I think GRRM did some irreparable damage to the fantasy genre because he was combining real history with fantasy and now people expect too much realism with fantasy.

No one has to be naked. You dont need incest. Your world can be whatever the fuck you want it to be. You do not have to follow true accurate history or rules. Its your world!!!!!
