead, he learned to meet you where you were, to build a bridge between logic and creativity.
And in return, you flourished.
Because while you could never be the scientist he had envisioned, you were something just as brilliant—his child, in all their chaotic, artistic, and endlessly fascinating glory.

What’s the difference between Malleus and Idia? What Idia was about to do when he overbloted would have been just as bad as what Malleus is doing if not worse. (Releasing a bunch of OBs and reshaping the world would not be safe for everyone) is it just just because Idia has a more sympathetic back story? I understand that some people have their likes and dislikes, but I’m just curious. I love all my Twisted boys!

I’d say that the scale of Idia and Malleus’s machinations (and even both backstories involving the loss of a family) are similar. I don’t believe that Idia has the “more sympathetic” backstory; if anything, Malleus is also very relatable and sad because we can all relate to the fear of change + losing someone precious to us and/or wanting to stay in a happy "fantasy" world (ie TWST) forever. However, the methodology and the context surrounding their actions are not the same. That’s what makes Malleus’s actions so much more despicable to me. (Again, I want to stress that this is NOT an objective take; this is my own interpretation of the events, characters, and their motivations.)
Firstly, Malleus is exerting much more power over his victims and actively tries to maintain it. He has already robbed ~20,000 people (the population of Sage's Island) of their autonomy and essentially gaslit them into accepting it as their reality. Idia has employed no such physical control or emotional manipulation. All he has done is release Phantoms out of Tartarus to rampage around the world. Both he and Malleus are putting Twisted Wonderland in danger, yes--but Malleus is also committing the moral wrong of forcing his will upon others' bodies and minds. The only other OB to involve manipulation/mind control is Jamil, and even then that was on a much smaller scale by comparison. Whenever someone so much as challenges Malleus's word, he or his autonomous magic goes out of its way to trick them back into sleep or pulling them even deeper in--and this act is much more insidious than just ripping away their free will altogether (as Jamil had). Malleus is knowingly and willingly deceiving thousands and thousands of people and making their brains process dreams as if they were reality. This makes them his mental captives. On top of that, Malleus literally controls TIME AND SPACE ITSELF within his magical force field. Idia's plan, meanwhile, never goes that far, and his control certainly does not extend to manipulating time and space itself. It could be argued that his plan was to "play hero" to save people from Phantoms so that he could make friends this way (which is also deception), but let's remember that this was Ortho's suggestion, not Idia's. We have no clue if Idia himself would carry it out, nor it he would go to the same lengths of gaslighting that Malleus did. For all we know, he could have just drawn the line at letting the Phantoms run free and being "friends" with everyone.)
Even if we assume that Idia was fully intending to be just as deceitful, Idia and Malleus are very different people, so the context and motivations behind their OBs are very different as well. The loss that Idia experienced was that of a close family member, and he has been living with that grief for several years prior to his OB. His OB, then, is in an attempt to adapt to that lingering guilt and shame and to "fix" the past. When he loses to the other boys, he offers to "go" with Ortho to right his wrongs, but ultimately doesn't. He's willing to take all of the "burden" upon himself for what he has done in the aftermath. Malleus meanwhile lost his parents, who are figures he never became intimate with or really got to know because they vanished before he had hatched. What Malleus experiences before his OB is the fear and anxiety of losing his loved ones (Lilia, Yuu, etc.), something which has not happened yet. He is unable to accept these changes, so his OB is a desperate act to prevent those events from unfolding. Idia is fixated on correcting the past, and Malleus is fixated on stopping the future--he's determining everyone's future for them. Because of this, I perceive Idia as a person with hang-ups about previous life experiences that he's overcorrecting for while Malleus comes off like a three-year old throwing a tantrum and making it everyone else's problem too.
Another element to this is that whereas Idia has to take some level of accountability for his actions (even if it's just a scolding and banishment from his parents + feeling upset with himself), Malleus never does, even in scenarios where he clearly endangers the lives of his classmates. He can claim it's "by accident" or "just a misunderstanding" all he wants, but it won't erase the worry or the battles his classmates went through for the "surprise party" he wanted to have for them. He always walks away feeling as though he "did the right thing". So what happens when Malleus, AGAIN, endangers the lives of several people? Is his power and social status going to save his ass AGAIN? Is he going to be let off way easier than everyone else AGAIN? That's what's really frustrating to me. Malleus's refusal to accept change AND the consistent lack of accountability feels very... juvenile. In-character for Malleus, but still very juvenile. Of course, book 7 isn’t done yet so maybe Malleus will eventually see the error of his ways. Right now though? All of the other stories leading up to this point in time rarely show Malleus taking accountability for anything he does, so my expectations of him aren’t great to say the very least.
A common defense I hear for Malleus is that "he's still considered a baby by dragon fae standards". Yes, I know this. However, that doesn't give Malleus a pass for his behavior. This has always been my stance on this matter. To me, he is still at least old enough to know what is an abuse of power or not--yet he acts so hypocritical, it's infuriating. For someone who is supposedly a good leader (according to his own dorm members) and cares so much about his relations to other countries (to the point where Malleus chides Sebek for being rude to other royals), I sure do see a lot of inappropriate emotional reactions and acts of abusing his power/social status (both of which can seriously harm his public image and sully his political reputation) 💀
But again, that’s just my interpretation and opinion on the matter; if you think differently then that’s valid as well!

on sua and speaking, ivan and class, and ivansua similarities
furthering my analysis on ivansua in a continuation of this post, where i talk about ivan's answers in their interview with andromeda media regarding their status as outstanding students
sua speaks comparatively little. ivan's long (thorough maybe...) responses are largely informed by his struggle to assimilate from the slums into unsha's life for him. he uses his interview as an opportunity to position himself as an archetypal success story. if you work hard, show deference, and practice correct behavior, you will be rewarded. by contrast, sua's responses are not tailored this way.
sua is trained to speak minimally. she is the youngest of seven (?) human pets, and nigeh's favorite daughter. she maintains this status by making herself into a perfect, empty doll—no desires, no dreams, no purpose outside of service to her mother. in her relationship with nigeh, she speaks only when she's made to (which means her self-advocacy skills are abysmal, something reflected in her relationship with her sisters, who bully her and push her around). speaking only when "necessary" is a behavior she internalizes as essential to survival, and something she replicates across her personal and professional relationships.
this particular answer highlights the difference(s) between ivansua's perspectives

ivan breaks down his studying methodology step-by-step. he clearly provides an ideal answer for the interviewer. it's almost comically perfect, the exact type of thing someone would want to hear from an outstanding student. ivan plays this role well. his analytical skills empower him to adapt to any circumstance and legitimize himself in an academic context.
however, sua answers with icy precision. the formula for success isn't complicated for her, it's a matter of observation and obedience. like her life with nigeh and her sisters, nothing was about sua's desires, or her personal aspirations, or her own goals. they were about following instructions, "memorizing" what was to be done and completing it without complaint or complication. vivinos has highlighted sua's "weak" mental state and inner darkness while qmeng has emphasized her aggression. it's important to square the two by acknowledging how sua has been subjugated by her family (as well as how it impacts her attitude and actions) AND the ways in which her agentive behaviors change her life, and mizi's.
ivansua are very similar. their survival depends on negotiating their power through participation in a system designed against their mental, physical, and emotional wellbeing. they are in stark contrast to mizi and till, who are more obviously deviant and openly rebel in some form or another, and who are not capable of or willing to perform this manipulation (which is why ivansua are drawn to them)
i wanted to highlight this part because they're discussing their respective fixations:

firstly, it's no surprise that the class ivan is the most confident in is music appreciation. the more he understands something, the more useful it becomes to him, and the more secure he feels.
more importantly, mizi's "deviance" (or academic struggles) requires that sua support her. this fosters their closeness. mizi's failures create a need for sua, which sua is eager to fill. by answering this way, she characterizes herself as kind and helpful. but she also hints at her and mizi's dependence on each other. it also highlights sua's "emptiness"—she does not have a favorite class, or one she's most confident in because she's developed a unique interest in a particular subject, or one that she herself had struggled with. her best class is the one that mizi needs help with. sua prioritizes mizi above all.
the order of these questions are also interesting to me. ivan doesn't anticipate that sua will be this open. as soon as she alludes to mizi, he takes it as license to mention till. prior to this answer, he talks mostly about his own affairs and is congenial. but here, he breaks the facade and says something surprisingly rude. there's a note of fascination in his tone. till's deviance—his rebellion, his ferocity, his disruptive habits, his indomintable spirit—separate him completely from ivan. he can't understand till, which stokes his all-consuming obsession with him. when sua says "yes, i think so," all i can imagine is her side-eyeing him like "get a grip."

hii, something i never understood is the difference between revivalist and recostructionist in the terms of helpol, may i ask you what are the differences?

These definitions are sometimes a little different, depending on the person, but the way I've come to understand it is like this:
Reconstructionism: A methodology of practice that attempts to recreate the religion based on ancient worship, typically according to academic and historical resources.
Revivalism: A methodology of practice that emphasizes modern adaptations for worship and the modern day (this does NOT mean revivalists don't engage in historically arrested worship, to clarify).

One is focused more on "change" (revivalism), while the other focuses more on "tradition" (reconstructionism). This is probably the easiest way I can find to explain the difference. These methods can, and do, exist on a spectrum.
Personally, I don't care much for these labels. I don't enjoy labeling myself or my practice, as it feels very limiting for me. If I absolutely had to choose a label, I feel my practice leans more towards reconstructionist offline. I feel that these labels are sometimes used as a means to divide the community into two groups, which I'm not a fan of. Obviously, they're not always used that way, and people use these labels all the time without making it an "Us vs. Them" thing.
Anyway, I hope this answers your question! These definitions vary between people, as I mentioned, and these are definitely not rigid definitions. This is just how I would describe these terms in my own words. Take care, Nonny! 🧡
