for a fun follow-up to this post
consider what all of this means for medications; how we digest them, what they actually need to do to be considered food within our body so that we will break them down and achieve the desired results
i learned a few years ago that the majority of supplements and vitamins aren't even bioavailable for the average person. this is largely dependent upon genetics, but an even bigger problem is that since supplements and vitamins are not regulated by the FDA, many (most?) of them are not bioavailable to any person.
chlorophyll supplements, for example, have zero definitive research on whether the human body even processes the chlorophyll. and people pay for this.
meanwhile, people with a homozygous mutation of the mthfr gene (known as the "motherfucker mutation") don't use folate as quickly because their liver cannot process it from many sources. even 90% of folic acid supplements aren't bioavailable at all to people with this gene because they're processed using a pathway their liver doesn't have. i have a heterozygous mthfr, which is a factor in my depression, chronic pain, gut issues, brain fog, and it probably contributed to my mom's hypothyroidism, if she's the one i got it from. people with these mutation have to get extra folate from extra sources because we just don't process most of them.
we know very little about most precription medications. ssris, for instance, aren't even proven to work let alone effect the brain chemistry in the specific ways shared cultural knowledge claims they do.
anything we swallow our body is going to try to digest, which means that medications are being processed the same way as nutrients, at least at first. so maybe we would have better understanding of medications, how they work, and what they really do if we studied them with that understanding from the start.
what medication choices would i have made differently if i knew how my body might break them down and that that might to do different parts of me? i don't mean the "may cause liver damage" nonsense, i mean does it leave a chemical burn? does it process so slowly it backs other important things up? does it bond with random liver molecules and steal them? does it fundamentally alter the way my liver will work every time it goes through it? will it leave residue, forever stuck in my liver, that scientists in the future will cut up and point to and exclaim "aha, z was on ssris, no wonder their liver has this purple spot!"
i've done a lot of drugs, things doctors told me would help xyz problem. interestingly, nobody thought i might be vitamin deficient until adulthood, nobody noticed i had the mthfr gene or a chronic b12 deficiency.
actually, i'm the one who figured out the b12 thing because my blood was always on the lowest acceptable levels* every time i had a blood draw, which is a pretty weird place to be for someone who is consuming b12 enriched energy drinks 3 or 4 times a day, right?
* (acceptable by the computer... if you look up the actual numbers, i was at severe risk of psychosis which answers quite a few questions.)
i would feel our scientific exploration and curiosity in these fields are gatekept to education, but i've been listening to a podcast recently ("Just Eat Normally" with Dr Rachel Evans) where nutritionists and dieticians have come on and talked about how hard it is even in that field of education to get into the details of what our bodies genuinely need without a "weight loss is good, actually" blanketed approach. the classes that cover bigger bodies are specialty and cost more. and so much of it is treated with a one size fits all approach.
as an aside, i have a terrible immune system. i catch every passing cold and flu and have had covid twice, the two times i was really exposed by someone in my house. i've posted on here before, i think, how my bad immune system was part of how i feel so far behind in school i had to test out, because i was missing up to 3 months a year from strep, the flu, rhinovirus, allergies, etc.
i've been trying to eat incredibly healthy recently because i hate having so many disturbances in my life from my weight and all of the nutritional imbalances that got me to this weight. i've been eating as wide a variety of foods as i can manage, in smaller portions but including impulse buying vegetables that look pretty, fruit that's easy to pick at for a few hours, different types of noodles and breads... besides losing nearly 30 lbs, i've had another strange effect.
everyone in my house has covid right now and i'm fine.
maybe i brought it home and didn't feel it, maybe my recent malaise was covid. maybe i'm coming down with it right now. but this is notable and weird because historically, i catch things immediately from exposure, i incubate them pretty quickly, and i'm physically devastated by them. my partner has been out of comission since monday, and this week i've been energetic and feeling fine. i never feel fine, even on good days; i have severe allergies and chronic, disabling illness. even if i get sick, there is no timeline that makes sense based on my norms.
having a wider variety of foods, and more of the macro *and* micronutrients that i've never really had the education to seek out or even realize i needed, purely by accident has changed how my body is responding to exposure to an illness i already know i'm particularly susceptible to.
even while i've been eating less overall.
Why are agriculture classes the first time I've learned extremely basic info about nutrition and how digestion works. Why isn't this stuff in health textbooks or any easily accessible resource about healthy eating.

Health update
I am feeling much better and my skin is clearing up
I took a shower this morning and my face didn't turn tomato red. It did itch a bit but nothing that was too bad.
I have a new concerning rash on the back of my neck and my derm said to put steroid cream and antifungal on it for 2 weeks. It should disappear.
My hand was still infected as well, so I'm on oral antibiotics again and that should take care of the issue hopefully. I was also advised to apply antibiotic and steroid creams on my hands for 2 weeks. My hands are much better, and they are less inflamed. But I'm concerned about some infection and some areas that are still inflamed. So the antibiotic and steroid mix can help here.
My face, neck, and ears are much better.
I think my skin got better after my phone call this week as well as my official psych evaluations. It makes me realize that this all happened due to chronic stress.
I think my stress is a big thing that I need to work on because it's making me sick. I will get the support I need, but it will take time. I am still working on my diet and fasting and am doing great. I am working now on hormone balance and liver detoxification. I don't have any major hormone issues besides some minor estrogen dominance. I can manage that by taking DIM. I do want to eat well and drink filtered reverse osmosis water. Minimizing plastic usage and using natural self care and cleaning products helps too (which I already kind of do, I might look into kitchen and laundry and soap options). Also it is impossible to avoid "toxins" 100%, so it helps to do the best you can and to regularly "detox". When I say that I mean to support your key detox organs like the gut, liver, lymphatic system, lungs, skin, and kidneys. I'm not talking about weird and dangerous enemas and teas.
I've been taking a DIM detox supplement for liver support. I think the key ingredient it has that really helps is NAC/glutathione. I might look into that in the future when I finish my supplement bottle. Methylated B vitamins, sulfur, amino acids like taurine, glutamine, cysteine, glycine, NAC/glutathione, magnesium, flavenoids, and antioxidants. I don't know how much of these I'm receiving from diet and supplementation (excluding the DIM Detox supplement I just started) alone, but NAC/glutathione is very important for liver health in general. If I can get that, then that would be great.
Drinking enough water and eating enough fiber helps a lot here. Eating foods rich in sulfur like cruciferous veggies helps too. Dandelion root tea is excellent for liver and kidney health, which in turn helps with detoxification. I actually got some the other day and I like the taste. But I just had 1 cup which isn't going to make a notable difference. I want to drink that combined with an anti-inflammatory diet rich in fiber and drinking 2 L minimum of filtered/reverse osmosis water.
Also there is a key process within phase 2 liver detoxification known as methylation. Some people have a genetic mutation in the MTHFR gene where they are unable to do this efficiently. That's why doctors recommend a methylated B complex. So vitamin B9 (folate) and B12 should be in methylated forms. NAC/glutathione also help here. I've never been tested for this, though. But I do take a multivitamin that has B vitamins.
I'll figure this out as more time passes. At least I'm taking a supplement that helps with both hormone balance and liver detoxification, and I liked the dandelion tea. I'm also drinking aloe juice daily. I need to work on healing psychologically, though.

Part of the problem is that CFS/ME is unlikely to be just one pathology. Its a syndrome - a collection of symptoms. Thats part of why I have never been satisfied with that answer and so have tried to keep up with research into its pathology and comparing things I've found to my own symptoms. Following that research is what led to me reading about MTHFR mutations, which led me to taking methylfolate, which led to me discovering I likely have both a methylcobalamin deficiency and pyridoxal-5-phosphate deficiency that are the root of my CFS, POTS, worsening MCAS, hypersomina, and other progressive symptoms.
MTHFR mutations affect between 1 in 3 and 1 in 10 people, depending on race and nationality. They cause a deficiency in methylfolate which raises homocysteine levels and has a similar effect to a B12 or folate deficiency, but with normal B12 and folate levels.
Cobalamin metabolism disorders are more rare, but they are also very easily missed because total B12 blood tests are normal. The most common cobalamin metabolism disorder causes a deficiency of the usable forms of B12, leading to elevated methylmalonic acid and elevated homocysteine as would be seen in a B12 deficiency from diet or malabsorption. More rare disorders, or currently unknown and so undiagnosed disorders, can cause just a deficiency of methylcobalamin which will have nearly identical symptoms to a methylfolate deficiency.
One research team has hypothesized that an autoimmune disorder affecting the production of adenosylcobalamin may be behind multiple sclerosis. Impaired B12 metabolism leads to defective myelin, which the immune system tries to clear out. Something similar happens with methylcobalamin deficiency. Only genetic cobalamin metabolism disorders are recognized currently, but there could be autoimmune forms as well.
Similarly, some researchers think MTHFR mutations may be behind the trifecta of hEDS, POTS, and MCAS where patients often also fit CFS/ME and fibromyalgia criteria. Methylfolate deficiency can cause methylcobalamin deficiency, and methylcobalamin deficiency also leads to faulty myelin. Methylfolate and methylcobalamin deficiencies also cause problems with how DNA is read to make proteins, leading to faulty collagen, more damaged enzymes, and a variety of other metabolic problems.
The degree of impairment of the enzymes involved would impact the degree of severity of the damage done to the body. A mild deficiency over time would cause a slow, steady increase in platelet count, worsening allergies/MCAS reactions (homocysteine can trigger mast cells), and a wide variety of neurological symptoms as myelin degrades from lack of methylcobalamin and homocysteine causes excitotoxity.
While researching this, I found that there may also be millions of people with a functional pyridoxal phosphate deficiency who have no idea and no doctor is going to know to look for it. The people at risk are those who take B6 supplements and anyone who takes replacement thyroid hormones, both T4 and T3.
Most B6 supplements are a form of the vitamin called pyridoxine hydrochloride. This form can't be used by the body as is, it has to be converted into pyridoxal phosphate first. However, if taken at dosages that overwhelm the conversion to PLP, the pyridoxine itself can bind to PLP receptors on enzymes and block their function.
Free thyroid hormones also bind to PLP receptors, and they do it readily. A study in the 1950s found giving thyroid hormones to rats significantly blocked PLP enzyme function, especially in the liver. Ingested thyroid hormones are free when absorbed by the intestines and sent to the liver, and from there they can bind to carrier proteins which keep them from going places they aren't supposed to. In order to stop the thyroid hormones from interfering with the PLP enzymes, you need to significantly increase how much PLP is in the blood. How much more exactly is unknown, but is likely proportionate to the hormone dosage. Without supplementation or a diet quite high in natural B6 sources (not fortified sources), the body will start to use up the PLP it has stored up in the past. How fast someone depleates their PLP stores depends on how much B6 they get from their diet and thyroid hormone dosage. Functional PLP deficiency has the same symptoms as B6 deficiency, but bloodtests for the vitamin will be "normal." The demand for the vitamin is greater, so standard deficiency screenings aren't going to be useful.
PLP deficiency can also cause elevated homocysteine. Since enzymes in the intestines and liver are the most affected, people may have changes in gastric motility and in liver function tests. Over time, as the stores of PLP run low, people would develop signs of an overall B6 deficiency including neuropathy, dry and cracking skin, mood and sleep changes (PLP is necessary to make serotonin, dopamine, and melatonin), systemic inflammation and oxidative stress (PLP is needed to make the bodies most abundant antioxidant), and liver damage or disease.
Theres also a number of genetic conditions that can affect PLP levels, and, theoretically, there could be an autoimmune impact of PLP metabolism, so people who aren't taking thyroid hormones or pyridoxine can still have a PLP deficiency with normal total B6. PLP levels can be specifically tested and would show low in most of those cases.
The best news about both methylcobalamin and PLP deficiencies is that they are treatable without a prescription. Methylcobalamin lozenges or liquid for sublingual administration (to avoid the first pass effect which would make the supplement useless for the specific conditions that cause low methylcobalamin) and pyridoxal-5-phosphate/P5P/PLP are both available for a fairly low price online. Both are water soluble with limited uptake to storage and no toxicity, so whatever isn't either used or stored will be excreted in urine.
I have had CFS since fall of 2015. I was diagnosed with Hashimoto's and began taking thyroid hormones spring of that year. The onset and progression of all of my symptoms match having either a methylfolate or methylcobalamin deficiency combined with a PLP deficiency.
Taking methylfolate when you dont have a deficiency will increase demand for methylcobalamin, and if you can't meet that demand you will get sicker. Thats what happened to me, even though I was taking a normal B12 supplement and had high serum B12 - thus indicating the methylcobalamin deficiency. I got a methylmalonic acid blood test which indicated no deficiency of adenosylcobalamin, which eliminated the more common cobalamin metabolism disorders. I'd rather be extra rare than have a trio of deficiencies though.
If you take methylcobalamin without a deficiency, nothing changes and you pee it out. If you take it and you have a methylfolate deficiency, nothing changes and you pee it out. If you take pyridoxal phosphate without a deficiency, nothing changes and you pee it out.
These supplements do not interfere with any medications. Thyroid hormones can impair PLP enzymes, but PLP has no effect on thyroid hormone receptors.
This means that methylcobalamin and PLP are easily accessible and safe supplements that people with CFS/ME, fibromyalgia, and/or hEDS trifecta can try even if they don't have access to a helpful doctor.
The remaining question is how long they take to have effect. I can tell you that the PLP helped me in a matter of days. Better energy, better executive function, and some improvement to sleep. The methylcobalamin will take longer to have impact as the major effect of the deficiency is damage to the nervous system.
My neurological symptoms primarily come from the lumbar region of my spinal cord and the reticular formation of my brainstem. I dont know of any way to predict what parts of the nervous system get affected most, but by listing out all of my symptoms and learning the functions of different parts, I was able to trace all of my neurological symptoms to those two places.
Based on other types of B12 deficiency and other adult-onset cobalamin metabolism disorders, treatment has the potential to lead to full healing of damage done in the course of the deficiency. Lasting damage seemed to mostly be in cases where patients presented with strokes, embolism, etc. which caused additional damage. Other patients have full recovery as long as they continue to have the form of B12 they need.
I've so far only been taking the methylcobalamin for less than a week. Thats not enough time to recover from 8 years of damage, unfortunately. Right now I just have to wait and not push my neurological limitations just because I have more energy and focus. I have some very objective symptoms that are what I'll be monitoring most, since others like chronic widespread pain are more easily influenced by other factors. Im hoping that I will be able to point to definite improvement of at least a symptom after a month, but it could take longer. There's honestly no way to know.
Now, disclaimer, I am not a doctor or any other licensed medical professional. I'm a disabled person with the resources to experiment on myself and the ability and education necessary to understand the science enough to connect the dots and weigh the risks. I weighed the risk of taking methylfolate, but I turned out to be an outlier with a 1 in 100 million condition rather than the 1 in 3 for my demographic (granted the 1 in 100 million may be wrong, but thats the approximate current diagnosis rate). However, since what I found indicates zero risk for trialing methylcobalamin and/or PLP, I feel comfortable sharing what I've learned and what I am doing. Its always a good idea to talk to a pharmacist about possible interactions and to talk to a doctor about any other conditions you may have, and taking any supplement is always at your own risk.
I also understand the desperation of people chronic pain, fibromyalgia, and/or CFS/ME for anything that may help their condition, and it is hard for me to keep information to myself while I wait to see how things go, when the information has the potential to start helping someone who may not be able to make it until I have everything in a clear presentation format and my personal results.
As I continue my own treatment trial, I will be reaching out to various doctors and researchers about this and sharing more about my history, research, and how things are going. I will also be working on having cited, edited, clear, and easy to distribute information rather than... this or "look into trying these supplements and just trust me" which sounds about as useful and trustworthy as "try yoga."
even if you did the barest minimum research, the Wikipedia page for chronic fatigue syndrome is so sad. it’s like “this is what we diagnose people with when we can’t find anything else wrong with them. but it’s definitely something physical because there are visible neurological changes seen in neuroimaging and weirdness with the immune system. full recovery rates are less than 5%. the most common treatment is CBT which ignores the fact it is a physical illness and tells sick people they’ll get better if they just stop thinking about being sick.”

Hello Joy! Long time follower and I really enjoy your blog. I just wanted to hope into your asks bc i've had a very frustrating day of doctors appointments and even if you don't have any advice, i was just hoping to rant to someone who'd been around the block with elusive medical illnesses (though please dont feel obligated to respond to this at all), and i apologize in advance for an almost certain lack of coherence.
I've been dealing with quite a lot of doctors appointments over the years, from treating insomnia, my adhd, and depression and anxiety, but long story short we have discovered 2 years ago that I have low b12. I have also been dealing with a chronic cough for 2.5 years. Unfortunately, oral supplements did not work for the b12 and i had to get monthly injections, which stopped when the pandemic hit and, despite doing for 4 months, didnt bring my levels up to normal anyway.
I recently restarted the injections, but my doctor will not approve my request to have more frequent injections, which i think i need, because its not fatally low (its at a 170 based on recent bloodwork). I finally saw a respirologist for my cough, and she has no idea what the cause of it is, even suggested that its psychsomatic, and put me on a different inhaler that i tried before (ive tried a million treatments for it, from post nasal drip meds to ppis for acid reflux to allergy meds, and the inhaler is the only thing that helped a little).
I've gained 20-25 pounds in the past 2 months, when ive been the same weight since i was a teenager (im 22), but my bloodwork continuously come back normal for everything but b12 and vitamin d, so my doctor also has no explanation for that (including normal tsh, and the last time i brought up testing other thyroid factors the doctor told me they dont do that and honestly made me feel silly for asking. Though, granted, my current doctor is a different one). I'm going insane because of how tired i am all the time, and the fact that I feel like a hypochondriac bc all my tests are always normal, and my doctor is only available for appointment once a month.
Sorry for the long block of text, i just feel, when looking at your blog, a sense of hope that eventually i'll have answers and you're just really great. ❤ i hope you had a really good day today ❤

When you dip below 250 is when you can start to experience chronic fatigue, mood disorders and worsening cognitive function.
170 is when you're on the cusp of things getting seriously bad and depending on your homocysteine levels can start to cause pain and nerve damage. This is also when my hematologist estimates I started to sustain prolonged damage to my nervous system. This was also when I started experiencing the symptoms of early onset dementia.
140 is when my iron and folate levels tanked suddenly and I was so symptomatic my (then) new doctor was worried I had cancer and rushed me through seeing seven specialists in a week. The same day as seeing the hematologist it dipped below 110 and I started hemolyzing and went into medical crisis as my kidneys and heart started to struggle. I owe that hematologist and his team my life.
The good news is the shots you are getting right now are keeping you stable, but your doctor should NOT be waiting for your levels to hit crisis mode to resolve this issue. Please do not give up on this, you deserve better treatment.
Also, what kind of supplements are you taking?
I don't absorb Cyanocobalamin well through my gut, which is the most common type of oral supplement prescribed. What I do absorb through the gut is Methylcobalamin, so if you haven't tried that yet, it might be worth a shot to try and help you manage your levels on your own. Same with folic acid, I don't absorb it for shit, but when I switch to a methylfolate supplement, my numbers start to climb*. If you have tried this and it's been ineffective, then I'm going to emphasize that you need to push your doctor to increase your number of shots to get your levels up because what you are living with is not an acceptable quality of life, and I am so sorry :(

Okay so here's the thing, they actually were from psychiatric testing, but I'm more making fun of an offhanded interpretation the psychiatrist I was seeing had said, her personal thoughts and observations that I'm completely disregarding lmao. The results themselves are pretty cut and dry, "x gene controls for y and has z result, but your copy is abc which means your body does things differently"
The actual importance of me finding out I have this (comt val/val) gene is that it shows that I'm hypermetabolizing dopamine, my brain basically just burns through the neurotransmitter that helps you feel happy, which in turn is gonna influence the kind of treatment I would receive for anything depression-related
Imo I found it to be helpful bc it did actually reveal that most of my chronic pain was the result of a genetic mutation as well (mthfr) and that's also something that can be treated. Funny little names attached to genes like I was posting about is just kind of a thing I'm betting probably happens so the info can be more appealing to people who don't really know the specifics about how the genes work, also clickbait and all that, but I personally got use out of the testing, so best of luck to you as well anon ✌

You know in your gut when something just doesn't feel right. I remember after I was suffering miscarriage after miscarriage the doctors couldn't find the physical reason I kept losing the babies. Everything was tested, and everything seemed fine and in good working order. However, on a deep level I felt like because I had worked so hard on the “spiritual reasons” my baby wasn't manifesting there still had to be a “physical reason” why I could only stay pregnant for up to 10 or 11 weeks. It wasn't until I moved states and found a more progressive doctor that she tested me for a gene mutation called MTHFR. Turned out that this was the reason I suffered recurring miscarriages. So I guess the moral of this story is to go with your gut, follow those hunches and keep seeking second and third opinions until your situation “feels right” again. The Answer

These past 8 years of my life I have felt like I’m suffocating. It feels like my body is lost at sea, with every wave my lungs slowly gaining more water, my body getting heavy by each day. Living with chronic pain is debilitating sucking the life right out of me. In the middle of a bad flare up I am stuck mentally and physically. Wishing that I could just “get better” or to “push past the pain.” Year after year with no answer. Doctor after doctor, pill after pill, test after test. At what point do you just give up? You don’t. You keep fighting till you find answers.
I once read an instagram post “Your body is trying to tell you something, listen to it. It’s not trying to fight against you, but for you.” Because of living with chronic inflammation I got into this mindset that my body is fighting against me. This body that God has giving me is failing in every area. As a Mama I want nothing more than to provide the very best for my daughter. Yet when a bad day hits me hard I can’t do anything but blame myself. Mentally I snap way to easily, sometimes physically I can’t even stand without excruciating pain shooting through my body. How do you even take care of another little soul when you can’t even take care of your own?
For years we thought all my inflammation was boiled down to certain foods. After having Serena I eliminated dairy, gluten, any processed foods. It helped calm the havoc in my body but still wasn’t making it go away completely. After a year I started removing any sugars as well (natural and unnatural) changed my skin care, my lotions, my deodorant, my drinks, everything. Again, it helped calm it down for me to have a couple days of sanity. But as soon as it went away something would happen and inflammation spiked. On top of this tormenting pain my anxiety has been debilitating.
I stumbled upon an awesome lady named Tavia who is a certified health, life and wellness coach. Her standpoint is to look at peoples overall health from a holistic and natural approach. The first thing she wanted me to get tested for was Lymes disease. She has put her Lymes disease in remission and is on a mission to help others find the real reason for their pain. The only time I have ever heard of it is through Avril Lavignes testimony. I thought to myself no way in heck do I have that! I’ve never been bitten by a tick. With less money in my pocket and a little blood later, I finally got my results.
Nine years of feeling lost at sea I finally have my answers. After countless nights of feeling like I’m mentally going insane. Truly questioning my own sanity, thinking that I’m just making this all up. I have been battling Lymes Disease. It spreads through any kind of bug bite. Typically people won’t even have a bulls eye rash when they get bitten. Going untreated for years can cause people arthritis and nervous system problems. I also have a gene mutation called MTHFR which doesn’t help my situation. Because of that gene not only is my body not able to properly detox but it can only soak up about 20% of the nutrients I need. The welts that I battle is my body trying to detox.
The next step is to see a specialist in Seattle that can help holistically heal my body from the inside out. With much fear of the unknown and still trying to process this, I would love for you to be a part of my ride. I step out of my comfort zone writing my story. Hoping that it can help someone understand that they can heal. That living with chronic pain is not something that we were created to do.
